Medical Description

Intuca tablet contains tucatinib, a prescription anticancer medicine belonging to the tyrosine kinase inhibitor class. It is used with trastuzumab and capecitabine to treat adults with unresectable or metastatic HER2-positive breast cancer, including cancer that has spread to the brain, after one or more prior HER2-targeted therapies for metastatic disease

Intuca tablet is an oral medication taken as prescribed by the doctor. A specialist oncologist initiates and monitors it. Treatment continues until disease progression or unacceptable toxicity, as determined by the treating doctor.

Uses

  • Intuca tablet is used in combination with trastuzumab and capecitabine to treat adults with advanced HER2-positive breast cancer that cannot be removed by surgery or has spread to other parts of the body, including the brain.
  • It is used in patients who have previously received one or more HER2-targeted treatments for metastatic breast cancer.

Contraindications

No specific contraindications exist; however, patients with a known severe hypersensitivity to tucatinib or any of its components should not take Intuca tablet.

  • Hand-foot syndrome (palmar-plantar erythrodysesthesia), causing redness, pain, swelling and blistering on the palms and soles

Pregnancy

Intuca tablet is not recommended during pregnancy because it may cause fetal harm. . Tell your doctor immediately if you are pregnant, think you may be pregnant, or become pregnant during treatment. Women who can become pregnant must use effective contraception during treatment and for a few weeks after the last dose. Men whose partners can become pregnant must also use effective contraception during treatment and for a few weeks after the last dose.

Breast Feeding

It is not known whether this medicine passes into breast milk. Because of the potential for serious harm to a nursing baby, you should not breastfeed during treatment with Intuca tablet or for 1 week after the last dose. Talk to your doctor about safe alternatives for feeding your baby and plan ahead with the support of your healthcare team.

Intuca tablet does not appear to directly affect the ability to drive or operate machinery. However, if you feel very unwell, tired, or dizzy due to side effects such as severe diarrhoea or fatigue, do not drive and let your doctor know.

Talk to your doctor if
  • You develop severe or persistent diarrhoea, as it can cause dehydration and kidney problems and may require interrupting treatment or adjusting the dose.
  • You notice signs of liver problems, such as yellowing of the skin or eyes, dark urine, severe tiredness, loss of appetite, or upper abdominal pain.
  • You have severe liver impairment (Child-Pugh C), as a dose reduction is required.
  • You develop signs of dehydration, such as excessive thirst, dark urine, dizziness, or rapid heartbeat.
  • You take strong CYP3A or moderate CYP2C8 inducers, or strong CYP2C8 inhibitors, as these may alter tucatinib levels and increase the risk of side effects.
  • You take sensitive CYP3A substrates, as tucatinib may increase their blood levels and adverse effects.
  • You are planning to have children, as the potential effects on fertility should be discussed with your doctor.

Directions for Use

  • Take Intuca tablet exactly as your doctor prescribed.
  • Swallow it whole with water; do not chew, crush, or split it, and do not take broken, cracked, or damaged tablet.
  • Intuca tablet can be taken with or without food.
  • Keep Intuca tablet in its original container with the desiccant packet and discard any unused tablets 3 months after opening the bottle.

Storage and disposal

  • Store at room temperature, 20-25°C (68-77°F), in the original container to protect from moisture; keep the desiccant packet in the bottle and replace the cap tightly after each use.
  • Do not freeze.
  • Discard any unused tablets 3 months after opening the bottle and keep out of reach of children and pets.

  • Never crush, chew, or split the tablets; swallow them whole, and do not take a tablet that is broken or cracked.
  • Contact your doctor immediately if you develop severe or worsening diarrhoea, as it can lead to dehydration and other complications.
  • Tell your doctor right away if you develop yellowing of the skin or eyes, dark urine, extreme tiredness, or pain in the upper right abdomen, as these may signal serious liver damage.
  • Keep the desiccant packet in the original bottle and discard any unused tablets 3 months after opening the bottle.
  • Women of childbearing potential must use effective contraception during treatment and for 1 week after the last dose; men with female partners of childbearing potential must also use contraception during treatment and for 1 week after the last dose.
  • Inform all your doctors, dentists, and emergency care providers that you are taking Intuca tablet, as it can interact with many other medicines.

  • There is no specific antidote for a Intuca tablet overdose. The side effects seen in an overdose are expected to be similar to or more severe than those already described, particularly diarrhoea and liver enzyme abnormalities. If you think you have taken too much of this medicine, contact your doctor or go to the nearest emergency department right away.
  • Since Intuca tablet is taken at home, always keep it in its original container and never share it with others.

Missed a Dose

  • If you miss a dose of Intuca tablet or vomit after taking a dose, do not take an extra tablet. Simply take your next dose at its regular scheduled time and continue as planned.
  • Do not take double doses to make up for a missed one. If you are unsure what to do, contact your doctor or pharmacist for advice.

How does this medicine work?

Intuca tablet works by blocking HER2, a protein that helps certain cancer cells grow and multiply. By blocking HER2, this medicine may slow the growth of HER2-positive cancer cells. It can also reach cancer cells in the brain, which is relevant when the cancer has spread there.

Interactions with other medicines

  • Strong CYP3A or moderate CYP2C8 inducers (e.g., rifampicin) may reduce tucatinib levels and effectiveness, so avoid them.
  • Strong CYP2C8 inhibitors (e.g., gemfibrozil) may increase tucatinib levels and side effects, so you may need to adjust the dose.
  • Tucatinib may increase levels of sensitive CYP3A substrates; you may need to adjust the dose of the other medicine.
  • Tucatinib may increase levels of P-glycoprotein (P-gp) substrates, such as digoxin, so monitoring or dose adjustment may be needed.
  • Tell your doctor about all prescription and OTC medicines, vitamins, supplements, and herbal products you take before starting treatment.
