Medical Description
Upadacol tablet contains upadacitinib as its active ingredient. It is a prescription medicine that belongs to a group called Janus kinase (JAK) inhibitors. JAK enzymes are proteins inside immune cells that pass along signals driving inflammation in several chronic conditions. By reducing the activity of these enzymes, Upadacol tablet lowers inflammation and helps control symptoms. It comes as a prolonged-release tablet available in three strengths (15 mg, 30 mg, and 45 mg), taken by mouth.
This medicine is used to treat seven different inflammatory conditions in adults, and in some cases in young people aged 12 years and older. It is used for rheumatoid arthritis, psoriatic arthritis, axial spondyloarthritis (including ankylosing spondylitis), and giant cell arteritis, all at 15 mg once daily. It is also used for atopic dermatitis (eczema) in adults and adolescents aged 12 years and older weighing at least 30 kg, at 15 mg or 30 mg once daily. For ulcerative colitis and Crohn's disease, it is started at a higher induction dose (45 mg once daily) for a number of weeks before moving to a lower maintenance dose.
Upadacol tablet is an oral medicine taken once daily at home, prescribed and supervised by a specialist doctor. It can be taken with or without food, and should be swallowed whole at the same time each day. Treatment is continued for as long as it keeps working and side effects remain manageable. Regular blood tests throughout treatment are an essential part of staying safe on this medicine.
Uses
- Rheumatoid arthritis (moderate to severe), used alone or with methotrexate after other medicines have not worked well enough
- Psoriatic arthritis (active), used alone or with methotrexate after other medicines have not worked well enough
- Axial spondyloarthritis, including non-radiographic axial spondyloarthritis and ankylosing spondylitis, after other medicines have not worked well enough
- Giant cell arteritis (inflammation of blood vessels in the head, neck, and arms)
- Ulcerative colitis (moderate to severe) in adults who did not respond well enough or did not tolerate previous therapy
- Crohn's disease (moderate to severe) in adults who did not respond well enough or did not tolerate previous therapy
Contraindications
- If you are allergic to upadacitinib or any of the other ingredients in this medicine, as a serious allergic reaction may occur
- If you have a severe active infection such as pneumonia or a serious skin infection
- If you have active tuberculosis (TB)
- If you have severe liver problems
- If you are pregnant, as this medicine must not be used during pregnancy

- Infections of the nose and throat (upper respiratory tract infections such as colds and sore throats)
- Acne

Other General Warnings
- You develop fever, feel unusually tired, notice unexplained wounds, or have dental problems, as Upadacol tablet can reduce the body's ability to fight infections and may worsen an existing infection; patients aged 65 and older are at a higher risk of infections, heart problems, and some types of cancer
- You develop a painful skin rash with blisters, as this may be shingles (herpes zoster), which this medicine can allow to reactivate; shingles needs early antiviral treatment to prevent it from becoming severe
- You have had tuberculosis in the past or have been in close contact with someone with TB, as your doctor will test you for TB before starting Upadacol tablet and may retest during treatment
- You have ever had hepatitis B or C, as these infections require careful monitoring before and during treatment
- You have previously had blood clots in the legs (deep vein thrombosis) or lungs (pulmonary embolism), or are at a higher risk of blood clots such as if you use hormonal contraceptives, have had recent major surgery, or have a blood clotting disorder; tell your doctor immediately if you develop swollen or painful legs, sudden breathlessness, or chest pain during treatment
- You develop sudden changes in your eyesight such as blurring or partial or complete loss of vision, as these may indicate a blocked blood vessel in the eye requiring emergency assessment
- You have had or currently have cancer, or smoke or have smoked in the past, as your doctor will assess whether this medicine is appropriate for you; regular skin examinations are recommended throughout treatment as non-melanoma skin cancers have been reported
- You have heart problems, or are aged 65 years or older, as your doctor will discuss whether Upadacol tablet is right for you in your specific situation
- You experience unexplained stomach pain especially with fever, nausea, and vomiting, as Upadacol tablet has been linked with a rare but serious complication called gastrointestinal perforation (a hole in the bowel)
- You have or have had diverticulitis, stomach or intestinal ulcers, or are taking anti-inflammatory medicines or strong pain medicines alongside Upadacol tablet, as the risk of bowel complications is increased
- You regularly see a whole tablet or tablet pieces in your stools, as this is a rare occurrence that should be reported to your doctor
- Before receiving any live vaccine during treatment, as live vaccines are not recommended while taking Upadacol tablet
Directions for Use
- Take Upadacol tablet exactly as your doctor tells you, once daily at around the same time each day
- Swallow the tablet whole with water; do not split, crush, chew, or break it before swallowing, as this changes how much medicine gets into your body
- Upadacol tablet can be taken with or without food
- Never stop taking Upadacol tablet unless your doctor tells you to stop, as your condition may flare without warning
Storage and disposal
- No special temperature conditions are required; store at room temperature.
- Store in the original blister pack to protect from moisture; do not remove a tablet from the blister until you are ready to take it.
- Keep out of reach of children and pets.

- Tell your doctor immediately if you develop sudden changes in your vision, as this may need urgent assessment
- Inform all your doctors, dentists, and emergency care providers that you are taking Upadacol tablet, as it affects the immune system and interacts with several medicines

Overdose
- If you take more Upadacol tablet than prescribed, contact your doctor or go to the nearest emergency department straight away. You may experience some of the side effects listed in the side effects section.
- Since Upadacol tablet is taken at home once daily, always keep it in its original packaging and never share it with anyone else.
Missed a Dose
- If you miss a dose of Upadacol tablet, take it as soon as you remember on the same day. If you forget your dose for an entire day, skip the missed dose entirely and take your usual dose the following day.
- Do not take double doses to make up for a missed one. If you are unsure what to do, contact your doctor or pharmacist for advice.

Interactions
Interactions with other medicines
- Avoid strong CYP3A4 inhibitors such as itraconazole, posaconazole, voriconazole, clarithromycin, and ketoconazole, as these can significantly raise the amount of Upadacol tablet in the blood and increase the risk of side effects; your doctor may reduce your dose if these medicines are necessary
- Avoid strong CYP3A4 inducers such as rifampicin (used for tuberculosis) and phenytoin (used for seizures), as these significantly reduce how much Upadacol tablet your body absorbs and make it less effective
- Medicines that affect your immune system such as azathioprine, 6-mercaptopurine, ciclosporin, and tacrolimus must be used with great caution alongside Upadacol tablet, as the combined effect on immunity can be excessive
- Non-steroidal anti-inflammatory medicines (NSAIDs), strong pain medicines (opioids), and corticosteroids can increase the risk of bowel perforation and diverticulitis when taken alongside Upadacol tablet; discuss with your doctor if you take any of these
- If you have diabetes, your anti-diabetic medicine dose may need to be reduced while on Upadacol tablet, as the medicine may affect blood sugar control
- Always tell your doctor about all medicines, supplements, vitamins, and herbal products you take before starting treatment
