Medical Description

Posoxil Suspension is a prescription antifungal medicine taken by mouth as a liquid. It contains posaconazole, an azole antifungal, and is used to treat oropharyngeal candidiasis, a yeast infection of the mouth and throat, including cases that have not responded to itraconazole or fluconazole. It is also used to prevent invasive Aspergillus and Candida infections in people with a severely weakened immune system. Posaconazole blocks the making of ergosterol, a key building block of the fungal cell membrane. It does this by inhibiting lanosterol 14-alpha-demethylase, the enzyme that converts lanosterol into ergosterol. Methylated sterol precursors then build up while ergosterol runs down, which weakens the structure and function of the fungal cell membrane. Posoxil Suspension should be taken during or immediately after a full meal, which greatly improves how much medicine the body absorbs. Do not take several medicines with Posoxil Suspension, including sirolimus, pimozide, quinidine, certain statins, and ergot alkaloids, because these combinations can cause serious harm. This medicine can prolong the heart's QT interval and may cause serious heart rhythm disturbances; it can also affect the liver and electrolyte levels. Tell your doctor about every medicine you take before starting treatment.

Uses

  • Posoxil Suspension is used in the management of:
  • Treatment of oropharyngeal candidiasis, including oropharyngeal candidiasis refractory to itraconazole and/or fluconazole.
  • Prophylaxis of invasive Aspergillus and Candida infections in patients 13 years of age and older who are at high risk of developing these infections due to being severely immunocompromised, such as haematopoietic stem cell transplant recipients with graft-versus-host disease, or those with haematologic malignancies and prolonged neutropenia from chemotherapy.
  • Zygomycosis in patients with disease refractory to other therapy, or patients who are intolerant of other therapy.

Contraindications

  • People with known hypersensitivity to posaconazole or other azole antifungal agents.
  • With sirolimus, because posaconazole markedly increases sirolimus blood levels and can cause sirolimus toxicity.
  • With CYP3A4 substrates that prolong the QT interval, such as pimozide and quinidine, because this can cause QT prolongation and a dangerous heart rhythm called torsades de pointes.
  • With HMG-CoA reductase inhibitors (statins) that are primarily broken down through CYP3A4, because this can lead to rhabdomyolysis, a serious breakdown of muscle.
  • With ergot alkaloids, because they can cause ergotism.

Talk to your doctor if
  • You are allergic to posaconazole or to any other azole antifungal medicine.
  • You take sirolimus, pimozide, quinidine, an ergot alkaloid, or a statin, as these must not be combined with this medicine.
  • You take ciclosporin or tacrolimus, as their doses will need to be reduced and their levels monitored.
  • You have a heart rhythm problem, a prolonged QT interval, or take any medicine that affects the heart rhythm.
  • You have an electrolyte disturbance, such as low potassium or magnesium.
  • You have liver problems, or develop yellowing of the skin or eyes, dark urine or unusual tiredness.
  • You have kidney problems.
  • You cannot eat a full meal, so your doctor can advise how to take each dose.
  • You are pregnant, think you may be pregnant, are planning a baby, or are breastfeeding.
  • You are considering this medicine for a child under 13 years of age.

Directions for Use

  • Take Posoxil Suspension exactly as your doctor has prescribed.
  • Take each dose during, or immediately after, a full meal. This greatly increases how much of the medicine your body absorbs.
  • If you cannot eat a full meal, take each dose with a liquid nutritional supplement or an acidic carbonated drink such as ginger ale.
  • Shake the bottle well before each dose.
  • Measure each dose with the calibrated dosing spoon supplied with the bottle, not with a household spoon.
  • Rinse the spoon with water after each use.
  • Do not stop treatment early, even if you feel better, unless your doctor tells you to.

Storage and disposal

  • Store Posoxil Suspension at around 25°C. Brief excursions between 15°C and 30°C are permitted.
  • Do not freeze it.
  • Keep the bottle tightly closed with the child-resistant cap, and keep the dosing spoon with it.
  • Keep it out of children's sight and reach.
  • Do not use it after the expiry date printed on the pack.
  • Do not throw medicines away with household waste or down the drain. Ask your pharmacist how to dispose of medicines you no longer need.

Quick Tips

  • Show your doctor or pharmacist a list of everything you take. This medicine has several combinations that must be avoided outright.
  • Report a fast or irregular heartbeat, fainting, or yellowing of the skin or eyes without waiting for your next appointment.
  • Finish the full course. Stopping early can cause the infection to return.
  • Take the oral suspension with food as directed, because inadequate food intake can reduce absorption.

Contact your doctor or the nearest emergency department immediately if you have taken more Posoxil Suspension than prescribed. Take the bottle with you so the treating team can see exactly what was taken. Posaconazole is not removed by haemodialysis.

Missed a Dose

If you miss a dose of Posoxil Suspension, take it as soon as you remember, unless it is almost time for your next dose. Do not take a double dose to make up for a missed one. If you are unsure, ask your doctor or pharmacist.

Interactions

Interactions with other medicines

  • Posoxil Suspension must not be taken with sirolimus, pimozide, quinidine, ergot alkaloids, or statins that are mainly broken down through CYP3A4.
  • Ciclosporin and tacrolimus: Posoxil Suspension increases their levels. Your doctor will reduce their dose and monitor the levels frequently.
  • Midazolam: levels can increase, so this combination needs careful monitoring.
  • Tell your doctor about any medicine that can affect the heart rhythm or prolong the QT interval.
  • Tell your doctor about all prescription medicines, over-the-counter medicines, supplements, and herbal products you are taking before and during treatment with Posoxil Suspension.
