Awiqli injection is a long-acting human insulin analog formulation. Its active ingredient is insulin icodec, which is an insulin analog. Awiqli injection is designed to bind reversibly to albumin in the bloodstream, creating a circulating depot that slowly and continuously releases insulin. This allows for effective management of background blood sugar levels over an entire week from just a single injection.
This medication is used as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It is suitable for both insulin-naive patients starting insulin therapy and for those transitioning from an existing daily basal insulin regimen.
Awiqli injection is an injectable medication that can be administered at home, but it must be initiated and supervised by a healthcare provider experienced in diabetes management. Due to its specialized once-weekly schedule and high concentration, proper clinical training on dose setting using the prefilled pen is required before starting treatment. Ongoing use is intended for long-term glycemic control, continuing as long as clinical benefits are maintained and side effects remain manageable.

Contraindications
- Awiqli injection should not be used:
- During active episodes of hypoglycemia (low blood sugar).
- In patients with a known allergy or severe allergy to insulin icodec, or any of the excipients.
Side effects
- Very Common Side Effects: Hypoglycemia (low blood sugar), which can cause shakiness, dizziness, sweating, fast heartbeat, hunger, confusion, or headache.
- Common Side Effects: Injection site reactions (pain, redness, bruising), lipodystrophy (skin pitting or thickening at the injection site), weight gain, rash, itching (pruritus), and mild fluid retention (peripheral edema).
- Serious Side Effects Requiring Prompt Medical Attention: Severe Level 3 hypoglycemia (resulting in seizures or loss of consciousness requiring third-party help); life-threatening drops in blood potassium (hypokalemia), causing muscle weakness or heart arrhythmias; severe whole-body allergic reactions (anaphylaxis); and new or worsening congestive heart failure.

Other General Warnings
- You experience persistent signs of low blood sugar (sweating, tremors, confusion)—treat with oral sugar immediately.
- You notice sudden shortness of breath, unexpected weight gain, or rapid swelling of your ankles and feet, as these are signs of fluid retention and congestive heart failure when combining insulin with thiazolidinediones (TZDs).
- You develop a severe physical illness, high fever, or face an upcoming surgery, as these stress events will alter your overall metabolic demands.
- You have an established history of advanced kidney or liver disease, which elevates your baseline risk for prolonged hypoglycemic episodes.
- You need to change your scheduled injection day, which is only permitted if at least 4 days have passed since your prior dose.
- Awiqli prefilled pens must never be shared between patients, even if the needle is changed, due to the severe risk of transmitting blood-borne pathogens.
- Patients with longstanding diabetes, nerve disease, or those on beta-blockers may have reduced symptomatic awareness (hypoglycemia unawareness) and require increased monitoring frequency.
Directions for Use
- Inject Awiqli subcutaneously once weekly on any day of the week, but always on the same day each week.
- Administer the injection into the thigh, upper arm, or abdomen.
- Rotate injection sites within the chosen region with each weekly dose to reduce the risk of lipodystrophy.
- Do not inject into skin areas that have pits, lumps, thickness, tenderness, bruising, or scars.
- Always attach a new, sterile disposable needle before each injection; discard the needle immediately after use.
- Check the flow (prime the pen) before every injection by dialing to 10 units and holding the dose button with the needle pointing up until a drop of insulin appears.
- Dial your prescribed dose (delivered in 10-unit increments). Fully insert the needle, press and hold the injection button until the counter reads 0, and count slowly from 1 to 6 before withdrawing the needle.
- Strict Restrictions: Never inject Awiqli intravenously or intramuscularly. Do not use in an insulin infusion pump.
- Do not use a syringe to remove solution from the pen to avoid dosing errors or overdose. Do not use if the pen is damaged, not stored correctly, or if there are any visible particles (solution should be clear & colourless).
- Always discuss with your physician before switching from ongoing insulin therapy to Awiqli, as dose adjustment may be required along with close glucose monitoring.
Storage and disposal
- Unopened Pens: Store in a clean refrigerator between 36°F to 46°F (2°C to 8°C) until the expiration date. Do not freeze, and do not store near the cooling element.
- In-Use Pens: Store in the refrigerator or at room temperature below 86°F (30°C) for up to 12 weeks. Keep the pen cap on to protect the solution from direct light.

- Always verify the prefilled pen's label before every injection to avoid dangerous mix-ups with daily insulins or non-insulin injectables.
- Never store your pen with the needle attached, as this can lead to contamination, infection, or needle blockages.
- Always carry a fast-acting sugar source (such as glucose tablets or fruit juice) to treat unexpected low blood sugar.
- If you have a visual impairment, do not rely on the pen's audible clicks; always have a trained person with normal vision assist you in setting the dose.
Dosage
Overdose
- An excess of insulin relative to food intake or physical activity can cause severe, prolonged, and life-threatening hypoglycemia and hypokalemia.
- Mild episodes are managed with fast-acting oral carbohydrates (such as juice or candy). Severe episodes involving seizures or coma require immediate emergency administration of glucagon or concentrated intravenous glucose.
- Because Awiqli injection remains active in the body for a full week, prolonged clinical monitoring and extra carbohydrate intake are necessary even after initial recovery.
Missed a Dose
- If 4 days or less have passed since your scheduled day, inject the missed dose immediately. You must then reset your weekly dosing day to match the day this makeup dose was taken.
- If more than 4 days have passed, skip the missed dose completely and take your next regular dose on your standard scheduled day. Never inject a double dose.

Interactions with other medicines
- Increased Hypoglycemia Risk: Co-administration with other antidiabetic agents, GLP-1 receptor agonists, SGLT-2 inhibitors, DPP-4 inhibitors, ACE inhibitors, angiotensin II receptor blockers (ARBs), fibrates, fluoxetine, MAOIs, salicylates, and sulfonamide antibiotics increases the risk of hypoglycemia and may require dose reductions of Awiqli injection.
- Decreased Insulin Efficacy: Corticosteroids, diuretics, atypical antipsychotics (e.g., olanzapine, clozapine), estrogens, oral contraceptives, thyroid hormones, niacin, and protease inhibitors can reduce the efficacy of Awiqli injection, potentially requiring an increased insulin dose.
- Symptom Masking: Beta-blockers, clonidine, guanethidine, and reserpine can blunt or entirely mask the classic warning signs of low blood sugar, such as a rapid heart rate or palpitations.
Interactions with food items
- Awiqli injection can be taken comfortably with or without food, as standard meal timings do not change its weekly absorption pattern.
- Avoid sudden, extreme dietary changes or prolonged fasting, as the week-long duration of action means background insulin levels cannot be quickly adjusted to accommodate skipped meals.
