Medical Description

Ifowem combipack injection is a combination pack containing ifosfamide, an alkylating anticancer medicine, and mesna, a protective medicine given alongside it. Ifosfamide is used to treat certain cancers including ovarian, breast, and testicular carcinoma, and malignant lymphoma. Mesna does not treat cancer itself; it is included specifically to protect the bladder and urinary tract from a toxic ifosfamide breakdown product, reducing the risk of bladder inflammation and bleeding (haemorrhagic cystitis).

Because ifosfamide is a potent chemotherapy medicine, trained healthcare professionals must prepare and administer this combination intravenously in a hospital setting, with close monitoring of blood counts, kidney function, urine, and neurological status throughout treatment. Adequate fluid intake is essential during treatment to help reduce urinary tract toxicity, while mesna provides additional protection against haemorrhagic cystitis.

Ifosfamide can cause suppression of bone marrow function in all patients receiving a full course, so regular blood tests are required; contact your medical team immediately if you develop fever, chills, or signs of infection. Ifosfamide can also affect the brain and nervous system, causing confusion, drowsiness, or in rare cases, seizures; these effects can be severe and may be fatal and need prompt medical attention if they occur. .

Because ifosfamide affects rapidly dividing cells, it commonly causes nausea, vomiting, hair loss, and lowers blood cell counts. Report any blood in your urine, pain when urinating, reduced urine output, breathlessness, or unusual bruising to your doctor immediately.

Before starting Ifowem combipack injection, tell your doctor about any kidney, liver, bladder, or nervous system problems, any prior pelvic radiotherapy, and all other medicines you are taking. Effective contraception is required during and for a period after treatment for both men and women, as ifosfamide can affect fertility.

  • If you have a known hypersensitivity to ifosfamide or any component of the formulation.
  • If you have a urinary outflow obstruction.
  • Severe bone marrow suppression, cystitis, renal or hepatic impairment, or an acute infection may also require treatment to be withheld or avoided, as indicated by your doctor.

Precautions and Warnings

Pregnancy

Pregnancy

Ifowem combipack injection may harm an unborn baby and should generally be avoided during pregnancy unless the potential benefit justifies the potential risk. Women who could become pregnant should use effective contraception during treatment and for a period afterward. Men should also use effective contraception during and after treatment as advised by their doctor, as ifosfamide may affect fertility and reproductive function.

Breast Feeding

Breast Feeding

Breastfeeding is not recommended during treatment with ifosfamide. as ifosfamide is excreted in breast milk and can cause serious effects in a nursing infant.

Alcohol

Alcohol

Alcohol may increase the risk or severity of CNS effects such as drowsiness or confusion and should be discussed with your doctor during treatment.

Driving

Driving

Manifestations of nervous system effects such as confusion or drowsiness may impair your ability to drive or operate machinery. If you feel unwell, tired, or confused, avoid driving until you have discussed it with your doctor.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You have kidney, liver, or bladder problems.
  • You have a history of seizures or nervous system disorders.
  • You develop fever, chills, or signs of infection during treatment.
  • You notice blood in your urine, pain when you urinate, or reduced urine output.
  • You develop confusion, unusual drowsiness, or seizures.
  • You are planning to have children, as ifosfamide can affect fertility in both men and women.

Directions for Use

  • Ifowem combipack injection is prepared and administered only by trained healthcare professionals as an intravenous infusion in a hospital setting.
  • You will be encouraged to drink or receive adequate fluids during and after each infusion to support mesna's protective effect on your bladder.
  • Keep all scheduled treatment and blood test appointments, as your doctor will monitor your blood counts, kidney function, and neurological status closely.

Storage and disposal

  • Hospital pharmacy staff store it in accordance with the manufacturer's instructions; the components require protection from excess heat.
  • Keep out of the reach of children and pets.
  • This is a hazardous cytotoxic medicine; clinical staff manage disposal of vials and infusion equipment according to hospital biomedical waste protocols.

Dosage

Overdose

  • There is no specific antidote for ifosfamide overdose.
  • If an overdose is suspected, emergency medical attention is required immediately; management involves supportive hospital care and close monitoring for nervous system, kidney, blood count, and mucosal toxicity.
  • In severe cases, dialysis may be considered, as it can remove ifosfamide from the blood.management should be directed by a specialist toxicology/oncology team.

Missed a Dose

If you miss a scheduled infusion appointment, contact your hospital or oncology clinic immediately to reschedule. Your doctor will adjust your treatment schedule safely based on your blood counts and recovery status.

How does this medicine work?

Ifosfamide is an alkylating anti-cancer medicine. It works by attaching chemical groups to the DNA of rapidly dividing cancer cells, damaging their genetic material so they can no longer copy themselves or survive, which stops the cancer from growing. As ifosfamide is broken down in the body, it produces a byproduct that can irritate and damage the bladder lining. Mesna binds to this byproduct in the urinary tract, neutralising it and protecting the bladder without reducing ifosfamide's anti-cancer activity.

Interactions

Interactions with other medicines

  • Other medicines that suppress bone marrow function, such as other chemotherapy agents, may add to the drop in blood cell counts.
  • Medicines that increase kidney toxicity risk, such as certain antibiotics and antifungals, may increase the risk of kidney problems when combined with ifosfamide.
  • Medicines that affect the central nervous system, such as sedatives or strong antiemetics, may increase the risk of confusion or drowsiness.
  • Medicines that affect the enzymes involved in ifosfamide metabolism may alter its effects or toxicity. Tell your doctor about all medicines you take rather than stopping or changing any medicine yourself.
  • Avoid live vaccines during treatment because they may increase the risk of serious infection.
  • Tell your doctor about all other prescription medicines, over-the-counter medicines, and herbal supplements you are taking.

Interactions with food items

  • No specific food interactions have been reported.
  • Adequate hydration is important during ifosfamide treatment, according to your treatment protocol.

Frequently Asked Questions (FAQs)

Mesna is not a cancer treatment itself. It protects your bladder from a toxic byproduct that forms as your body breaks down ifosfamide, reducing your risk of bladder inflammation and bleeding.
Ifosfamide can significantly lower blood cell counts. Blood tests let your doctor monitor this, manage the risk of infection or bleeding, and confirm that your counts are adequate before further treatment.
Tell your medical team immediately. While mesna reduces this risk, it does not prevent it in every patient, so report and assess any blood in the urine promptly.
