Medical Description
Venclexta tablet is used for the treatment of adults with chronic lymphocytic leukemia (CLL) and in combination with other medicines for certain patients with Acute Myeloid Leukaemia (AML). It contains venetoclax as its active ingredient. It is a prescription cancer medicine in a class of medicines called BCL-2 inhibitors (B-cell lymphoma 2 inhibitors).
Certain blood cancer cells overexpress BCL-2, a protein that prevents natural cell death (apoptosis). Venclexta tablet blocks the BCL-2 protein, helping restore the cancer cells’ natural ability to die.
Venclexta tablet is an oral medicine taken once daily under specialist supervision. Treatment for CLL and AML begins with a gradual dose increase (ramp-up) to reduce the risk of tumour lysis syndrome (TLS) and continues until disease progression or unacceptable toxicity.

Contraindications
- Strong CYP3A inhibitors such as ketoconazole, posaconazole, clarithromycin, and certain HIV medicines should generally be avoided during the venetoclax ramp-up phase for CLL/SLL, unless specifically prescribed by your doctor, as they can significantly increase venetoclax levels and the risk of tumour lysis syndrome (TLS).
Side effects
- Low white blood cell count (neutropenia): Increases the risk of serious infections.
- Febrile neutropenia: Fever combined with a low white blood cell count.
- Low platelet count (thrombocytopenia): Can cause easy bruising or bleeding.
- Low red blood cell count (anaemia): Can cause tiredness and breathlessness.
- Upper respiratory tract infections: Includes colds, sore throats, and runny noses.

- Sepsis:A serious infection that can spread throughout the body.

It can cause kidney failure requiring dialysis, irregular heartbeat, seizures, or death. (Note: This has been reported after a single 20 mg dose).

Other General Warnings
- You develop fever, chills, reduced urination, muscle cramps, irregular heartbeat, nausea, or unusual weakness, particularly when starting treatment or after restarting it, as these may indicate tumour lysis syndrome (TLS).
- You develop fever, persistent cough, chills, or other signs of infection, as Venclexta tablet can cause low white blood cell counts and increase infection risk.
- You have reduced kidney function, as this may increase TLS risk and require closer monitoring.
- You start any new medicine, especially CYP3A or P-glycoprotein inhibitors or CYP3A inducers, as these can significantly alter venetoclax levels.
- You take warfarin or other blood thinners, as monitoring may be required due to potential changes in their effects.
- You have severe liver impairment (Child-Pugh C), as dose adjustment and closer monitoring may be required.
- Avoid live vaccines during treatment and until your B-cell counts have recovered, unless your doctor advises otherwise.
- You are planning to father a child, as venetoclax may affect male fertility based on animal studies.
Directions for Use
- Take Venclexta tablet exactly as your doctor prescribed, daily with a meal and water, at about the same time each day.
- Never stop Venclexta tablet or change your dose without first consulting your doctor.
Storage and disposal
- Store in the original container at or below 86°F (30°C) to prevent moisture damage.
- Do not freeze.
- Keep out of reach of children and pets.

Overdose
Missed a Dose
- If you miss a dose within 8 hours of the usual time, take it as soon as possible with food and water, then continue your normal schedule.
- If more than 8 hours have passed, skip the missed dose and take the next dose at the usual time.
- If you vomit after taking a dose, do not take an extra dose that day.
- Do not double the dose to make up for a missed dose.

Interactions with other medicines
- Strong CYP3A inhibitors (e.g., ketoconazole, posaconazole, clarithromycin, and certain HIV medicines) can significantly increase venetoclax levels and require dose adjustment or avoidance, particularly during the ramp-up phase.
- Strong or moderate CYP3A inducers (e.g., rifampicin, carbamazepine, phenytoin, and St. John’s Wort) should be avoided, as they may reduce venetoclax effectiveness.
- Warfarin: Venetoclax may increase warfarin levels; closer INR monitoring is recommended.
- P-gp substrates such as digoxin may require separate administration and monitoring.
- Tell your doctor about all medicines, vitamins, supplements, and herbal products you take before starting treatment.
Interactions with food items
- Venclexta tablet must be taken with a meal and water at every dose, as food significantly increases the amount of medicine absorbed into the body; taking it on an empty stomach substantially reduces its levels and effectiveness.
- Avoid grapefruit, grapefruit juice, Seville oranges (often found in marmalade), and starfruit throughout your treatment, as these contain natural substances that can raise Venclexta tablet blood levels and increase the risk of TLS and other serious side effects.
- Drink plenty of water and fluids every day, especially during the ramp-up phase, as your doctor advises, to help prevent TLS.
- Avoid alcohol during treatment, as it can worsen fatigue and diarrhoea, which are already common side effects.
