Medical Description

Aldactone 100mg Tablet is used to clear excess fluid from the body and to treat heart failure. It is prescribed for swelling caused by heart failure, liver cirrhosis or kidney disease, for resistant high blood pressure, and for overproduction of the hormone aldosterone by the adrenal glands.

It contains spironolactone, which blocks aldosterone, the hormone that normally tells the kidneys to retain salt and water and to excrete potassium. Blocking it makes the body lose salt and water while holding on to potassium, which is why it is called a potassium-sparing diuretic.

Take it in the morning with food, as evening doses will disturb your sleep with trips to the bathroom. The effect builds gradually over several days to two weeks, so do not increase the dose yourself if nothing seems to happen at first.

Breast enlargement and tenderness in men and menstrual changes in women occur because spironolactone also blocks hormone receptors. Blood potassium can rise, so seek prompt medical advice if you develop muscle weakness, numbness, tingling or an irregular heartbeat.

Do not use potassium supplements or potassium-based salt substitutes without medical advice. Attend your blood tests for potassium and kidney function, and tell your doctor about kidney disease, Addison's disease, or any vomiting and diarrhoea.

Uses

  • Aldactone Tablet is used in the treatment of oedema (fluid retention) in cases of congestive heart failure, liver and kidney dysfunction.
  • It helps in treating or preventing low levels of potassium in the blood (hypokalemia) and high blood pressure.
  • It is also used in managing primary aldosteronism (a condition that arises due to extra fluid in the body due to excessive aldosterone hormone).

Contraindications

  • If you are allergic to spironolactone or any other ingredient of Aldactone Tablet.
  • If you are unable to pass urine.
  • If you have Addison’s disease (symptoms are stomach pain, weakness, weight loss or low blood pressure).
  • If you are already taking any potassium supplements.
  • If you are diagnosed with high potassium levels in the blood or are already taking a water pill (like Eplerenone) that retains potassium.
  • If you are pregnant or you think you may be pregnant or are breastfeeding.

Missed a Dose

  • Missing a dose of Aldactone Tablet may reduce the effectiveness of the treatment and may lead to treatment failure. Hence, it is better to follow the prescribed schedule for optimal results.
  • If you have missed taking any dose of this medicine, take it as soon as you remember, if it is time for your next dose, skip the missed dose and continue with your regular dosing schedule.
  • Do not take a double dose of this medicine to compensate for the missed one.

How does this medicine work?

  • Aldactone 100mg Tablet contains spironolactone, which blocks the receptor that the hormone aldosterone normally acts on in the kidney. Aldosterone instructs the kidney to reabsorb sodium and water back into the body and to push potassium out into the urine.
  • By occupying that receptor, spironolactone reverses these instructions. Sodium and water pass out in the urine instead of being retained, which reduces swelling and lowers the volume and pressure of blood in the circulation, while potassium is kept in the body rather than lost.
  • In heart failure, aldosterone also drives scarring and stiffening of the heart muscle and blood vessels. Blocking it reduces this damaging process, which is why spironolactone improves outcomes in heart failure beyond simply removing fluid.

  • Tell your healthcare provider about all the medicines you take, including prescription and over-the-counter medicines, vitamins and herbal supplements. Aldactone Tablet may affect the way other medicines work and other medicines may affect how Aldactone Tablet works.
  • Especially, if you are taking potassium supplements, medicines used for heart-related disorders like digoxin or other water pills, antibacterial like trimethoprim, etc.
  • Aldactone Tablet, when taken along with blood-pressure-lowering medicines like Captopril and lisinopril, may lower the blood pressure excessively and you may feel giddy, thus should be used with caution.
  • Concomitant use of Aldactone Tablet along with blood thinners like Heparin, antibiotics like Cotrimoxazole, painkillers (NSAID's) like Indomethacin, etc.(NSAID's), can cause an increase in the blood potassium levels.

  • Aldactone Tablet can interact with high-potassium fruits and vegetables such as papaya, bananas, kiwis, oranges, cantaloupe, prune juice, honeydew melons, tomatoes, white and sweet potatoes, asparagus, pumpkin and Brussels sprouts.
