Medical Description

Lymfuda tablet is a prescription anticancer medicine belonging to a group of chemotherapy medicines called antimetabolites. Doctors mainly use it to treat B-cell chronic lymphocytic leukaemia (CLL), especially in patients with sufficient bone marrow. However, it is started as a first-line treatment only in patients with advanced-stage CLL. Lymfuda tablet works by interfering with DNA production, which prevents rapidly growing cancer cells from multiplying and can ultimately cause them to die

Lymfuda tablet should be swallowed whole with water, at the same time each day. Your doctor will decide the exact dose and duration of your Lymfuda tablet treatment. Because it can affect your blood cell counts, your healthcare team will regularly monitor your blood tests before and during treatment. If tablet of Lymfuda tablet are accidentally opened or damaged, do not inhale or come into contact with the contents. If you come into contact with it, wash the affected area with water immediately.

Common side effects during treatment with Lymfuda tablet include fatigue, weakness, fever and chills, low blood cell counts (affecting red cells, white cells, and platelets), nausea, vomiting, loss of appetite, diarrhoea, and leg swelling. Serious side effects include severe or long-lasting decrease in blood cell counts, life-threatening autoimmune destruction of red blood cells, serious lung problems (including pneumonia and rare cases of lung failure), confusion or other nervous system effects, tumour lysis syndrome, and an increased risk of serious infections.

Before starting treatment with Lymfuda tablet, tell your doctor about all medicines, supplements, and herbal products you take, and if you have kidney problems. Your doctor may adjust the dose if your kidney function declines. Lymfuda tablet should not be used during pregnancy, as it can harm the unborn baby. Women and men with partners who are or could become pregnant should use effective contraception during treatment and for 6 months after the last dose. Avoid breastfeeding during treatment. Contact your healthcare team immediately in case of a missed dose or overdose, and if you notice symptoms such as fever, chills, unusual bleeding or bruising, signs of infection, shortness of breath, cough, breathing changes, pale skin, extreme fatigue, dark urine, confusion, severe headache, vision changes, numbness, weakness, agitation, or other new neurological symptoms. Lymfuda tablet should not be combined with pentostatin because it may cause serious lung damage. Regular blood tests are done to monitor for infections, blood cell counts, tumour lysis syndrome and other effects throughout treatment.

Uses

Doctors may prescribe Lymfuda tablet for the treatment of B-cell chronic lymphocytic leukaemia (CLL) in patients with sufficient bone marrow. It is started as a first-line treatment only in patients with advanced-stage CLL (Rai stages III/IV).

Contraindications

Lymfuda tablet should not be used if
  • You have a known severe hypersensitivity or systemic allergic reaction to fludarabine, or other inactive ingredients in the formulation.
  • You are pregnant, as it may harm the unborn baby.
  • You are breastfeeding. Breastfeeding should be avoided during treatment.
Avoid live vaccines during and after treatment unless your doctor advises otherwise

Side effects

  • Low white blood cell count, low red blood cell count, low platelet count, reduced bone marrow function, and autoimmune hemolytic anemia (a condition where the body attacks its own red blood cells).
  • Fever, chills, infections, malaise, fatigue, weakness, anorexia
  • Nausea and vomiting, diarrhea, inflammation of the inner lining of the mouth, gastrointestinal bleeding
  • Lung infection (pneumonia), shortness of breath, cough, serious lung problems such as severe breathing difficulty, bleeding in the lungs, lung scarring, and lung failure.
  • Weakness, agitation, confusion, visual disturbances, coma, peripheral neuropathy (including wrist-drop), progressive multifocal leukoencephalopathy
  • Swelling (usually in the legs) and fluid buildup around the heart
  • Bleeding and inflammation in the bladder, which may cause blood in the urine.
  • Skin rash
  • Tumor lysis syndrome (cancer cells breaking down quickly can upset the body’s chemical balance and affect kidney function)

Lymfuda tablet can cause fetal harm and should generally not be used during pregnancy unless the potential benefit clearly outweighs the risk. Women who can become pregnant and men with partners who can become pregnant should use effective contraception as advised by their doctor.

Use caution. Lymfuda tablet can cause side effects such as severe fatigue, confusion, weakness, or visual disturbances that may impair your alertness. Avoid driving or operating machinery if you feel unwell following treatment.

There is no specific established alcohol interaction with Lymfuda tablet. However, alcohol may worsen fatigue, dizziness, or other treatment-related symptoms. Discuss alcohol consumption with your doctor.

