Medical Description

Cromide Forte is a prescription medication used to manage abdominal pain in Irritable Bowel Syndrome (IBS). It is a combination of three medicines - Chlordiazepoxide (5mg) + Clidinium (2.5mg) + Dicycloverine/Dicyclomine (10mg). Irritable Bowel Syndrome (IBS) is a functional disorder characterised by abdominal discomfort or pain with alteration of bowel habits (diarrhoea or constipation) in the absence of any other causative agent. The exact cause of this syndrome is not known, but multiple factors like gut immunity, psychological stress, the kind of food consumed, and the brain-gut interaction are indicated in Irritable Bowel Syndrome (IBS). Affected people may experience abdominal bloating, crampy abdominal pain, diarrhoea or constipation. Clidinium and Dicyclomine are antispasmodics and Chlordiazepoxide is a benzodiazepine. The mode of action of Cromida Forte is through the combination of actions of these ingredients, which help relieve the symptoms of Irritable Bowel Syndrome (IBS). Directions for use of Cromida Forte should be strictly followed. The medication should be taken only after consulting a doctor. It should not be abruptly started or stopped. It is advised to take the tablet at the same time every day. Side effects of Cromida Forte include dry mouth, dizziness, constipation, urinary retention, blurring of vision, nausea, and sleepiness.

Contraindications

  • Do not take Cromida Forte tablets or take with caution:
  • If you are hypersensitive to Chlordiazepoxide, Clidinium, Dicyclomine or any other ingredients in this tablet.
  • If you have angle-closure glaucoma.
  • If you are under 6 or over 75.
  • If you are under 65 but have underlying comorbidities
  • If you have any heart disease; this medication can worsen cardiovascular conditions and is contraindicated to such persons.
  • If you have an enlarged prostate, suffer from urinary retention, or other obstructive urinary conditions.
  • If you have a lung disorder like respiratory failure, sleep apnea, or other conditions of respiratory insufficiency.
  • If you have liver dysfunction; this medicine is contraindicated to such persons.
  • If you have myasthenia gravis; it is contraindicated to such persons.
  • If you have ulcerative colitis, reflux esophagitis, and obstructive gastrointestinal conditions.

Directions for Use

Storage and disposal

Store the tablets in a cool and dry place away from direct light and moisture. Do not use the medicine after its date of expiration has passed. Keep the medicine away from children and pets. Do not flush the medication down the toilet. Dispose of any extra or expired medicines with biomedical waste.

Dosage

Overdose

In the event of overdose, the affected person may present with sedation, seizure, coma, increased heart rate, low blood pressure, dilated pupils, dry mouth, and difficulty in passing urine or stools. An overdose can even lead to severe respiratory depression, liver failure, kidney failure, coma, and death. In case of a suspected overdose, immediately rush the affected person to the nearest tertiary care centre for emergency management as this condition can be life-threatening.

Missed a Dose

If you miss a dose of this tablet, it is advised to take it as soon as possible. If it is almost time for the next dose, it is recommended to skip the missed dose and go back to the regular dosing schedule. Do not double the dose of the tablet.

Mode of Action

How Does It Work?

Clidinium and Dicyclomine are anticholinergic agents that act on the nerve endings of smooth muscles to cause muscle relaxation. There is a neurotransmitter called acetylcholine that acts as a chemical messenger between nerves and smooth muscles. The release of acetylcholine causes muscles to contract (spasm). Clidinium and Dicyclomine block the action of acetylcholine, leading to antispasmodic effects. These agents mainly act on the smooth muscles of the digestive tract, treating spasms of the intestine. Clidinium also decreases secretions in the gastrointestinal (GI) tract. Chlordiazepoxide belongs to a class of medications called benzodiazepines. It produces sedating and calming effects in users by acting on the brain to increase the effects of a neurotransmitter called GABA (gamma-aminobutyric acid).

Interactions

Interactions with other medicines

  • This medication should not be used with central nervous system (CNS) depressants like opioids; this can be life-threatening.
  • This medication should not be mixed with other benzodiazepines.
  • It should not be used with antacids; this decreases the absorption of Cromide Forte.
  • The concomitant use of this medication with anti-glaucoma agents decreases the efficacy of eye medicines, causing an increase in intraocular pressure.
  • Atropine-like substances can worsen the side effects of Cromide Forte like urinary retention, glaucoma, constipation, dry mouth, etc.
