Medical Description
Foiri injection is a prescription anticancer medication belonging to the class of drugs known as topoisomerase I inhibitors ( camptothecin derivatives). It is primarily used for the treatment of advanced or metastatic colorectal cancer. In the body, irinotecan is converted to an active substance called SN-38. It blocks an enzyme called topoisomerase I, which cancer cells need to copy and repair their DNA. This causes DNA damage and prevents cancer cells from growing and multiplying, eventually leading to cancer-cell death.
Foiri injection is given as an intravenous (IV) infusion by a healthcare professional. Because irinotecan can cause significant side effects, your doctor will monitor your blood counts, liver function, and symptoms such as diarrhoea and signs of infection during treatment.

Directions for Use
- Foiri injection is always prepared and administered by a trained healthcare professional as an intravenous infusion in a hospital or clinic.
- Do not attempt to self-administer or prepare this medication at home.
- Ensure proper hydration and follow all pre-medication instructions (such as receiving antiemetics or atropine if needed for cholinergic symptoms) provided by your care team.
- Keep all scheduled appointments for your treatment cycles, blood tests, and clinical evaluations.
Storage and disposal

- Keep out of the reach of children and pets in a secure medical environment.

Quick Tips
- Keep all scheduled blood test appointments so your doctor can check your white blood cell counts and liver function before every cycle.
- Ensure you have your prescribed anti-diarrheal medication at home before your infusion begins, and know exactly how to use it at the very first sign of late diarrhea.
- Report any fever, chills, dizziness, or persistent diarrhea to your medical team immediately.
- Watch for early cholinergic symptoms during your infusion (like excessive sweating or abdominal cramps) and notify your nurse right away so they can treat it.
- Inform all your healthcare providers that you are taking this medication before undergoing any other medical procedures or dental work.
Dosage
Overdose
- If an overdose is suspected, emergency medical intervention must be initiated immediately in a hospital setting.
- Symptoms of an overdose can include severe myelosuppression, profound neutropenia, and severe gastrointestinal complications.
- Management involves supportive hospital care, growth factor administration, aggressive anti-diarrheal protocols, and close clinical monitoring.
Missed a Dose
- If you miss a scheduled infusion appointment, contact your hospital or oncology clinic immediately to reschedule.
- Do not try to make up a missed session on your own, as your doctor will safely adjust your treatment schedule based on your blood counts and recovery status.

Interactions
Interactions with other medicines
- Avoid St. John's wort (a herbal supplement), as it can significantly decrease irinotecan blood levels and effectiveness.
- Avoid live or attenuated vaccines during treatment and for several months afterward due to immunosuppression.
- Inform your doctor about all prescription drugs, over-the-counter medicines, and herbal supplements you are taking, especially strong CYP3A4 inhibitors (e.g., ketoconazole) or inducers (e.g., rifampicin, anticonvulsants) which alter drug clearance.
- Use caution with neuromuscular blocking agents, as irinotecan can prolong their activity.