Talk to your doctor if
  • You experience fever, chills, persistent cough, or other signs of infection, as Lymfuda can weaken the immune system.
  • You notice unusual bruising, tiny red spots on the skin, or bleeding, which may indicate a low platelet count.
  • You experience confusion, severe headache, vision changes, numbness, weakness, agitation, or other new neurological symptoms immediately.
  • You notice unusual pale skin, extreme fatigue, or dark urine, which can be signs of autoimmune hemolytic anemia.
  • You have kidney problems. Your dose may need to be adjusted, and you’ll be monitored more closely.

Directions for Use

  • Lymfuda tablet can be taken with or without food.
  • Swallow the tablet whole with water. Do not open, break, or chew the tablet.
  • Lymfuda tablet should always be prescribed by your doctor; do not take it on your own.
  • Your doctor will decide the dosage and duration of treatment with Lymfuda tablet; follow their instructions exactly.

Storage and disposal

  • Store Lymfuda tablet in normal lighting conditions at 25°C (77°F); short term temperature variations are allowed between 15°C to 30°C (59°F to 86°F). Store it away from excess heat and moisture.
  • Keep this medication tightly closed, and out of reach of children and pets, and in a safe location.
  • Since Lymfuda tablet is a hazardous drug, ask your pharmacist or healthcare team how to safely dispose of unused or expired tablet. Do not discard them in household waste.

Quick Tips

  • Keep all scheduled blood test appointments so your doctor can monitor your blood cell counts.
  • Tell your healthcare providers that you are taking Lymfuda tablet before any medical procedure or blood transfusion.
  • If tablet of Lymfuda tablet are accidentally opened or damaged, avoid inhalation or contact of the contents with the skin or mucous membranes. If you come into contact, wash the affected area with soap and water immediately. If it gets in your eyes, wash for 15 minutes or more with gently flowing water. Contact your doctor if you have a skin reaction or if the drug gets in your eyes.
  • Since Lymfuda tablet can cause harm to an unborn baby, a pregnancy test may be performed in women of reproductive potential before starting treatment.
  • Women of reproductive potential should use effective contraception during treatment with Lymfuda tablet and for 6 months after the last dose.
  • Male patients with pregnant partners or female partners of reproductive potential should use barrier contraception (such as condoms) during treatment with Lymfuda tablet and for 6 months after the last dose.
  • High doses of Lymfuda may rarely cause serious nervous system effects, including confusion, vision loss, or coma. Your doctor will monitor you closely.
  • Lymfuda can severely lower red blood cells, white blood cells, and platelets. Regular blood tests will be done to monitor this.
  • In rare cases, Lymfuda may cause a serious reduction in all blood cell types (pancytopenia), which can take months to recover from.
  • Lymfuda may rarely cause the body’s immune system to attack its own red blood cells, which can be serious. Contact your doctor immediately if you notice unusual tiredness, paleness, or yellowing of the skin or eyes.
  • In patients with a large amount of cancer in the body, rapid breakdown of cancer cells can affect kidney function and body chemistry, especially early in treatment.
  • If you need a blood transfusion during treatment, your doctor will use specially treated (irradiated) blood to reduce the risk of a rare but serious reaction.

Dosage

Overdose

  • Contact your healthcare team or seek emergency medical help immediately in case of accidental overdose with Lymfuda tablet. Do not wait for symptoms to appear.
  • Overdose with Lymfuda tablet can cause central nervous system damage such as blindness and coma, severe reduction in blood cell counts, bone marrow suppression, risk of severe infections, and may lead to death.
  • There is no known antidote for Lymfuda tablet overdose. Your healthcare team will monitor you and provide supportive care to manage symptoms and blood cell counts.

Missed a Dose

Do not take a double dose of Lymfuda tablet in case of a missed dose. Contact your doctor or healthcare team as soon as possible for guidance.

How does this medicine work?

Lymfuda tablet contains fludarabine, which is a purine nucleotide antimetabolite agent. Inside cancer cells, fludarabine interferes with the enzymes and building blocks that abnormal lymphocytes need to copy their DNA and multiply. This prevents these cancer cells from growing and multiplying, eventually causing them to die.

Interactions

Interactions with other medicines

  • Avoid or discuss live vaccines with your doctor during and after treatment, as your severely suppressed immune system can increase the risk of serious infections.
  • Use of Lymfuda tablet with pentostatin is not recommended because of the risk of severe and potentially fatal pulmonary toxicity.
  • Tell your doctor about all prescription medicines, over-the-counter medicines, vitamins, and herbal supplements you take, especially medicines that weaken the immune system or reduce blood cell counts.

Interactions with food items

  • Take Lymfuda tablet at the same time every day, with or without food.
  • A high-fat meal may slightly delay the absorption of Lymfuda tablet.
  • Follow your doctor’s advice regarding your diet when taking Lymfuda tablet.
