Medical Description

Human Mixtard 30/70 100IU is used to control blood glucose in people with diabetes who require insulin. It contains human insulin in a fixed mixture, thirty per cent soluble insulin for rapid cover at mealtimes and seventy per cent isophane insulin for slower background cover through the day.

Insulin allows glucose to move from the bloodstream into muscle and fat cells for energy and stops the liver releasing more glucose. Replacing it lowers blood sugar and prevents the long-term damage to eyes, kidneys, nerves, heart and blood vessels that uncontrolled diabetes causes.

Roll and tip the vial gently until the liquid is uniformly cloudy before every dose, then inject under the skin about thirty minutes before a meal. Never inject into a vein, never shake the vial hard, and always rotate the injection site.

Low blood sugar is the main risk and can develop quickly, causing sweating, trembling, hunger, a pounding heart, confusion and, if untreated, loss of consciousness. Always carry fast-acting sugar and treat a low immediately with fifteen grams of glucose.

Store unopened vials in the refrigerator and never freeze them; keep the vial in use at room temperature away from heat and light. Discard any insulin that looks clumped, frosted or fails to mix evenly, and never skip a meal after injecting.

This medicine can be used during pregnancy if your doctor recommendsHowever, close monitoring of blood sugar levels is suggested as the dose requirements may change with each trimester of pregnancy.

Breast Feeding

Breast Feeding

You can use this while you are breastfeeding.

Driving

Driving

Insulin is known to cause low blood sugar levels and these can lead to impairment in your driving abilityAlways keep sugar or a bar of chocolate in your car with you while drivingIn case you experience any of the symptoms of low blood glucose levels like dizziness, drowsiness, visual disturbances and sweating, stop driving immediately and have sugar/ juice.

Alcohol

Alcohol

It is advisable not to consume alcohol when you are taking this medicine as it interferes with the way insulin worksConsumption of alcohol may lead to an increased risk of both low blood glucose level and high blood glucose level Avoid excessive use of alcohol while taking insulin.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You have recurrent low blood sugar levels
  • Be very careful about symptoms of low blood sugar like sweating, anxiety, fast heartbeat, headache, intense hunger, restlessness, dizziness, irritability. If you experience these, immediately have sugar (Do not consume artificial sweeteners). Eat something as soon as possible. Do not take insulin in such a scenario. If still the symptoms persists, seek emergency medical help
  • You have frequent fever and infections
  • You have diseases related to liver and kidney
  • You have thyroid disorders
  • You are switching to different insulin medicine brand
  • You have pain, itching and swelling at the injection site, you should continuously change the site of injection
  • Avoid doing heavy exercise
  • You are planning to change your diet preferences
  • You should always carry sugar candy with you
  • You are taking insulin but not taking adequate meals, your blood glucose can drop drastically.

Directions for Use

  • Take this injection as per the technique suggested by your doctor or nurse
  • This injection is to be taken under the skin of the thigh or upper arm or abdomen
  • Do not take this injection in vein or in muscles
  • Pull a skin, fold up to take this injection
  • Wait for at least 6 sec before removing the needle from the injection site
  • Always rotate the site of injection, do not take concurrent injections at the same site.

Storage and disposal

  • Store between 2°C to 8°C in the refrigerator
  • Do not freeze it
  • Keep it out of the reach of children
  • Do not open the carton of vials to protect it from light
  • Any unused portion should not be stored and should be appropriately discarded.

Quick Tips

  • Roll and tip the vial gently until the liquid is uniformly cloudy before every single dose; never shake it hard.
  • Inject about thirty minutes before your meal, and never skip or delay that meal afterwards.
  • Rotate injection sites and move at least a finger-width from the last spot to prevent lumps forming under the skin.
  • Keep using the same body area for the same time of day, since absorption differs between abdomen, thigh and arm.
  • Use a new needle every time and never share needles, syringes or vials with anyone.
  • Store unopened vials in the fridge, never in the freezer, and keep the in-use vial at room temperature away from heat and sunlight.
  • Discard insulin that is clumped, frosted, discoloured or will not mix evenly.
  • Always carry fast-acting sugar and treat a low immediately with about fifteen grams of glucose.
  • Check your glucose before driving and never drive when your reading is low.
  • Never drink alcohol on an empty stomach, and check your sugar before bed after drinking.
  • Do not stop insulin during illness; requirements usually rise, so check glucose more often and seek advice.
  • Never change your insulin brand, type or strength without medical supervision.

Dosage

Overdose

  • Excess insulin will lead to low blood sugar levels, called hypoglycemia
  • Symptoms like dizziness, fainting, sweating and tremors may be seen
  • If you have the blood glucometer with you, test immediately and confirm
  • Whether or not you test your levels, it is important to take some glucose water/juice/eat sugar or chocolates immediately and then rush to your doctor.

Missed a Dose

  • Missed dose may lead to high blood sugar levels which can cause symptoms like feeling thirsty, excessive urination, loss of appetite, drowsiness, fruity smell from breath, etc.
  • If you missed any dose of medicine take it as soon as you remember
  • If it's time for your next dose then skip the missed dose and continue with your regular dose schedule
  • Do not take a double dose of medicine to compensate the missed one as taking double dose can cause a drastic fall in blood glucose which can be dangerous.

How does this medicine work?

