Medical Description
Zejula tablet is prescribed as a maintenance treatment for adults with advanced or recurrent epithelial ovarian, fallopian tube, or primary peritoneal cancer whose tumor has responded (completely or partially) to platinum-based chemotherapy.
Zejula tablet contains niraparib, an oral targeted cancer medicine called a PARP inhibitor. Certain cancer cells rely on specific repair enzymes called PARP-1 and PARP-2 to fix damaged DNA and survive. Zejula tablet blocks these enzymes. When blocked, the cancer cells accumulate fatal DNA damage and die. This treatment is particularly effective against cancer cells that already struggle to repair DNA, such as those with BRCA gene mutations or homologous recombination deficiency (HRD).
Maintenance therapy is taken after chemotherapy to help keep the cancer from returning or growing for as long as possible. Depending on the indication, your doctor may use an approved diagnostic test to identify the relevant genetic markers before starting treatment.
Uses
- Maintenance treatment of adults with advanced high-grade ovarian, fallopian tube, or primary peritoneal cancer who have achieved a complete or partial response following first-line platinum-based chemotherapy.
- Maintenance treatment of adults with platinum-sensitive, relapsed high-grade serous epithelial ovarian, fallopian tube, or primary peritoneal cancer who have achieved a complete or partial response to platinum-based chemotherapy.

- MDS / AML: Rare, potentially fatal bone marrow disorders (Myelodysplastic Syndrome / Acute Myeloid Leukemia). Symptoms include persistent fever, severe fatigue, frequent infections, or unexplained bleeding.
- Severe Bone Marrow Suppression (Pancytopenia): Severe reductions in blood cell counts.
- Hypertensive Crisis: Severely elevated blood pressure.
- PRES (Posterior Reversible Encephalopathy Syndrome): Rare brain disorder causing severe headaches, seizures, vision loss, or confusion.
- Pneumonitis: Non-infectious lung inflammation.

Directions for Use
- Take Zejula tablet once daily by mouth at roughly the same time every day.

How does this medicine work?
- Niraparib inhibits PARP-1 and PARP-2 enzymes involved in DNA repair. By inhibiting PARP activity and increasing PARP–DNA complex formation, niraparib promotes DNA damage, which can lead to apoptosis and cancer cell death.
- Niraparib has shown cytotoxic effects in cancer cells with or without BRCA1/2 deficiencies and anti-tumour activity in preclinical models with BRCA1/2 mutations or homologous recombination deficiency (HRD).
Interactions
Interactions with other medicines
- Niraparib may affect how some medicines are transported and processed in the body. Tell your doctor about all prescription medicines, over-the-counter medicines, vitamins and herbal supplements you take before starting niraparib.
- Niraparib is not significantly broken down by an enzyme called CYP3A4, so interactions involving this enzyme are not expected to be significant.
- Blood pressure medicines: Niraparib can increase blood pressure. Your doctor may check your blood pressure regularly and adjust your blood pressure medicine if needed.
