Medical Description
Telma 40 tablet is an antihypertensive medication used to manage high blood pressure (hypertension) and reduce the risk of serious cardiovascular events such as heart attacks and strokes. Its active ingredient is telmisartan, a member of the angiotensin II receptor blocker (ARB) class of drugs.
The medication works by inhibiting the binding of angiotensin II, a natural body substance that constricts blood vessels and promotes the retention of water and salt. By blocking this action, Telma 40 helps relax and widen blood vessels, lowering overall pressure in the circulatory system and allowing the heart to pump blood more effectively throughout the body.
Before starting treatment, it is important to inform your doctor if you have a history of kidney disease, liver impairment, or heart failure. Patients with diabetes should be particularly cautious, as telmisartan can potentially lower blood glucose levels and may require an adjustment in antidiabetic medications. Telma 40 can increase blood potassium levels (hyperkalemia), the use of potassium-rich supplements or potassium-containing salt substitutes should be avoided.
Side effects may include dizziness, back pain, or flu-like symptoms. Because this medication can cause lightheadedness, especially when starting the treatment or increasing the dose, you should avoid driving or operating heavy machinery until you know how the medicine affects you. This medication is strictly contraindicated during pregnancy, as it can cause severe developmental harm or death to the unborn baby.
Telmikind 40, Tazloc 40, Telvas 40, Telista 40, and Eritel 40 are some other medicines that contain telmisartan as their main ingredient.

- If you are pregnant or may be pregnant, treatment should be discontinued if pregnancy is detected.
- If you have an obstruction or blockage in your bile duct (which carries bile from the liver to the gallbladder).
- If you have severe liver disease, liver failure, or kidney function problems.
- If you have diabetes or kidney impairment and you are treated with Aliskiren (to treat high blood pressure).

- You have kidney disease or have had a kidney transplant or liver disease.
- You are known to have narrowing or blockage of the blood vessels of one or both kidneys.
- You are having diarrhoea vomiting as this can cause dehydration and lead to low blood pressure. Consume plenty of water and fluids.
- You suffer from water and salt retention and an imbalance of various blood minerals (raised aldosterone levels).
- You have an increased level of potassium in your blood.
- You have diabetes, have a low blood pressure problem (hypotension) or have any heart disorder.
- You are taking blood pressure-lowering medicines such as ramipril or lisinopril.

- Since Telma 40 can naturally increase potassium levels in your blood, avoid using potassium-rich salt substitutes or supplements unless specifically advised by your healthcare provider.
- Drink plenty of water throughout the day, but avoid consuming large amounts all at once. Limit alcohol intake, as it can interfere with blood pressure regulation and increase the risk of dizziness.
- Aim for at least 30 minutes of moderate exercise daily, such as brisk walking or cycling. Maintaining a healthy weight reduces the overall workload on your heart.
- If you feel dizzy, especially when starting the medication, make a habit of standing up slowly from a sitting or lying position to prevent a sudden drop in blood pressure.
- Avoid smoking entirely, as it causes immediate and long-term damage to your blood vessels. Additionally, limit caffeine from coffee or energy drinks, which can cause temporary blood pressure spikes.
- Support your cardiovascular health by getting 7 to 8 hours of sleep each night and by practicing relaxation techniques such as deep breathing or meditation to manage stress.
- Keep a regular log of your home blood pressure readings to share with your doctor. If you experience severe diarrhea or vomiting, increase your fluid intake (such as ORS) to prevent dehydration and dangerously low blood pressure.

- If a medicine used for heart disorders like digoxin is used concomitantly with Telma 40, a dose adjustment may be required.
- Some medicines can affect the way Telma 40 works or Telma 40 itself can reduce the effectiveness of other medicines taken at the same time.
- To avoid any possible interactions, tell your doctor about all the medicines, supplements, or herbals you are taking.
