Medical Description

Dotin tablet is an antidepressant and is used to treat psychotic and neurotic depression, as well as an anxiety disorder. Treating depression unresponsive to previous treatments and chronic nerve pain in some conditions are some of the other uses of Dotin tablet. It consists of Dosulepin or Dothiepin as its active ingredient. The mode of action of the Dotin tablet increases serotonin (a chemical messenger in the brain). Dotin tablets also help stabilise the chemical substances and lower the symptoms. This medicine is not addictive, but weight gain, confusion, increased heart rate are some of the side effects of the Dotin tablet. It is important to follow your doctor's direction for using the Dotin tablet, as it has a relatively high toxicity in overdose. Do not stop using Dotin medicine suddenly, as it can cause withdrawal symptoms like sweating, tremors and cold. Dotin tablets are not recommended for children and adolescents below 18 years of age. The usage should be monitored as it can cause suicidal thoughts.

Uses

  • As per CDSCO, Dotin tablet is used for:
  • Treatment of depression.
  • Treatment of major depressive disorders, such as psychotic depression (severe depressive illness includes some form of psychosis such as hallucinations, delusions, etc.), neurotic depression (neurotic depression is a depression in an emotionally unstable person).
  • Treatment of depression unresponsive to other antidepressants.
  • Management of anxiety disorder.
  • Managing some types of nerve pain, such as fibromyalgia, and to prevent migraines.

Contraindications

  • If you have a known allergic reaction to any of its components.
  • If you are already using other antidepressants or antipsychotics. It can cause hallucinations, seizures, and different life-threatening side effects.
  • If you have any serious heart condition, like a heart attack, irregular heart rhythm
  • If you have a rare blood-related illness known as porphyria.
  • If you have previously been diagnosed with bipolar disorder or mania
  • If you have severe liver impairment
  • If you are currently expecting or planning for a pregnancy.
  • If you are breastfeeding.
  • If you have closed-angle glaucoma, a severe eye condition
  • If you notice any sudden mood changes and self-harming or suicidal thoughts.
  • Note - Dotin tablets are not recommended for children and adolescents younger than 18 years.

Missed a Dose

If you have missed a single dose of the Dotin Tablet, take it as soon as you recall. If you remember missing a dose of this medicine when it's almost the time to take the next Antidep tablet, skip the missed dose and take the next one. Do not take both the tablets together, as it can lead to an overdose. Try to talk to your doctor if you have any confusion about the dosage or you missed multiple doses simultaneously.

How Does It Work?

Dotin contains dosulepin as the main ingredient. It belongs to the tricyclic antidepressant class of medicines. It is believed that it works by stopping the reuptake of chemical messengers like norepinephrine and serotonin, neurotransmitters present at the end of the nerve cells in the brain. By preventing their reuptake (reabsorption), the effects of these messengers last longer. Thus, it helps in regulating mood swings and managing depression.

Interactions

Interactions with other medicines

  • Do not use Dotin Tablet if you are already using other antidepressants like MAO inhibitors such as linezolid, methylene blue and phenelzine, as they can lead to severe side effects like tremors, severe seizures, etc. It can sometimes even lead to death. At least a 14-day gap should be between taking these two medications.
  • Do not take other antidepressants with a similar composition like amitriptyline, imipramine, etc. They can exaggerate the side effects of Dotin medicine.
  • Do not use Dotin tablets with other medicines that cause sedation and drowsiness, like tranquillisers, opioid or narcotic pain relievers and muscle relaxants, without consulting a doctor.
  • If you use antibiotics, antifungal, migraine medicines, Dotin tablets can decrease their effects.
  • Dotin tablets can have interactions and side effects with stomach ulcers, antimalarial medicines, heart medicines, and blood thinners.

Interactions with food items

  • Try to avoid drinking grapefruit juice and cranberry juice, or eating those fruits, while you are taking Dotin Tablets, as they can increase the levels of this medicine in your blood. It can lead to unwanted adverse effects.
