Medical Description

Envira Injection is used in the management of inflammatory arthritis, including rheumatoid arthritis, juvenile idiopathic arthritis, psoriatic arthritis, ankylosing spondylitis, and non-radiographic axial spondyloarthritis. It is also used for psoriasis, including paediatric plaque psoriasis; inflammatory bowel diseases, including Crohn’s disease and ulcerative colitis in adults and children; hidradenitis suppurativa (a chronic skin condition causing painful lumps and abscesses); and uveitis, including paediatric uveitis (inflammation inside the eye). Envira Injection is a tumor necrosis factor (TNF) blocker that contains adalimumab. It works by binding specifically to TNF-alpha, a natural protein involved in normal inflammatory and immune responses, and blocking its interaction with TNF receptors on the surface of cells. Raised levels of TNF are found in the joints of people with rheumatoid arthritis, juvenile idiopathic arthritis, psoriatic arthritis, and ankylosing spondylitis, and in psoriasis plaques, where they drive inflammation and joint damage. Envira Injection can increase the risk of serious infections including tuberculosis (TB), bacterial sepsis, and invasive fungal infections. Lymphoma and other cancers may occur in children and adolescents treated with TNF blockers. Hepatosplenic T-cell lymphoma, although rare, but may occur in adolescent and young adult males with Crohn's disease or ulcerative colitis who were also taking azathioprine or 6-mercaptopurine. Patients are tested for TB before and during treatment. Envira Injection is given as an injection under the skin, under the guidance and supervision of a doctor. Envira Injection may cause side effects such as infections (including upper respiratory infection and sinusitis), injection-site reactions, headache, and rash. Warning signs that should be immediately reported to your doctor and need urgent medical attention include signs of infection (such as persistent fever), unusual bruising, bleeding, or paleness, signs of a serious allergic reaction, new or worsening heart failure symptoms, new nervous system symptoms, and signs of a lupus-like illness or liver problems. Before starting Envira Injection, you should discuss your complete medical history with your doctor, including any current or recurring infections, exposure to or history of tuberculosis, travel to or residence in areas where TB or fungal infections are common, hepatitis B infection, any cancer, heart failure, nervous system disorders such as multiple sclerosis, and latex sensitivity. You should be up to date with vaccinations before starting and avoid live vaccines during treatment and should also tell your doctor about pregnancy or plans to become pregnant.

Breast Feeding

Breast Feeding

It is not known if Envira Injection is present in breast milk. Your doctor will weigh the benefits of breastfeeding with your need for the medicine.

Headache, confusion, tingling, and, rarely, nervous system or vision problems (such as optic neuritis) have been reported as side effects during Envira Injection treatment. Avoid driving or operating machinery if you have any of these symptoms, and consult your doctor.

It is best to discuss alcohol use with your doctor, because TNF blockers can raise liver enzymes and may cause severe liver problems.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You have an active infection, or a chronic or recurring infection.
  • You have been exposed to tuberculosis, or have lived in or traveled to areas where TB or fungal infections are common.
  • You have hepatitis B or are a carrier of the hepatitis B virus.
  • You have or have had cancer, or you have had prolonged immunosuppressant treatment.

Directions for Use

  • Envira Injection is given as an injection under the skin (subcutaneously), into the thigh or abdomen by a trained healthcare professional in the doses recommended by your doctor.
  • It is recommended to use a different site for each injection, and do not inject into skin that is tender, bruised, red, or hard.

Quick Tips

  • Getting tested for tuberculosis is necessary before starting treatment and at intervals during the process, even if the initial test is negative.
  • Immediately inform your healthcare team if you experience fever, chills, cough, or other infection symptoms.
  • During treatment, avoid live vaccines and consult your doctor before any immunizations.
  • If possible, ensure children are current on their immunizations before beginning.
  • Regular skin checks are important to detect skin cancer, especially if you've had long-term immunosuppressant or PUVA therapy for psoriasis.
  • Inform your doctor if you have hepatitis B or are a carrier of the virus. Also, report any unusual bruising, bleeding, pallor, or persistent fever.

Dosage

Overdose

Because Envira Injection is administered by a healthcare provider, the chance of overdose is unlikely. There is no specific antidote for Envira Injection overdose. Treatment is supportive, with appropriate monitoring.

Missed a Dose

If you miss a dose, contact your doctor or pharmacist as soon as possible for advice. Take Envira Injection exactly as your doctor has prescribed and do not stop until your doctor tells you to.

Interactions

Interactions with other medicines

  • Using Envira Injection with abatacept or anakinra is not recommended, because it increases the risk of serious infections.
  • Using Envira Injection with other biologic DMARDs or other TNF blockers is not recommended due to a possible increase in the risk of infections and other interactions. A higher rate of serious infections has been seen in rheumatoid arthritis patients treated with rituximab who then received a TNF blocker.
  • Live vaccines should be avoided while on Envira Injection.
  • Azathioprine or 6-mercaptopurine used together with a TNF blocker has been linked to a rare, aggressive lymphoma (hepatosplenic T-cell lymphoma). Use this combination with caution.
  • Envira Injection may affect CYP450 enzymes, which help break down some medicines. If you start or stop Envira Injection while taking medicines such as warfarin, cyclosporine, or theophylline, your doctor may monitor their effects or blood levels and adjust the dose if needed.
  • Patients should tell their doctor about all prescription medicines, over-the-counter medicines, supplements, and herbal products they are taking before and during Envira Injection treatment.

Frequently Asked Questions (FAQs)

Do not stop or skip treatment without speaking to your doctor first. Your doctor can help manage side effects and decide whether treatment should be continued, delayed, or adjusted.
Patients on Envira Injection may receive vaccinations, except for live vaccines. Talk to your doctor before any vaccination.
