Medical Description

Janumet 50/500mg Tablet is used alongside diet and exercise to control blood sugar in adults with type 2 diabetes when a single medicine is not enough. It contains sitagliptin and metformin, which lower glucose through two separate mechanisms.

Metformin reduces the glucose the liver releases and improves how well muscle responds to insulin. Sitagliptin blocks the enzyme DPP-4, protecting the gut hormones that prompt the pancreas to release insulin after a meal and reduce glucagon, so insulin is released only when glucose is actually rising.

Take it twice daily with or immediately after food, which substantially reduces the nausea, metallic taste and loose stools metformin can cause. Do not stop when your readings improve; that improvement is the treatment working.

Seek urgent medical attention for severe persistent upper abdominal pain spreading to the back, which may indicate pancreatitis, or for unusual muscle pain, rapid breathing and extreme tiredness, which may indicate the rare complication lactic acidosis.

Kidney function must be checked before starting and at least yearly, and the medicine stopped before contrast scans and surgery. Long-term metformin lowers vitamin B12, so levels should be monitored periodically.

Istamet 50/500mg tablet, Sitazor 50/500mg tablet and Glura M 500mg tablet are a few more tablets that also contain sitagliptin and metformin.

Frequently Asked Questions (FAQs)

Janumet 50/500mg Tablet is used to control blood sugar in adults with type 2 diabetes. It is prescribed alongside diet and exercise when one medicine alone is not keeping glucose in the target range, and it combines two treatments that lower blood sugar in different ways.
Metformin reduces the amount of glucose the liver releases and helps your muscles use insulin better. Sitagliptin works on a separate pathway, protecting the gut hormones that tell the pancreas to release insulin after eating. Because they act on different problems, the two together lower blood sugar more than either alone.
On its own it rarely does, because sitagliptin only stimulates insulin when glucose is actually rising and metformin does not force insulin release at all. The risk becomes real if you also take a sulfonylurea such as glimepiride, or insulin, in which case those doses often need reducing.
The metformin component commonly causes nausea, a metallic taste, loose stools and abdominal discomfort, and taking the tablet with or immediately after a meal reduces this considerably. Starting at a low dose and building up slowly also helps your gut adjust.
It is a rare but serious build-up of acid in the blood associated with metformin, occurring mainly when the kidneys cannot clear the drug. Warning signs are unusual muscle pain, deep or rapid breathing, severe tiredness, stomach pain with vomiting, and feeling very cold. Seek emergency care if these appear. Risk rises with dehydration, severe illness, heavy alcohol use and kidney impairment.
Yes, usually. Metformin must be stopped before any scan using iodinated contrast dye and before surgery under general anaesthesia, because a temporary drop in kidney function can allow it to accumulate. Your doctor will tell you when to stop and when it is safe to restart, normally at least 48 hours afterwards.
Long-term metformin reduces absorption of vitamin B12, and the resulting deficiency can cause tiredness, tingling and numbness in the hands and feet. This is often mistaken for diabetic nerve damage. Your doctor should check your B12 level periodically, particularly if you have been on treatment for several years.
Inflammation of the pancreas has been reported with sitagliptin and similar medicines. The warning sign is severe, persistent pain in the upper abdomen that may spread through to the back, often with vomiting. Stop the medicine and seek urgent medical attention if this occurs.
Kidney function determines whether you can take metformin at all. Your doctor will check it before starting and at least once a year, more often if it is already reduced. The dose is lowered when function declines, and the medicine is stopped altogether when it falls below a defined level.
Heavy or binge drinking should be avoided entirely. Alcohol increases the risk of lactic acidosis with metformin and can cause unpredictable low blood sugar, particularly if you drink without eating. If your doctor permits alcohol, keep it modest and always take it with food.
Mention kidney or liver disease, heart failure, a history of pancreatitis, gallstones, alcohol dependence, and any planned scan or surgery. Tell your doctor if you are pregnant, planning pregnancy or breastfeeding, and list all other medicines including diuretics and painkillers.
Take it as soon as you remember with food, unless your next dose is nearly due, in which case skip it. Never take two doses together, as this sharply increases the risk of stomach upset. Tell your doctor if you frequently forget.
