Medical Description

Telvas 3D Tablet is used to treat high blood pressure in adults whose readings are not adequately controlled by two medicines. It contains telmisartan, amlodipine and hydrochlorothiazide, which lower blood pressure by three distinct mechanisms.

Telmisartan blocks the receptor for angiotensin II, a hormone that tightens blood vessels and drives salt and water retention. Amlodipine blocks calcium entry into artery wall muscle so the vessels relax and widen. Hydrochlorothiazide makes the kidney excrete extra salt and water, reducing circulating volume.

Take it once daily in the morning, with or without food, since the diuretic will otherwise disturb your sleep. Avoid grapefruit, which raises amlodipine levels. Do not stop when readings improve.

Ankle swelling, flushing and headache come from vessel widening, while dizziness on standing reflects the combined pressure drop. Because two components move potassium in opposite directions, levels are checked by blood test.

This medicine must not be used in pregnancy. Hydrochlorothiazide also makes skin far more sensitive to sunlight and has been linked with certain skin cancers, so use sun protection and have new or changing lesions checked.

Contraindications

  • If you are allergic to Amlodipine, Telmisartan, Hydrochlorothiazide or any other ingredients of Telvas 3d Tablet.
  • If you have low blood pressure.
  • If you have problems with your liver or kidney.
  • If you suffer from any heart conditions like shock, irregular heartbeats, heart attack, low pulse or heart output, etc.
  • If you have diabetes or some kidney issues and are taking medicines containing aliskiren to treat your high blood pressure.
  • If you are pregnant for more than 3 months.
  • If you have low potassium, low sodium, high calcium or high uric acid levels in your blood.

  • You are following a low-salt diet.
  • You are experiencing diarrhoea and vomiting.
  • You have heart disease.
  • You suffer from asthma or other allergies.
  • You have issues with your kidneys or liver.
  • You are living with diabetes.
  • You recently had a heart attack.
  • You have difficulties with your adrenal glands.
  • You are afflicted by an autoimmune condition called lupus.
  • Your doctor has informed you that you have elevated potassium levels in your blood.
  • You experience severe diarrhoea that leads to weight loss or eye pain, or diminished vision.
  • You are undergoing treatment involving high doses of water tablets (furosemide, hydrochlorothiazide).
  • You have skin cancer or exhibit any skin reactions (skin rash, redness, sunburn) when exposed to sunlight.
  • You are taking other medications to manage high blood pressure, such as Ramipril, Lisinopril, Aliskiren, etc.
  • Telvas 3d Tablet is not recommended for patients under the age of 18 years.

Directions for Use

Telvas 3d Tablet should be taken as directed by your doctor. Swallow it whole with a glass of water. Do not cut, break or chew the medicine. It can be taken with or without food. It would be best if you took it at a fixed time for optimal results. Do not consume it more than that prescribed by your doctor.

Storage and disposal

Telvas 3d Tablet should be stored at room temperature, protected from moisture and heat. Keep them out of the reach and sight of children and pets.

Missed a Dose

In case you’ve missed any dose, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose. Avoid taking a double dose to compensate for the missed one.

How does this medicine work?

  • Telvas 3D Tablet lowers blood pressure through three separate mechanisms, which is why it can control readings that two medicines could not.
  • Telmisartan occupies the receptor that angiotensin II acts on. That hormone tightens the muscle of blood vessel walls and triggers aldosterone release, which makes the kidney retain salt and water. Blocking the receptor prevents both.
  • Amlodipine blocks L-type calcium channels in artery wall muscle. Since calcium entry is what causes that muscle to contract, blocking it relaxes and widens the arteries, lowering the resistance the heart pumps against.
  • Hydrochlorothiazide acts in the distal tubule of the kidney, blocking the transporter that reabsorbs sodium and chloride. The retained salt draws water into the urine, reducing the volume of blood in circulation.
  • The three also offset each other's drawbacks. A thiazide alone activates the hormone system telmisartan blocks; amlodipine alone causes ankle swelling that telmisartan and the diuretic partly relieve; and telmisartan raises potassium while the thiazide lowers it. Combining them therefore gives a larger and better-tolerated effect than escalating any single medicine.

Interactions with other medicines

  • Telvas 3d Tablet may interact with other medications and alter the treatment response. Please inform your doctor if you are receiving any other treatments, over-the-counter medications (purchased without a prescription), or herbal preparations. Additionally, notify your doctor if you have any scheduled surgery or vaccinations.
  • The concomitant use of this medicine with lithium (used for psychiatric conditions) is not recommended. If its use is essential, your doctor may regularly monitor your blood lithium levels.
  • You should refrain from taking medications such as aliskiren, captopril, and candesartan alongside this medicine, as this may heighten your risk of experiencing additional side effects.
  • Simultaneous use of Telvas 3d Tablet with other antihypertensive medications (such as prazosin, hydrochlorothiazide, and indapamide) is not recommended, as this may amplify the blood pressure-lowering effects of this medicine.
  • Medications like spironolactone and potassium supplements may elevate your blood potassium levels if taken concurrently and should therefore not be used simultaneously.
  • The concomitant use of dantrolene (used to treat muscle stiffness and spasms) with this medicine should be avoided.
  • Medicines such as baclofen, aspirin, ibuprofen, amifostine, phenobarbital, colesevelam, simvastatin, antacids (aluminium magnesium hydroxide), ketoconazole (used for fungal infections), rifampicin (antibiotic), ritonavir (used to treat HIV), verapamil (for heart conditions), tacrolimus (to boost the immune system), and ciclosporin (to suppress the immune system) should be used with caution alongside Telvas 3d Tablet.
