Medical Description

Citalin Forte is a prescription medication. It is a combination of escitalopram (10mg) and clonazepam (0.5mg). Uses of Citalin Forte include the treatment of generalised anxiety disorders and depression. It is important to note that it has habit-forming potential due to its mode of action. Careful monitoring while on this medication is required and necessary. It is also important to note that this medication should not be abruptly stopped as it may cause withdrawal effects in users, which may be serious. The medication should be taken as recommended by the doctor. The tablet should preferably be taken at the same time everyday for its best effect. This also decreases the chances of relapse. It is also recommended to complete the entire course of the medication as prescribed by the treating physician and not abruptly stop taking the drug.

Contraindications

  • The concomitant use of this medication with MAOI (monoamine oxidase inhibitors) like Selegiline, rasagiline, isocarboxazid etc., is strictly contraindicated as it may cause serotonin syndrome. In case of use of these medications, Citalin Forte should not be taken until after two weeks of stopping their use and vice versa, as it can be potentially fatal.
  • Patients with severe liver disease must avoid taking Citalin Forte as the liver metabolises these medications. In severe liver disease, there may be decreased metabolisation resulting in increased concentrations of the drug and therefore excessive sedation and respiratory depression.
  • Clonazepam is contraindicated in patients with acute narrow-angle glaucoma.
  • Use cautiously in elderly patients.
  • Use cautiously in patients at risk of falling and in debilitated patients.

  • Sexual dysfunction (primarily decreased libido, absence of orgasms, and male ejaculatory delay)
  • Nausea
  • Increased sweating
  • Fatigue
  • Lethargy
  • Motor impairment (impaired coordination, impaired balance, dizziness)
  • Abrupt discontinuation of this medication can lead to withdrawal effects like dizziness, nausea, lethargy, irritability, anxiety, confusion, headache, sweating, tremors etc.
  • Long term use may cause the user to develop tolerance.

Alcohol consumption while on this medication is contraindicated. Both alcohol and Clonazepam are CNS depressants and may cause increased sedation and other adverse effects, including life-threatening effects like respiratory depression and coma.

  • Abrupt stoppage of Citalin Forte can cause withdrawal side effects. Consult your doctor before stopping this medication.
  • Driving/ use of heavy machinery is not advised while using this medication.
  • Do not mix alcohol with this medication, as it can be life-threatening.
  • This medication has habit-forming tendencies; hence it is recommended to follow the directions of your treating physician precisely and have a regular follow-up.

Storage and disposal

Store in a cool and dry place away from direct light and moisture. Do not use the medicine after the date of expiry. Keep the tablets away from children and pets. Do not flush the medication down the toilet. Dispose of it with biomedical waste only.

Overdose

The overdose of this medication can cause cardiac arrhythmias, sleepiness, double vision, respiratory depression, fall in BP, decreased heart rate, cardiac arrest, and coma. In case of a suspected overdose, visit an emergency room immediately for urgent intervention.

Missed a Dose

If you miss a dose of this tablet, it is advised to take it as soon as possible. If the duration missed is too long, and it is almost time for the next dose, it is recommended to skip the missed dose and go back to the regular schedule. Do not double the dose of the tablet.

Interactions

Interactions with other medicines

  • The medication levels are affected by other tablets that use the same enzymes for their metabolism. For example, carbamazepine, rifampin, and phenytoin.
  • This medication should not be used with a specific group of medications called MAO inhibitors as it may cause a potentially fatal side effect called serotonin syndrome. This can cause headaches, confusion, nausea, and dizziness.
  • Other medications contraindicated due to the possibility of inducing serotonin syndrome include- antidepressants, triptans, fentanyl, lithium, tramadol, tryptophan, buspirone, amphetamines.
  • It may cause an increased risk of bleeding when used with aspirin, NSAIDs (painkillers), and other medications which affect blood clotting.

Interactions with food items

  • There’s no known interaction of this tablet with food.

Delayed ejaculation and low sexual desire are common side effects of this medication. Discuss with your treating physician regarding this for other options and treatment. Do not abruptly stop this medication without the explicit advice of your doctor.
