Medical Description
Predmet 16 Tablet is a higher-strength corticosteroid used to bring significant inflammation under control and suppress an overactive immune system. It is prescribed in rheumatic flares, severe allergies, inflammatory skin, bowel and eye disease, certain blood disorders, some cancers and after transplantation.
It contains methylprednisolone, about five times as potent as hydrocortisone. It enters cells, binds a receptor and acts on DNA, suppressing the genes that produce inflammatory messengers while activating those that make anti-inflammatory proteins.
Its particular advantage is a clean separation between anti-inflammatory and salt-retaining activity, so at equivalent strength it causes less fluid retention and less rise in blood pressure than prednisolone, which matters in heart or kidney disease.
At this dose, raised blood sugar, increased appetite, redistribution of fat towards the face and trunk, poor sleep and greater susceptibility to infection are all likely. Take it in the morning with food, and report any fever or unexplained illness promptly.
The dose is normally stepped down rather than stopped abruptly. Report persistent hip, groin or shoulder pain, discuss bone protection if treatment will be prolonged, and carry a steroid card.
Medrol 16 tablet, Zempred 16 tablet, Ivepred 16 tablet and Rednisol 16 tablet are some other medications with methylprednisolone as the active substance.

- It causes less fluid retention than prednisolone, which is why it is chosen in heart or kidney disease.
- Take the full dose in the morning with food.

- Carry a steroid card and show it to any doctor, dentist or surgeon.
- Report fever, sore throat, cough or any sign of infection promptly, as steroids mask them.
- Monitor blood sugar closely if you have diabetes; the rise at this dose can be substantial.
- Report persistent hip, groin or shoulder pain rather than dismissing it as your underlying illness.
- Discuss calcium, vitamin D and bone protection at the start if your course will be prolonged.
- Avoid grapefruit juice and ibuprofen-type painkillers.
- Avoid contact with chickenpox and measles if you have never had them.
- Report mood changes, and seek urgent help for thoughts of self-harm.

How does this medicine work?

- It also plays a critical role in maintaining normal body functions during stress.
Interactions
Interactions with other medicines
- Some medicines can affect the way Predmet 16 tablet works or this medicine itself can reduce the effectiveness of other medicines taken at the same time.
- Tell your doctor about all the medicines, supplements, or herbals you are currently taking or might take to avoid any possible interaction.
- Especially, if you are taking antitubercular medicines, antibiotics, blood pressure reducing medicines, oral contraceptives, antacids, and antidiabetic medicines.
- Immunosuppressants like cyclosporine and tacrolimus, you are more likely to experience side effects of either of the medicine.
- Concomitant use of painkillers can cause the risk of stomach bleeding.
- Medicines used to treat muscle disorders (myasthenia gravis) such as distigmine, neostigmine, and pancuronium, if taken along with Predmet 16 tablet, can lead to muscle weakness.
Interactions with food items
- Take Predmet 16 Tablet with or just after food. At this strength the stomach irritation caused by steroids is more pronounced, and an empty stomach makes indigestion considerably more likely.
- Watch sugar and refined carbohydrate closely, as the rise in blood glucose scales with dose. If you have diabetes, monitor from the first day and expect to need more treatment temporarily.
- Keep salt moderate. Methylprednisolone retains less fluid than prednisolone, but salt still raises blood pressure, so limit pickles, papads, packaged snacks and processed food.
- Ensure adequate calcium and vitamin D through milk, curd, paneer, ragi, sesame, green leafy vegetables and safe sun exposure, particularly if your course will run for months.
- Include good protein such as dal, eggs, fish, chicken and paneer, since steroids waste muscle at higher doses and over longer courses.
- Avoid grapefruit juice, which blocks the enzyme clearing methylprednisolone and can raise its level.
- Limit alcohol, which adds to stomach irritation and raised blood sugar.
