Medical Description

Fistent injection is a prescription hormonal medication that belongs to a class of drugs known as estrogen receptor antagonists. It is primarily used for the treatment of breast cancer. The medication binds to estrogen receptors, reduces their activity, and promotes their degradation, thereby reducing estrogen signalling in breast cancer cells.

This medicine is supplied as a sterile solution in pre-filled syringes intended for intramuscular administration. Because of its specific formulation and delivery requirements, a trained healthcare professional must administer it slowly and carefully, typically as two separate intramuscular injections into the buttocks (gluteal area) over time.

Treatment with Fistent injection requires medical supervision. Your doctor determines the recommended dose. Regular clinical monitoring is essential throughout therapy to assess treatment effectiveness and manage side effects.

Patients receiving this therapy should be fully informed about potential side effects, including injection site discomfort, hot flushes, fatigue, musculoskeletal pain, and possible liver enzyme elevations. Close communication with your healthcare team helps ensure that any symptoms are addressed promptly and treatment is maintained safely.

Uses

Fistent injection is used for:
  • Treatment of estrogen receptor-positive, locally advanced or metastatic breast cancer in postmenopausal women, including those who have not previously received endocrine therapy or whose disease has relapsed or progressed following endocrine therapy.
  • In combination with palbociclib for hormone receptor-positive, HER2-negative locally advanced or metastatic breast cancer in women who have received prior endocrine therapy.
  • In combination with capivasertib for hormone receptor-positive, HER2-negative locally advanced or metastatic breast cancer with specified PIK3CA, AKT1 or PTEN alterations following recurrence or progression on or after endocrine-based therapy.

Contraindications

  • If you have a known hypersensitivity or severe allergic reaction to fulvestrant or any of the inactive excipients, such as castor oil.
  • If you have severe liver impairment.
  • If you are pregnant or breastfeeding.
  • It should not be used in children and adolescents under 18 years, as safety and efficacy have not been established.

Side effects

Very common side effects (occurring frequently):
  • Injection site reactions (pain, inflammation, or mild bruising at the injection site)
  • Nausea and loss of appetite
  • Asthenia (weakness) and fatigue
  • Hot flushes
  • Musculoskeletal pain (joint pain, back pain, or muscle stiffness)
  • Diarrhea
Other reported side effects:
  • Urinary tract infections
  • Headache, dizziness
  • Elevated liver enzymes (AST, ALT, ALP)
  • Peripheral edema (fluid retention and swelling in the limbs)
  • Skin rash, itching, or allergic skin reactions
Serious but less common side effects:
  • Severe hypersensitivity reactions or anaphylaxis (rare)
  • Thromboembolic events (blood clots)
  • Liver impairment or hepatitis (rare)
  • Deep gluteal hematoma, sciatica, or nerve damage at the injection site if not administered carefully
  • Potential loss of bone mineral density

Precautions and Warnings

Pregnancy

Pregnancy

No. Fistent injection can cause serious harm to an unborn baby. It is indicated only for postmenopausal women, and effective contraception must be used if there is any potential for exposure.

Breast Feeding

Breast Feeding

No. Breastfeeding is strictly not recommended while receiving Fistent injection due to the potential risk of serious adverse effects on the nursing infant.

Driving

Driving

Use caution. Fistent injection can cause side effects such as fatigue or weakness that may impair your alertness. Avoid driving or operating machinery if you feel unwell following an injection.

Alcohol

Alcohol

It is best to limit or avoid alcohol during treatment; discuss alcohol consumption with your doctor, particularly if you have liver problems or experience treatment-related side effects.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You have pre-existing mild to moderate liver impairment, as careful monitoring is required.
  • You have severe kidney impairment.
  • You have a bleeding disorder or are taking anticoagulant medications, due to the intramuscular injection route.
  • You experience severe pain, swelling, or redness at the injection site that does not subside.
  • You notice signs of liver problems, such as yellowing of the skin or eyes, dark urine, or upper abdominal pain.
  • You have osteoporosis or other concerns regarding bone health.

Directions for Use

  • Fistent injection is always administered by a trained healthcare professional as a slow intramuscular injection into the buttocks.
  • Do not attempt to self-administer or prepare this medication at home.
  • Follow the prescribed monthly schedule closely as advised by your doctor.
  • Keep all scheduled clinic appointments for your injections and clinical check-ups.

Storage and disposal

  • Store the prefilled syringes in a refrigerator at 2-8°C in their original carton to protect them from light.
  • Do not freeze.
  • Keep out of the reach of children and pets.
  • Clinical staff manages proper disposal of syringes, needles, and medical packaging in accordance with hospital biomedical waste protocols.

Quick Tips

  • Keep all scheduled clinic appointments for your monthly intramuscular injections to ensure steady treatment efficacy.
  • Report any severe pain, swelling, or persistent lumps at the injection site to your nurse or doctor immediately.
  • Stay physically active and maintain a healthy diet to help manage weight changes, fatigue, and hot flushes.
  • Tell your healthcare professional if you have liver problems, kidney problems, bleeding disorders, low platelet counts, osteoporosis, or a history of blood clots.
  • Inform all your healthcare providers that you are taking this hormone therapy before undergoing any other medical evaluations.

Dosage

Overdose

If an overdose or incorrect administration interval occurs, medical supervision and supportive care should be initiated immediately by your healthcare provider. Because the medication is administered by a professional in a clinical setting, accidental overdose is extremely rare. Management focuses on symptomatic treatment and observation.

Missed a Dose

If you miss a scheduled injection appointment, contact your clinic as soon as possible to reschedule. Do not receive an additional dose without medical advice.

How does this medicine work?

Fistent injection is a selective estrogen receptor downregulator (SERD). It binds competitively to estrogen receptors and reduces estrogen receptor signalling, reducing estrogen's stimulatory effects on estrogen receptor-positive breast cancer cells.

Interactions

Interactions with other medicines

  • Inform your doctor about all prescription drugs, over-the-counter medicines, and herbal supplements you are taking, especially other hormone therapies or anticoagulants.
  • Use caution with anticoagulants (blood thinners), as intramuscular injections can carry an increased risk of bruising or bleeding at the injection site.

Interactions with food items

  • No specific food restrictions are required, as the medication is administered via injection and not taken by mouth.
  • Maintain a balanced diet rich in calcium and vitamin D to support bone health during long-term anti-estrogen therapy.
  • Avoid excessive alcohol consumption during treatment, as it can worsen fatigue, hot flushes, and place extra strain on your liver.

Frequently Asked Questions (FAQs)

Fulvestrant is formulated as an oil-based solution designed for slow, sustained release into the bloodstream. Giving it as a slow intramuscular injection into the gluteal muscle allows the medication to be absorbed steadily over several weeks.
No, a trained healthcare professional must administer this medication in a clinic or hospital setting because it requires specific slow intramuscular injection techniques to ensure patient safety and comfort.
Mild pain, tenderness, or bruising at the injection site is common. Inform your healthcare professional if pain, swelling, or other injection-site symptoms are severe or persistent.
Yes, because fulvestrant lowers estrogen activity in the body, hot flushes are a very common side effect. Dressing in layers and staying well-hydrated can help manage this symptom.
Your doctor will determine how long you should continue treatment based on your condition, how you respond to treatment, and how well you tolerate it.
