Scleroxil Injection

• Acute non-lymphocytic leukemia (ANLL) in adults
• Breast cancer
• Non-Hodgkin's lymphoma (NHL)

• If you have a known severe hypersensitivity or allergy to Scleroxil injection or any ingredient in the formulation.
• If you have baseline severe heart dysfunction or significantly reduced left ventricular ejection fraction (LVEF).
• During breastfeeding or pregnancy.
• If you have previously received mitoxantrone, anthracyclines, or other cardiotoxic medicines, your doctor will assess your cumulative exposure and cardiac risk before treatment.
• Severe bone marrow suppression may also require delaying or avoiding treatment, depending on the indication and your blood counts.

• Low white blood cell count (neutropenia/leukopenia), increasing susceptibility to infections
• Low red blood cell count (anemia), causing tiredness, pale skin, and weakness
• Low platelet count (thrombocytopenia), leading to easy bruising or bleeding
• Blue-green discoloration of the urine, and sometimes the sclera (whites of the eyes), or nails for a few hours after treatment
• Nausea, vomiting, diarrhea, and loss of appetite
• Hair thinning or temporary hair loss (alopecia)
• Menstrual changes or temporary/permanent absence of periods (amenorrhea)

• Mouth sores or inflammation (stomatitis/mucositis)
• Fatigue, generalized physical weakness, and fever
• Mild skin rash or itching
• Temporary liver enzyme abnormalities

• Heart Damage (Cardiotoxicity): Sudden shortness of breath, swelling of the ankles or feet, rapid weight gain, chest tightness, or irregular heartbeat. Mitoxantrone can cause reduced heart function and heart failure, which may occur during treatment or after treatment has ended.
• Secondary Cancers: Increased risk of developing treatment-related acute myeloid leukemia (t-AML) over time.
• Severe Infections: High fever with severe chills, sore throat, or burning during urination while white blood cell counts are low.
• Infusion Site Reactions (Extravasation): Stinging, burning, swelling, or redness near the IV insertion site during or after the infusion.

Other General Warnings

• You have a pre-existing heart condition, prior heart failure, or previous radiation therapy to the chest.
• You have previously received chemotherapy drugs belonging to the anthracycline family (such as doxorubicin or epirubicin).
• You have a history of liver disease or impaired hepatic function.
• You develop a high fever, persistent cough, or signs of an active infection.
• You notice severe swelling in your feet or legs, shortness of breath, or sudden weight gain.
• You are receiving treatment for MS, as your doctor will need to monitor your LVEF before each dose and track your cumulative lifetime dose.
• You have received a cumulative lifetime mitoxantrone dose, as further treatment may increase the risk of cardiotoxicity.

• Scleroxil injection is administered strictly by trained healthcare professionals in a hospital, clinic, or specialized infusion center via intravenous (IV) infusion.
• Infusions typically take several minutes, depending on the dose and treatment regimen your doctor prescribes. It is given on a specific cycle schedule.
• Your healthcare team will monitor the IV site closely during administration to prevent drug leakage into surrounding tissue (extravasation).
• Do not skip or delay your scheduled hospital infusion visits without consulting your doctor directly.

• Hospital staff store vials in a controlled clinical environment, protect them from light, and keep them at the manufacturer's specified room temperature or refrigerated.
• Reconstitution and dilution are performed exclusively by clinical staff in accordance with strict hazardous drug safety guidelines.
• Medical personnel dispose of all unused product, IV bags, tubing, and needles in designated biological chemotherapy disposal units.

• Do not be alarmed if your urine or the whites of your eyes turn a blue-green color for 1 to 2 days after treatment; this is a normal, temporary effect of the drug.
• Keep all scheduled appointments for blood tests and heart scans (ECHO or MUGA scans), so your doctor can monitor your health safely.
• For patients with MS, evaluate LVEF before each dose, and do not continue treatment if LVEF falls below the lower limit of normal or shows a clinically significant reduction.
• Report any sudden shortness of breath, leg swelling, rapid weight gain, or fever to your medical team immediately.
• Maintain good oral hygiene using a soft-bristled toothbrush to help prevent mouth sores.
• Both male and female patients must use effective birth control during treatment and for the specified period after the final dose as advised by your doctor.

Overdose

• An overdose is very unlikely because Scleroxil injection is calculated, prepared, and administered strictly by oncology specialists in a clinical environment.
• If an accidentally higher dose is given, the treatment will be stopped, and the healthcare team will closely monitor your blood cell counts, liver function, and heart status while providing supportive hospital care.

Missed a Dose

• If you miss your infusion appointment or need to reschedule, contact your clinic immediately.
• Your healthcare team will adjust your treatment calendar to ensure you complete your treatment cycle safely without receiving an extra dose to compensate for the missed appointment.

Interactions with other medicines

• Other anthracyclines / cardiotoxic medicines: Combined or prior use with drugs like doxorubicin, epirubicin, or trastuzumab may increase the risk of severe heart damage. Your doctor will consider your previous exposure to these medicines when assessing your overall cardiac risk.
• Myelosuppressive agents: When combined with other chemotherapy or immunosuppressive medications, they increase the risk of dangerously low blood counts and severe infections.
• Live vaccines: Avoid live vaccines during treatment unless your doctor specifically recommends them.
• Medicines used to prevent tumour lysis syndrome: In patients receiving chemotherapy for leukemia or other cancers with a high tumour burden, medicines such as allopurinol may be used to manage increased uric acid levels. Your doctor will decide whether such treatment is needed based on your risk of tumour lysis syndrome.
• Tell your doctor about all prescription and OTC medicines, vitamins, supplements, and herbal products you take before starting treatment.
