Medical Description

Clobator belongs to the class benzodiazepines. Its active ingredient is Clobazam. It is used to treat anxiety. It is also used in the treatment of epilepsy, especially in nonresponsive cases to other medications. Epilepsy is a condition in which there are repeated episodes of convulsions or loss of consciousness. These convulsions may also be seen as a part of significant conditions such as Lennox Gestaut syndrome. Clobator is used in children over two years of age for the same. Anxiety is a condition characterised by increased restlessness and worry in daily activities that might be accompanied by trembling, excessive sweating or raised heart rate. The medication should be taken as recommended by your doctor. The tablet should be taken at the same time every day as this ensures the best action of the medication, decreasing the chances of relapse of symptoms significantly. It is also recommended to complete the entire course of the medication as prescribed by the treating physician. Abruptly stopping this medication can cause worsening symptoms.

Uses

  • As per CDSCO, Clobator is primarily used to manage the following conditions:
  • Acute and chronic anxiety states
  • As adjunctive therapy in patients with refractory epilepsy (not responding to other medications)

Contraindications

  • It is contraindicated in the following cases:
  • If there is a history of hypersensitivity to clobazam or any of the active ingredients in this medication
  • If the patient is less than two years of age
  • If the patient suffers from liver or kidney disease
  • If the patients have a history of substance misuse

Side effects

  • Side effects of Clobator include:
  • Fever
  • Constipation
  • Lethargy
  • Drowsiness (somnolence)
  • Excessive salivary secretion
  • Skin rashes

Pregnancy

There is inadequate data regarding the use of Clobator during pregnancy. However, it has the potential to cause damage to the foetus. Inform your doctor if you are pregnant or planning to conceive.

Driving

Driving or operating heavy machinery after taking a Clobator is not advisable as it can result in extreme sedation or drowsiness.

Other General Warnings

Talk to your doctor if
  • Inform your doctor if you take any other medicines as a dose adjustment may be required.
  • Inform your doctor if you notice any redness of the skin, peeling of the skin, or skin rash. This medication can cause severe skin reactions such as Stevens Johnson syndrome and toxic epidermal necrolysis. These require immediate discontinuation of the medicine after consulting a doctor.
  • Inform your doctor if you have any liver or kidney disorders.
  • Inform your doctor if you experience any sudden mood changes or low mood.

Directions for Use

  • Take the medicine as prescribed by the physician.
  • Take the medicine at the same time every day for optimal pharmacological effects of this medication.
  • Swallow this medication as a whole. Do not crush, chew or break this tablet.
  • Finish the entire course of this medication as prescribed by your doctor. Abrupt stoppage of the medication can cause withdrawal or side effects. Consult your doctor before stopping this medication.
  • Do not drive/operate heavy machinery while using this medication.
  • Do not drink alcohol while on this medication

Storage and disposal

Store in a cool and dry place away from direct light and moisture. Do not use the medicine after the date of expiration. Keep the tablets away from children and pets. Do not flush the medication down the toilet. Dispose of it with biomedical waste only.

Overdose

Symptoms of an overdose include difficulty breathing, confusion, weakness, extreme drowsiness/sedation, slow/shallow breathing, loss of consciousness, and slowed reflexes. In case of any accidental or suspected overdose, it is recommended to visit the nearest tertiary care centre for medical attention.

Missed a Dose

If you miss a dose of this tablet, it is advised to take it as soon as possible. If it is almost time for the next dose, it is recommended to skip the missed dose and go back to the regular schedule. Do not double the dose of the tablet.

Mode of Action

How Does It Work?

Clobator contains clobazam that belongs to a class of medications known as benzodiazepines. It increases the levels of a specific chemical messenger in the brain known as GABA (gamma amino butyric acid). By increasing its activity it relieves the symptoms of anxiety and epilepsy.

Interactions

Interactions with other medicines

  • This medicine should not be taken with opioids or other benzodiazepines as it can cause a severe drop in respiratory rate.
  • Clobator interacts with several medications that are metabolised by a different type of enzymes belonging to CYP class. This includes itraconazole, fluvoxamine, rifampicin, etc.

Interactions with food items

  • It does not have any specific interactions with any food.

Clobator can cause lethargy and drowsiness in some cases. In case of any increased discomfort, consult your doctor. Do not discontinue the tablet.

Clobator can interact with certain antifungal medications that use the same enzymes for metabolism. This means that a dose adjustment may be needed if two such medicines are taken together. It is always advisable to inform your doctor about any medication that you are taking.

Abruptly stopping Clobator tablets can result in anxiety, tremors and other symptoms. These are known as withdrawal symptoms. For this reason, the tablet should not be stopped without consulting a doctor. Visit your treating physician to understand what dose is recommended.
