Medical Description

Qpen tablet is used to treat schizophrenia in adults and adolescents above 13 years of age and manic episodes of bipolar disorder in children and adolescents above 10 years of age. It is also used as a supportive medicine in treating major depressive disorder. This medicine contains quetiapine as its active ingredient, which is an atypical antipsychotic medicine. Qpen tablet binds to several chemical receptor sites in the brain and the nerves. It modulates the release of those chemicals in the brain that regulate mood, appetite, sleep and thoughts. It helps in improving negative as well as positive symptoms of schizophrenia. Qpen tablets are not advisable for treating psychotic symptoms in elderly people having dementia. It should be given with caution and in low doses to elderly people as there is an increased risk of falls, paralysis, and death.

Uses

  • As per USFDA, Qpen tablet is used in:
  • Schizophrenia above the age 13 years.
  • Maniac episode of bipolar disorder type 1 above 10 years.
  • Depressive episode of bipolar disorder in adults.
  • Major depressive disorder as a supportive treatment.

Contraindications

  • If you had an allergy to the active ingredient in Qpen in the past.
  • If you had an allergic reaction to any of the inactive ingredients in Qpen.
  • If you have been diagnosed with dementia and psychosis.

Pregnancy

Qpen in pregnancy should be avoided as it may cause risks to the unborn child and the newborn baby. The child may show signs of withdrawal if the mother has taken it during pregnancy. The child may develop restlessness, loose muscles, breathing difficulty and feeding problems. Talk to the doctor if you get pregnant while taking treatment with Qpen.

Breast Feeding

You should avoid taking Qpen tablets while breastfeeding. It may pass through the breast milk and cause poor feeding, lethargy and slow development in the babies. Discuss your treatment plan with the doctor if you are breastfeeding the baby.

Driving

Qpen tablets may cause sleepiness, slow reflexes, shakiness, dizziness and confusion. This may not be safe for driving. You should not drive till you are sure that the side effects of Qpen have subsided.

Alcohol

You should avoid taking alcohol with Qpen as it increases the side effects of Qpen and can lead to excessive sleepiness, tremors, vomiting, breathing problems, dizziness and loss of consciousness.

Talk to your doctor if
  • You have dementia with psychosis, and you are above 65 years of age.
  • You have a history of fits.
  • You have a history of strokes.
  • You have thoughts of hurting yourself.
  • You have heart, liver or kidney disease.

  • You have noticed unusual changes in your feelings and thoughts.
  • You have stiffness of the muscles, fever and sweating.
  • You have been taking other medicines for your condition.
  • You have been taking some vitamins or herbal preparations.

Directions for Use

  • Qpen should be swallowed whole with a glass of water.
  • It should be taken without food as fatty meals affect the absorption of Qpen tablets.
  • Follow the doctor's advice about the time and dose of the medicine.

Storage and disposal

Qpen tablets should be stored at room temperature up to 250 C. Store the medicine away from sunlight and moisture. Keep them where children and pets cannot reach them. Leftover tablets should be disposed of safely to avoid any accidental usage.

Overdose

The symptoms of overdose of Qpen tablet are excessive sleepiness, shakiness, aggressiveness, low blood pressure, fast heartbeat, tremors, confusion, inability to pass urine, severe breathing problems, coma and death. Go to the nearest hospital in case of an overdose.

Missed a Dose

Take the tablet only if you have missed a dose close to the actual time of that dose. Skip the missed dose if it is time for the next dose. Take the next dose at a regular time. Do not take two doses together, even if you have missed a dose.

Mode of Action

How Does It Work?

Qpen tablet acts by binding to the multiple chemical receptor sites in the brain. This slows down or stops the responses from these sites. These sites regulate the levels of several chemicals in the brain responsible for mood, memory, sleep and learning. By blocking these sites, Qpen balances the release of these chemicals in different areas of the brain and nerves making them less effective for causing psychotic symptoms. Qpen can bind to these sites long enough to control the symptoms. It doesn’t remain attached to them for a very long time, making it safer from relapse and some of the side effects.

Interactions

Interactions with other medicines

  • Medicines used to treat high blood pressure may get enhanced in their action due to the blood pressure-lowering effect of Qpen.
  • Medicines used in parkinsonism, such as levodopa, may become less effective with Qpen.
  • Medicines used to treat seizures such as carbamazepine and phenytoin reduce the levels of Qpen in the blood.
  • Medicines used to treat fungal infections such as itraconazole, ketoconazole increase the levels of Qpen in the blood.
  • Medicines such as belladonna, atropine, dicyclomine increase the risk of a blockage in the gut, difficulty in swallowing food and constipation when taken with Qpen.
  • Medicine used in treating tuberculosis, such as rifampicin, reduces the availability of Qpen in the blood.

Frequently Asked Questions (FAQs)

The time taken by Qpen tablets to produce effect varies for different symptoms. Usually, thoughts and hallucinations start improving within one or two weeks of the treatment. However, it may take up to three months to see the full effect of Qpen. Symptoms keep on improving as you continue your treatment with Qpen. Your doctor shall tell you about the length of the time you shall need to take the medicine.

You should avoid driving till you are sure that you do not have side effects of Qpen tablets such as sleepiness, headache or dizziness. Avoid any situation that may cause dehydration, like staying in the sun for long or in an overheated environment.

Your doctor may ask you to get blood and urine tests done regularly to check your sugar and fat levels, to see the counts of infection-fighting cells and blood-clotting cells. Doctors may advise you for eye check-ups also. Tell your doctor if you get easy bruising or bleeding from the gums.
