Spegra tablet is a fixed-dose combination antiretroviral medicine containing three active ingredients: dolutegravir, emtricitabine, and tenofovir alafenamide. It is prescribed for the management of HIV-1 infection. Combining these antiviral agents into a single daily tablet improves treatment adherence and effectively suppresses the virus. This medicine must be taken daily under strict medical supervision.
Dolutegravir belongs to a class of medicine called integrase strand transfer inhibitors (INSTIs). It works by blocking the integrase enzyme, which HIV uses to insert its viral DNA into the host cell's DNA, thereby preventing the virus from multiplying. Emtricitabine and tenofovir alafenamide are nucleoside analog reverse transcriptase inhibitors (NRTIs). They work by interfering with the reverse transcriptase enzyme, incorporating themselves into the viral DNA chain, and causing premature termination of viral replication.
When used together, this triple-medicine combination effectively lowers the viral load and delays disease progression. Regular monitoring of liver and kidney health, as well as Hepatitis B status, is required during therapy.

- If you have had a previous hypersensitivity (allergic) reaction to dolutegravir, emtricitabine, or tenofovir alafenamide.

- If you have severe renal (kidney) impairment with an estimated creatinine clearance below 30 mL per minute.

- You have both HIV-1 and Hepatitis B infection, as stopping this medicine may worsen your Hepatitis B.
- You have liver problems or notice symptoms such as yellowing of your skin or eyes, dark urine, or pain in the upper right side of your stomach.
- You develop signs of a severe allergic reaction, such as rash, fever, blisters, swelling of the face, or difficulty breathing.
- You have kidney disease, a history of kidney transplant, or are experiencing changes in urination or kidney function.
- You experience unusual muscle pain, severe weakness, shortness of breath, or feel unusually cold in your hands and feet (possible lactic acidosis).
- Your immune system begins to change, and you develop symptoms of infection, such as fever, cough, or inflammation (immune reconstitution syndrome).
Directions for Use
- Spegra tablet is to be taken exactly as advised by your doctor.
- Take the medicine with a glass of water. It can be taken with or without food.
- Take it at a fixed time every day for optimal results and to maintain steady medicine levels in your body.
Storage and disposal
- Store below 30°C (86°F).
- Protect from moisture by keeping it in its original container and tightly closing the bottle.
- Do not remove the desiccant packet from the bottle.
- Keep it out of the reach of children and pets.
- Do not throw away any medicines down the drain or into household waste.

Interactions
Interactions with other medicines

- Especially tell your doctor if you are taking:
- Dofetilide (taking these together is strictly contraindicated).
- Antacids, laxatives, buffered medications, or sucralfate containing aluminum or magnesium (must be taken 2 hours before or 6 hours after Spegra tablet).
- Oral calcium or iron supplements, including multivitamins (must be taken 2 hours before or 6 hours after Spegra tablet, unless taken together with food).
- Metformin (your doctor may need to limit your dose and monitor your blood sugar).
- Anti-seizure medications (such as carbamazepine, oxcarbazepine, phenytoin, phenobarbital).
- Herbal remedies for depression and anxiety, specifically St. John's wort (Hypericum perforatum).
- Antimycobacterials like rifampin, rifabutin, or rifapentine.
- Other HIV medicines (such as efavirenz, etravirine, nevirapine, or fosamprenavir/ritonavir).
Interactions with food items
- Spegra tablet can be taken with or without food. Taking it with food may help reduce common side effects, such as nausea.
- If you need to take calcium or iron supplements, you can take them at the exact same time as Spegra tablet only if you take them with food. If you are taking them on an empty stomach, you must space them out (2 hours before or 6 hours after Spegra tablet).
- Avoid taking herbal supplements containing St. John's wort, as this can severely lower the effectiveness of the medicine.
