Medical Description

Zejula tablet is prescribed as a maintenance treatment for adults with advanced or recurrent epithelial ovarian, fallopian tube, or primary peritoneal cancer whose tumor has responded (completely or partially) to platinum-based chemotherapy.

Zejula tablet contains niraparib, an oral targeted cancer medicine called a PARP inhibitor. Certain cancer cells rely on specific repair enzymes called PARP-1 and PARP-2 to fix damaged DNA and survive. Zejula tablet blocks these enzymes. When blocked, the cancer cells accumulate fatal DNA damage and die. This treatment is particularly effective against cancer cells that already struggle to repair DNA, such as those with BRCA gene mutations or homologous recombination deficiency (HRD).

Maintenance therapy is taken after chemotherapy to help keep the cancer from returning or growing for as long as possible. Depending on the indication, your doctor may use an approved diagnostic test to identify the relevant genetic markers before starting treatment.

Uses

Zejula tablet is used as a maintenance treatment for adults with:
  • Maintenance treatment of adults with advanced high-grade ovarian, fallopian tube, or primary peritoneal cancer who have achieved a complete or partial response following first-line platinum-based chemotherapy.
  • Maintenance treatment of adults with platinum-sensitive, relapsed high-grade serous epithelial ovarian, fallopian tube, or primary peritoneal cancer who have achieved a complete or partial response to platinum-based chemotherapy.

Contraindications

If you have a known severe allergy or hypersensitivity to any ingredient in this medicine.

  • Severe Bone Marrow Suppression (Pancytopenia): Severe reductions in blood cell counts.
  • Hypertensive Crisis: Severely elevated blood pressure.

Precautions and Warnings

Pregnancy

Pregnancy

No. Zejula tablet can cause fetal harm or pregnancy loss. Effective contraception is required during treatment and for several months after your last dose. Consult your doctor before starting the treatment.

Breast Feeding

Breast Feeding

No. It is unknown whether niraparib passes into breast milk. Do not breastfeed during treatment or for a few months following your last dose. Consult your doctor before starting the treatment.

Driving

Driving

Use caution. Zejula tablet can cause fatigue, dizziness, and headache, which may affect your ability to drive or operate machinery safely. If you feel very tired or unwell, do not drive and let your doctor know.

Alcohol

Alcohol

It is best to avoid alcohol during your treatment course. If you have any questions about whether any amount of alcohol is safe for you, speak to your doctor before making that decision.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • Develop persistent fever, severe weakness, or unexplained bruising (potential signs of MDS/AML or severe bone marrow suppression).
  • Experience significant spikes in blood pressure or heart rate.
  • Develop sudden, severe headaches, vision changes, or confusion (signs of PRES).
  • Have heart problems or high blood pressure.
  • Have liver problems, as dose adjustment may be required in moderate hepatic impairment.

Directions for Use

  • Take Zejula tablet once daily by mouth at roughly the same time every day.
  • Swallow tablets whole; do not chew, crush, or split.
  • You may take it with or without food. Taking your dose at bedtime can help reduce nausea.
  • Your initial dose depends on your body weight and baseline blood counts. Always follow your doctor’s prescribed dosage.

Quick Tips

  • Taking your tablet at bedtime can help reduce nausea.
  • Keep all scheduled weekly and monthly blood test appointments; they are essential for monitoring your bone marrow safety.
  • Monitor your blood pressure regularly at home or at your local clinic.
  • Women must use effective birth control during treatment and for several months after the last dose.
  • Do not breastfeed during treatment and for a few months after the final dose.

Dosage

Overdose

If you suspect an accidental overdose, contact emergency medical services or your doctor immediately. Treatment involves stopping the drug and managing symptoms, as there is no specific antidote.

Missed a Dose

  • If you miss a dose, skip it and take your next dose at the regularly scheduled time. Do not take extra tablets to make up for a missed dose.
  • If you vomit after taking a tablet, do not take an extra dose; take your next scheduled dose as usual.

How does this medicine work?

  • Niraparib inhibits PARP-1 and PARP-2 enzymes involved in DNA repair. By inhibiting PARP activity and increasing PARP–DNA complex formation, niraparib promotes DNA damage, which can lead to apoptosis and cancer cell death.
  • Niraparib has shown cytotoxic effects in cancer cells with or without BRCA1/2 deficiencies and anti-tumour activity in preclinical models with BRCA1/2 mutations or homologous recombination deficiency (HRD).

Interactions

Interactions with other medicines

  • Niraparib may affect how some medicines are transported and processed in the body. Tell your doctor about all prescription medicines, over-the-counter medicines, vitamins and herbal supplements you take before starting niraparib.
  • Niraparib is not significantly broken down by an enzyme called CYP3A4, so interactions involving this enzyme are not expected to be significant.
  • Blood pressure medicines: Niraparib can increase blood pressure. Your doctor may check your blood pressure regularly and adjust your blood pressure medicine if needed.

Interactions with food items

  • No specific interactions have been reported.
  • Try to avoid alcohol during treatment to prevent worsening fatigue, dizziness, and nausea.
  • Drink adequate fluids daily to support your kidneys and overall health.

Frequently Asked Questions (FAQs)

HRD-positive means your tumor cells have a reduced ability to repair damaged DNA. For first-line maintenance treatment, HRD-positive status is defined by a deleterious or suspected deleterious BRCA mutation and/or genomic instability. Your oncologist may use an approved diagnostic test to identify the relevant biomarker.
Maintenance treatment is started after chemotherapy has successfully shrunk or cleared your tumor. Instead of replacing chemotherapy, it helps sustain that response and delays the cancer from growing back.
Zejula tablet commonly lowers blood counts (platelets, red blood cells, and white blood cells). Frequent blood tests allow your doctor to adjust your dose or temporarily pause treatment before severe complications arise.
