Medical Description

Tamimex tablet contains tamoxifen citrate, an anti-oestrogen medicine used to treat breast cancer in pre- and post-menopausal women. It works by blocking the effect of oestrogen on breast cancer cells that depend on this hormone to grow, helping to slow or stop the growth and spread of the cancer.

This medicine increases the risk of blood clots and can cause changes to the lining of the womb, including a small increase in the risk of womb cancer, so your doctor will want to know about any history of blood clots and will ask you to report any abnormal vaginal bleeding promptly. Tamoxifen has also been associated with QT prolongation, particularly when used with other medicines that can prolong the QT interval or in patients with relevant risk factors.

Because Tamimex tablet affects hormone-sensitive tissue throughout the body, it commonly causes hot flushes, fatigue, and changes in your menstrual cycle, vaginal discharge, or bleeding. It can also affect the eyes over long-term use, so report any changes in vision to your doctor. Contact your doctor urgently if you develop leg pain or swelling, chest pain, sudden breathlessness, or unusual vaginal bleeding.

Before starting Tamimex tablet, tell your doctor about any history of blood clots, heart rhythm problems, or if you are taking coumarin-type blood thinners, as this medicine can significantly increase their effect. Tell your doctor if you are pregnant or breastfeeding, as this medicine is not recommended during this period.

Uses

Treatment of breast cancer in pre- and post-menopausal women.

Contraindications

  • If you have a known hypersensitivity to tamoxifen citrate or any ingredient of this medicine.
  • If you are pregnant.
  • If you have a history of blood clots in the veins (deep vein thrombosis or pulmonary embolism), when this medicine is being used to reduce breast cancer risk rather than to treat existing cancer.
  • If you are taking coumarin-type anticoagulants concurrently, when this medicine is being used to reduce breast cancer risk.
  • A history of blood clots is not, by itself, a general contraindication when tamoxifen is being used to treat breast cancer; your doctor will assess the risks and benefits.

Precautions and Warnings

Pregnancy

Pregnancy

No. Tamimex tablet must not be administered during pregnancy. Women who could become pregnant should discuss appropriate contraception with their doctor, as premenopausal patients require careful assessment before starting treatment.

Breast Feeding

Breast Feeding

No. Tamoxifen citrate and its metabolites are excreted into breast milk and can accumulate over time, so breastfeeding is not recommended during treatment.

Alcohol

Alcohol

This is not specifically addressed in the prescribing information. Discuss alcohol use with your doctor.

Driving

Driving

This medicine is generally considered safe for driving, but fatigue has been reported with its use. If you feel unusually tired, use caution when driving or operating machinery.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You have a history of blood clots or an irregular heart rhythm.
  • You are taking blood-thinning medicines.
  • You notice any abnormal vaginal bleeding, discharge, or menstrual changes.
  • You develop leg pain or swelling, chest pain, or sudden breathlessness.
  • You notice any changes in your vision.
  • You develop a severe skin rash or blistering.
  • You are scheduled for surgery, as your doctor may advise temporarily stopping this medicine depending on your risk of blood clots.

Directions for Use

  • Take Tamimex tablet exactly as prescribed by your doctor, at around the same time each day.
  • Swallow the tablet with water.
  • Continue taking this medicine for as long as your doctor advises, even if you feel well.
  • Do not stop taking this medicine without first speaking to your doctor.

Storage and disposal

  • Do not store above 30 degrees C. Keep in the original container.
  • Keep out of the reach of children and pets.
  • This is a hazardous cytotoxic medicine; return any unused or expired tablets to your pharmacy for safe disposal rather than discarding them in household waste.

Quick Tips

  • Report any unusual vaginal bleeding, spotting, or discharge to your doctor promptly.
  • Report any leg pain or swelling, chest pain, or sudden breathlessness immediately, as these can indicate a blood clot.
  • Report any changes in your vision to your doctor.
  • Keep regular gynaecological check-ups as advised by your doctor.
  • Discuss elective surgery in advance with your doctor, as your doctor may advise temporarily stopping treatment depending on your individual risk of blood clots.

Dosage

Missed a Dose

If you miss a dose, take it as soon as you remember unless it is close to your next scheduled dose; in that case, skip the missed dose and continue your normal schedule. Do not take a double dose to make up for a missed one.

Overdose

  • There is no specific antidote for tamoxifen citrate overdose, and treatment must be symptomatic.
  • Very high overdoses may produce changes in heart rhythm, including QT prolongation.
  • If an overdose is suspected, seek medical attention immediately.

How does this medicine work?

Tamimex tablet contains tamoxifen citrate, which blocks oestrogen receptors on breast cancer cells. Many breast cancers rely on oestrogen to stimulate their growth. By occupying these receptors and preventing oestrogen from binding to them, tamoxifen citrate blocks this growth signal, helping to slow or stop the growth and spread of hormone-sensitive breast cancer.

Interactions

Interactions with other medicines

  • Coumarin-derivative anticoagulants (such as warfarin): tamoxifen citrate can significantly increase their blood-thinning effect, increasing bleeding risk; this combination requires close monitoring.
  • Anastrozole: do not combine with this medicine.
  • Potent CYP2D6 inhibitors (such as paroxetine, fluoxetine, quinidine, cinacalcet, and bupropion): should be avoided where possible, as they may reduce the effectiveness of this medicine.
  • Other cytotoxic (chemotherapy) medicines: combining them with tamoxifen citrate can increase the risk of blood clots.
  • Other medicines known to prolong the QT interval may have an additive effect on heart rhythm.
  • Tell your doctor about all other prescription medicines, over-the-counter medicines, and herbal supplements you are taking.

Interactions with food items

No specific food interactions are documented in the prescribing information.

Frequently Asked Questions (FAQs)

Tamimex tablet can cause changes to the lining of the womb, including a small increased risk of womb cancer. Any abnormal vaginal bleeding or discharge should be reported promptly so your doctor can assess it.
This medicine increases the risk of blood clots, so your doctor needs to know your personal and family history to assess your overall risk and monitor you appropriately.
Some antidepressants, particularly paroxetine and fluoxetine, can reduce how well Tamimex tablet works by blocking an enzyme needed to activate it. Tell your doctor about any antidepressants you are taking.