  • Insulin is crucial to allow entry of blood glucose into each cell of the body
  • Inside the cell, this glucose is used as fuel
  • In type 1 diabetes mellitus, the body fails to produce adequate insulin
  • This leads to glucose accumulating in the blood and cause high blood glucose
  • This medicine lowers blood sugars by stimulating glucose uptake by cells, tissue and muscles, especially by skeletal muscles and fat and by inhibiting glucose production by the liver.

Interactions

Interactions with other medicines

  • You may have symptoms of low blood sugar if you are taking other medicines for diabetes, depression (fluoxetine), medicines for fever and pain (salicylates), ramipril, lisinopril
  • You may have high blood sugar if you are taking oral contraceptives, thiazide, steroids, thyroxine, medicines used to treat severe diarrhoea and abnormal growth of body organs like octreotide, lanreotide
  • If you are taking a medicine pioglitazone to treat diabetes, be cautious, as some cases of heart attack and heart failure have been reported
  • Some medicines block your body's response to low blood sugar, so you may not experience symptoms like headache, hunger pangs, sweating, anxiety when your blood sugar drops. This can lead to a serious fall in blood sugar. Inform your doctor if you are on medicines like Betablockers (Metoprolol, Propranolol, Atenolol), Clonidine, Guanethidine and Reserpine.

Interactions with food items

  • Meal timing is part of the treatment with this insulin. Inject about thirty minutes before eating, and never inject and then skip, delay or greatly reduce the meal, as a low blood sugar will follow.
  • Keep the carbohydrate content of your meals reasonably consistent from day to day. A much smaller meal than usual for the same dose is a common cause of a hypo.
  • Always carry fast-acting sugar such as glucose tablets, sweets or juice. Treat a low with about fifteen grams, wait fifteen minutes, recheck, and then eat a longer-acting snack if your next meal is not due.
  • Alcohol should never be taken on an empty stomach. Eat alongside it, keep the quantity modest, and check your glucose before bed, as the risk of an overnight low is real.
  • Plan for exercise, which lowers glucose during and for many hours afterwards. You may need extra carbohydrate beforehand or a dose reduction; discuss this with your doctor.
  • During illness, do not stop your insulin even if you are eating little. Requirements usually rise with fever and infection. Check glucose more often, take small carbohydrate-containing drinks and contact your doctor for advice.

Frequently Asked Questions (FAQs)

Human Mixtard 30/70 100IU is used to control blood sugar in people with diabetes who need insulin. It is given as an injection under the skin and is suitable for type 1 diabetes and for type 2 diabetes when diet, exercise and tablets are no longer enough to keep glucose in the target range.
It describes the mixture inside the vial. Thirty per cent is soluble or regular insulin, which acts quickly to cover the meal you are about to eat, and seventy per cent is isophane insulin, which is suspended with protamine so it is absorbed slowly and provides background cover for many hours. One injection therefore does two jobs.
The isophane portion settles as a white sediment, leaving clear liquid above it. If you draw up without mixing, you may inject mostly fast insulin or mostly slow insulin, and your glucose will swing unpredictably. Roll the vial gently between your palms and tip it up and down until the liquid is evenly cloudy. Never shake it hard.
About thirty minutes before eating, so that the fast-acting portion is working as the food is absorbed. Injecting immediately before or after a meal allows glucose to rise first and then risks a low later. Never inject and then delay or skip the meal.
Repeatedly injecting the same spot causes the fat under the skin to thicken into lumps, a condition called lipohypertrophy. Insulin absorbed from these lumps is erratic and often reduced, which causes unexplained swings in glucose. Move at least one finger-width from the last site each time and rotate systematically.
The abdomen, the front and outer thigh, the buttocks and the outer upper arm are all used. Absorption is fastest from the abdomen and slowest from the thigh and buttock. Keep using the same general area for the same time of day so absorption stays predictable, but move the exact spot within it.
Sweating, trembling, hunger, a pounding heart, tingling around the lips, anxiety, difficulty concentrating, blurred vision, irritability and confusion. Treat immediately with fifteen grams of fast-acting sugar, such as three or four glucose tablets, a small glass of juice or four teaspoons of sugar in water, then recheck after fifteen minutes.
Because a low can develop quickly and, if it becomes severe, you may be unable to treat yourself. Always carry glucose tablets or sweets, check your glucose before driving, and never begin a long drive if your reading is low or falling.
Keep unopened vials in the refrigerator between two and eight degrees Celsius, never in the freezer and not against the back wall or cooling element. The vial in use can be kept at room temperature below thirty degrees, away from direct heat and sunlight, for the period stated on the pack. Never use insulin that has been frozen.
No. Discard it. After mixing, the suspension should look uniformly white and cloudy. If it stays clear, contains clumps or solid particles, or has a frosted appearance on the inside of the glass, the insulin has been damaged and will not work reliably.
Yes, considerably. Alcohol blocks the liver from releasing stored glucose, so it can cause a low blood sugar many hours later, often overnight. It also masks the warning signs and makes them harder for others to recognise. Never drink on an empty stomach, always eat with alcohol, and check your glucose before bed.
Mention kidney or liver disease, thyroid problems, heart failure, and any history of severe or unrecognised low blood sugars. Tell your doctor if you are pregnant, planning pregnancy or breastfeeding, if you take beta-blockers or steroids, and if your work, travel or exercise pattern is about to change.
