Medical Description

Foiri injection is a prescription anticancer medication belonging to the class of drugs known as topoisomerase I inhibitors ( camptothecin derivatives). It is primarily used for the treatment of advanced or metastatic colorectal cancer. In the body, irinotecan is converted to an active substance called SN-38. It blocks an enzyme called topoisomerase I, which cancer cells need to copy and repair their DNA. This causes DNA damage and prevents cancer cells from growing and multiplying, eventually leading to cancer-cell death.

Foiri injection is given as an intravenous (IV) infusion by a healthcare professional. Because irinotecan can cause significant side effects, your doctor will monitor your blood counts, liver function, and symptoms such as diarrhoea and signs of infection during treatment.

Uses

Treatment of metastatic colorectal cancer.

Contraindications

  • If you have a known hypersensitivity or severe allergic reaction to irinotecan hydrochloride trihydrate or any of the inactive excipients.
  • If you have a chronic inflammatory bowel disease and/or a bowel obstruction.
  • If you have severe bone marrow failure.
  • If you have significantly elevated baseline bilirubin levels (more than 3 times the upper limit of normal, indicating severe liver issues).
  • If you are pregnant or breastfeeding.
  • If you have a severe decline in your general health and physical ability to perform daily activities (classified by doctors as a poor performance status).

Side effects

  • Late diarrhea (occurring more than 24 hours after infusion, which can be severe and dehydrating)
  • Bone marrow suppression leading to neutropenia (low white blood cell count increasing infection risk), anemia, and thrombocytopenia
  • Nausea, vomiting, and loss of appetite
  • Temporary hair loss (alopecia)
  • Asthenia (weakness) and fatigue

Precautions and Warnings

Pregnancy

Pregnancy

Foiri injection may harm the unborn baby. Avoid pregnancy during treatment and use effective contraception during treatment and for the period advised by your doctor after the last dose. Male patients should also use contraception as advised.

Breast Feeding

Breast Feeding

No. Breastfeeding is contraindicated while receiving Foiri injection due to the potential risk of serious adverse effects on the nursing infant.

Alcohol

Alcohol

Discuss alcohol consumption with your doctor, particularly if you experience nausea, diarrhea, dehydration, or liver problems.

Driving

Driving

Use caution. Foiri injection can cause side effects such as dizziness, visual disturbances, or severe fatigue within 24 hours after an infusion, which may impair your alertness. Avoid driving if you feel unwell.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You experience severe, prolonged diarrhea (especially if it begins more than 24 hours after your infusion), as this requires immediate medical attention and aggressive hydration.
  • You develop a fever, chills, or signs of infection while your blood counts are low.
  • You experience sudden shortness of breath, cough, or breathing difficulties.
  • You have a history of Gilbert's syndrome, as specialized monitoring is required.

Directions for Use

  • Foiri injection is always prepared and administered by a trained healthcare professional as an intravenous infusion in a hospital or clinic.
  • Do not attempt to self-administer or prepare this medication at home.
  • Ensure proper hydration and follow all pre-medication instructions (such as receiving antiemetics or atropine if needed for cholinergic symptoms) provided by your care team.
  • Keep all scheduled appointments for your treatment cycles, blood tests, and clinical evaluations.

Storage and disposal

  • Hospital pharmacies and clinical staff handle storage of Foiri injection vials strictly according to pharmaceutical guidelines (typically stored at controlled room temperature, protected from light).
  • Keep out of the reach of children and pets in a secure medical environment.
  • Clinical staff must properly dispose of vials, infusion bags, and administration equipment in accordance with hospital biomedical waste protocols.

Quick Tips

  • Keep all scheduled blood test appointments so your doctor can check your white blood cell counts and liver function before every cycle.
  • Ensure you have your prescribed anti-diarrheal medication at home before your infusion begins, and know exactly how to use it at the very first sign of late diarrhea.
  • Report any fever, chills, dizziness, or persistent diarrhea to your medical team immediately.
  • Watch for early cholinergic symptoms during your infusion (like excessive sweating or abdominal cramps) and notify your nurse right away so they can treat it.
  • Inform all your healthcare providers that you are taking this medication before undergoing any other medical procedures or dental work.

Dosage

Overdose

  • If an overdose is suspected, emergency medical intervention must be initiated immediately in a hospital setting.
  • Symptoms of an overdose can include severe myelosuppression, profound neutropenia, and severe gastrointestinal complications.
  • Management involves supportive hospital care, growth factor administration, aggressive anti-diarrheal protocols, and close clinical monitoring.

Missed a Dose

  • If you miss a scheduled infusion appointment, contact your hospital or oncology clinic immediately to reschedule.
  • Do not try to make up a missed session on your own, as your doctor will safely adjust your treatment schedule based on your blood counts and recovery status.

How does this medicine work?

Foiri injection is a topoisomerase I inhibitor. After entering the body, it is converted into an active substance called SN-38.SN-38 blocks an enzyme called topoisomerase I, which cancer cells need to copy and repair their DNA. This causes DNA damage and stops cancer cells from growing and multiplying, eventually leading to their death.

Interactions

Interactions with other medicines

  • Avoid St. John's wort (a herbal supplement), as it can significantly decrease irinotecan blood levels and effectiveness.
  • Avoid live or attenuated vaccines during treatment and for several months afterward due to immunosuppression.
  • Inform your doctor about all prescription drugs, over-the-counter medicines, and herbal supplements you are taking, especially strong CYP3A4 inhibitors (e.g., ketoconazole) or inducers (e.g., rifampicin, anticonvulsants) which alter drug clearance.
  • Use caution with neuromuscular blocking agents, as irinotecan can prolong their activity.

Interactions with food items

Maintain a well-balanced diet and consume abundant fluids (drinking plenty of water daily) to stay hydrated, especially if experiencing gastrointestinal side effects.

Frequently Asked Questions (FAQs)

Foiri injection can cause early diarrhoea during or soon after treatment and late diarrhoea more than 24 hours later. Late diarrhoea can become severe and cause dehydration. Your doctor may recommend anti-diarrheal medications and will provide instructions on when to start it.
Irinotecan suppresses bone marrow activity, leading to temporary drops in your white blood cells (increasing infection risk) and other blood cells. Blood tests ensure your body has recovered safely before you receive your next dose.
