Medical Description

Eliwel is a prescription medication. It contains the drug Amitriptyline, which belongs to a group of medications called tricyclic antidepressants. Apart from its use in the treatment of major depressive disorders, it is also commonly used to manage neuropathic pain and migraine in varying doses. Depression is a mood disorder resulting in hopelessness, helplessness, and worthlessness. It is thought to arise due to an imbalance in the brain, among several other causes. This imbalance could be due to a lower level of chemical substances in the brain called neurotransmitters. Amitriptyline acts on multiple brain pathways to increase levels of certain neurotransmitters, effectively decreasing the symptoms of depression. The medication should be taken as recommended by your treating physician. It is generally advisable to take the tablet at the same time every day as this ensures the best action of the medication, decreasing the chances of relapse of symptoms significantly. It is also recommended to complete the entire course of the medication as prescribed by the treating physician. Abruptly stopping this drug can cause worsening symptoms.

Contraindications

Hypersensitivity Acute recovery phase following heart attack/myocardial infarction (MI) Concurrent use with cisapride, which is used to treat gastric reflux Contraindicated within 14 days of monoamine oxidase inhibitors (MAOIs), like Selegiline and Rasagiline. Should not be used with medications that can cause QT prolongation Should not be used with medications that also increase serotonin levels Should be used with caution in patients with angle-closure glaucoma, urinary retention, seizures Should not be used immediately before surgery as mixing with anaesthetic agents may cause heart abnormalities like arrhythmias Contraindicated in children < 12 years of age Should be used cautiously in adolescents and young adults <24 years of age as it may increase suicidal ideation

Directions for Use

  • This medication should be taken as prescribed by the treating physician.
  • It is recommended to take this medication on a regular schedule, i.e., at the same time every day, for optimal pharmacological effects of this medication.
  • Swallow this medication whole. Do not crush, chew or break this tablet.
  • It is recommended to finish the entire course of this medication as prescribed by your treating doctor. Abrupt stoppage of the drug can cause withdrawal side effects. Consult your doctor before stopping this medication.
  • This medication may require 2-4 weeks for onset of action. Do not increase the dose without explicit advice from your treating physician.
  • Driving/ use of heavy machinery is not advised while using this medication.

Storage and disposal

Store in a cool and dry place away from direct light and moisture. Do not use the medicine after expiration. Keep the tablets away from children and pets. Do not flush the medication down the toilet. Dispose of it with biomedical waste only.

Dosage

Overdose

  • Amitriptyline toxicity is seen in doses over 5 mg/kg. Amitriptyline toxicity can be severe and even fatal. In the event of an overdose, the patient may present with sedation, seizure, coma, abnormal heart rhythm, dilated pupils, dry mouth. In case of a suspected overdose, immediately rush the patient to the nearest tertiary care hospital for emergency management as it is life-threatening. Do not consume the medication in a dose higher than prescribed by the doctor.

Missed a Dose

If you miss a dose of this tablet, it is advised to take it as soon as possible. If the duration missed is too long, and it is almost time for the next dose, it is recommended to skip the missed dose and go back to the regular schedule. Do not double the dose of the tablet.

Mode of Action

How Does It Work?

Amitriptyline is a tricyclic anti-depressant (TCA). It acts on certain chemical messengers in the brain that convey signals along nerve pathways, otherwise known as neurotransmitters. The neurotransmitters involved here are serotonin and norepinephrine. After their action, these chemicals are reabsorbed by the nerves, a process known as reuptake. Amitriptyline blocks this reuptake process and results in a higher level of the drug. Chronic use can produce long-term changes in this pathway. In addition, it also has sedative properties. When used for long, there is an increase in the levels of BDNF (brain-derived neurotrophic factor). BDNF plays an important role in causing depression. Higher levels of BDNF can reduce symptoms of depression. The action of this drug on serotonin and nor-epinephrine is also indicated in the management of neuropathic pain and migraine. It is important to note that Eliwel as an anti-depressant requires about 2-4 weeks of continued use for the optimal onset of action to begin.

Interactions

Interactions with other medicines

  • This medication causes prolongation of QTc interval and may result in cardiac arrhythmias. Do not take it with astemizole, cisapride, disopyramide, ibutilide, indapamide, pentamidine, pimozide, procainamide, quinidine, sotalol, terfenadine.
  • This medication should not be used with MAOIs (Monoamine oxidase inhibitors) or within 14 days of the use of MAOIs and vice versa. Examples of MAOIs include Selegiline, Rasagiline.
  • This medication should not be used with other drugs known to increase serotonin levels, like isocarboxazid, phenelzine, procarbazine, safinamide, selegiline, tranylcypromine, sertraline, as it may cause Serotonin syndrome, a potentially fatal condition.
  • This medication should not be used with alcohol or other CNS depressants.
  • This medication should not be mixed with opioid medications or benzodiazepines.
  • Amitriptyline gets metabolised through CYP3A4. Several drugs alter the activity of CYP3A4, and thus dose should be cautiously regulated, as well as the entire patient medication regimen checked for CYP3A4 inducers and inhibitors.

Frequently Asked Questions (FAQs)

Amitriptyline must be used for at least three months following management of presenting symptoms to prevent depression. Sudden stoppage of the drug can significantly cause a higher risk of your symptoms returning, as well as produce withdrawal symptoms. If you want to stop the drug, your doctor will slowly taper the dose and stop it. Do not stop it on your own.

This medication should not be mixed with alcohol as it can cause increased side effects.
