Medical Description

Flumusa Forte is a prescription medication used in the treatment of depression and associated anxiety. It is a combination of two medications, Fluoxetine (20 mg) and Alprazolam (0.5mg). Depression is a condition characterised by a pervasive low mood. People present with complaints like feeling low, feelings of hopelessness and dread, decreased motivation, and loss of interest in daily routine, which can be debilitating. Multiple brain pathways and chemical messengers (chiefly serotonin, norepinephrine and dopamine) are indicated in depression. Anti-depressant medications target these neurotransmitters and pathways to help relieve the symptoms of depression. Alprazolam is a benzodiazepine that produces calming, sedative effects on the brain, whereas Fluoxetine is a selective serotonin re-uptake inhibitor (SSRI) that regulates the chemical messenger - Serotonin. The medication should be taken as recommended by the doctor. It should not be abruptly started or stopped, as it may cause withdrawal effects. Taking the tablet at the same time every day is advisable for maximum efficacy.

Contraindications

  • It is contraindicated in patients with known fluoxetine, alprazolam or benzodiazepine hypersensitivity, or known allergies to any components in this medication.
  • Patients with abnormal heart rhythms should not use this medication.
  • It is contraindicated in patients with acute narrow-angle glaucoma.
  • This medication is contraindicated in patients with myasthenia gravis.
  • This medication should not be used in patients with conditions that cause respiratory insufficiency and obstructive sleep apnea syndrome.
  • One should use it with caution in case of any history of substance abuse.
  • It should be used with caution in those with liver and kidney disease.
  • There is a US black box warning for use in children and young adults, as it can increase suicidal ideation.

Driving

Storage and disposal

Store in a cool and dry place away from direct light and moisture. Do not use the medicine after the date of expiration. Keep the tablets away from children and pets. Do not flush the medication down the toilet. Dispose of it with biomedical waste only.

Overdose

Symptoms of overdose include: Fainting, somnolence (drowsiness), impairment of motor functions – dizziness, poor coordination, muscle weakness, loss of balance, low blood pressure (hypotension), shallow breathing (hypoventilation), coma, and even death. In a case of an overdose with this medication, respiration, blood pressure, and pulse rate require monitoring. Intravenous fluids are necessary, and an adequate airway should be maintained. In case of a suspected overdose, rush the patient to a tertiary care centre with ICU facilities for further evaluation and treatment.

Missed a Dose

If you miss a dose of this tablet, it is advised to take it as soon as possible. If the duration missed is too long, and it is almost time for the next dose, it is recommended to skip the missed dose and go back to the regular schedule. Do not double the dose of the tablet.

Mode of Action

How Does It Work?

Fluoxetine is a Selective Serotonin Re-uptake Inhibitor (SSRI). It works by acting on brain pathways involving the chemical substance Serotonin. It prevents the re-absorption of serotonin by nerves that cause serotonin accumulation. Serotonin plays a role in regulating mood, personality, and wakefulness in the central nervous system. It has minimal effect in preventing re-uptake of nor-epinephrine and dopamine. Alprazolam belongs to a class of medications called Benzodiazepines. These medicines primarily act on the brain pathways to increase the action of a chemical messenger, otherwise known as a neurotransmitter called GABA (gamma amino butyric acid). This results in sedation, calming effects, and muscle relaxation.

Interactions

Interactions with other medicines

  • This medication should not be mixed with alcohol, opioids, or any other CNS depressant, as it can cause worsening of side effects such as sedation, respiratory depression and coma and can be dangerous.
  • It should not be mixed with medications that cause the arrest of bleeding, such as aspirin, NSAIDs, heparin, warfarin, and other anti-coagulants as Fluoxetine in itself causes decreased activity of platelets. This may lead to bruising, nose bleeds, or internal bleeding.
  • This medicine should not be used concomitantly or within 2 weeks of using Monoamine oxidase inhibitors (MAOIs) like Selegiline, Rasagiline, etc. It may cause serotonin syndrome - a potentially threatening life issue.
  • The use of Fluoxetine with thioridazine, pimozide, or monoamine oxidase inhibitors, including linezolid or methylene blue, is also contraindicated.
  • Patients who are taking other medications that increase serotonin levels should be educated regarding the risks.
  • It should not be used with tablets that cause abnormal heart rhythms.
  • It should not be used with medications that affect the activities of the enzymes involved in metabolising the components of this medicine. It can otherwise cause increased side effects.
  • Inform your doctor regarding your current medications at the time of prescription of Flumusa Forte.
