Medical Description

Trimium transcap is used to manage severe chronic obstructive pulmonary disease (COPD) in patients who continue to have symptoms despite regular treatment. It is intended for inhalation using a special device called a Rotahaler; do not swallow the capsule. Use it at the same time each day, inhale deeply, and rinse your mouth afterwards. Make sure not to use it more than once a day.

The three active ingredients in this medicine work together to help improve breathing. Tiotropium and formoterol help relax the airway muscles and open the lungs, while ciclesonide reduces inflammation in the airways. This combination helps reduce wheezing, breathlessness, and flare-ups when used regularly.

Let your doctor know if you have any medical conditions such as eye issues, prostate problems, heart disease, or diabetes. Certain medications like steroids, antidepressants, or enzyme blockers may interact with this inhaler. Always consult your doctor if you are pregnant, breastfeeding, or planning surgery. Do not use this inhaler to treat sudden breathing problems. Carry a rescue inhaler for emergencies.

Store the capsules in a cool, dry place away from sunlight and never freeze them. Do not wet or swallow the capsules. If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular schedule. In case of overdose, seek immediate medical attention.

Uses

Trimium transcap is used in patients with severe COPD and a history of repeated exacerbations, who have significant symptoms despite regular therapy with long-acting bronchodilators.

Mode of Action

How Does It Work?

  • Tiotropium bromide works by relaxing the muscles in the airways. It blocks a specific receptor in the lungs (called muscarinic receptors), which helps open up the air passages, making it easier to breathe. It is mainly used for the long-term treatment of chronic obstructive pulmonary disease (COPD) and asthma.
  • Formoterol fumarate works by relaxing the muscles in the airways, allowing you to breathe more easily. It is a long-acting bronchodilator that activates beta-2 receptors in the lungs, leading to widened air passages and improved airflow. It is used in the treatment of asthma and COPD to control symptoms such as wheezing and shortness of breath.
  • Ciclesonide is a corticosteroid that reduces inflammation in the airways. It works by decreasing swelling and irritation in the lungs, making it easier to breathe. It is used to help control asthma and prevent symptoms like wheezing and shortness of breath.

Interactions

Interactions with other medicines

  • Avoid taking other anticholinergic medicines with tiotropium.
  • Formoterol may interact with diuretics, steroids, antidepressants, or beta-blockers, increasing the risk of heart or potassium problems.
  • Strong enzyme blockers, such as ketoconazole, can increase ciclesonide levels and side effects; avoid unless necessary.
  • Individuals with kidney problems should be closely monitored when using tiotropium.
  • No dose adjustments are typically required for patients with liver or kidney problems when using ciclesonide or formoterol.
