• If you have a known severe hypersensitivity or allergy to carfilzomib or any ingredient in the formulation.
• During breastfeeding.

• Low red blood cell count (anemia), causing tiredness, pale skin, and weakness
• Low platelet count (thrombocytopenia), leading to easy bruising or bleeding
• Low white blood cell count (lymphopenia/neutropenia), increasing susceptibility to infections
• Fatigue, generalized physical weakness, and fever
• Nausea, diarrhea, constipation, and vomiting
• Shortness of breath (dyspnea) and cough
• Muscle spasms, back pain, joint pain, or pain in the arms/legs
• Swelling of the lower legs, feet, or hands (peripheral edema)
• Headache, dizziness, and trouble sleeping (insomnia)

• Numbness, tingling, or burning sensation in the hands or feet (peripheral neuropathy)
• High blood pressure (hypertension)
• Increased blood creatinine levels indicating changes in kidney function.
• Upper respiratory tract infections or bronchitis
• Low potassium levels (hypokalaemia)

• Heart Problems: Sudden shortness of breath, chest pain, irregular heartbeat, sudden swelling of ankles/legs, or severe dizziness (signs of heart strain or failure).
• Kidney Problems: Sudden decrease in urination, dark urine, or significant facial/body swelling.
• Infusion Reactions: Fever, chills, joint pain, chest tightness, or skin flushing occurring shortly after an infusion.
• Severe Infections: High fever with severe chills, or a cough that brings up mucus.
• Blood Clotting / Bleeding Events: Unexplained leg pain/swelling, sudden chest pain, coughing up blood, or unusual bleeding/bruising.
• Tumor Lysis Syndrome (TLS): Muscle cramps, weakness, heart palpitations, or confusion caused by the rapid breakdown of cancer cells.
• Thrombotic microangiopathy, including thrombotic thrombocytopenic purpura (TTP) and haemolytic uraemic syndrome (HUS), which can be life-threatening.
• Posterior reversible encephalopathy syndrome (PRES), which may cause seizures, headache, confusion, visual disturbances, or loss of consciousness.
• Severe liver injury, including liver failure, has also been reported.

• Carfimib injection is administered strictly by trained healthcare professionals in a hospital, clinic, or specialized infusion center via intravenous (IV) infusion.
• Infusions typically take a few minutes, depending on the dose and treatment regimen your oncologist prescribes. It is given on a specific cycle schedule
• Your healthcare team will provide appropriate hydration before and, when required, after your doses, along with premedication, to reduce the risk of renal complications and infusion-related reactions.
• Do not skip or delay your scheduled hospital infusion visits without consulting your oncologist directly.

• Hospital staff store vials in a controlled clinical environment, protect them from light, and keep them refrigerated.
• Clinical staff handle reconstituted and diluted solutions exclusively in accordance with strict hazardous drug safety guidelines.
• Medical personnel dispose of all unused product, IV bags, tubing, and needles in designated biological chemotherapy disposal units.

• Keep all scheduled appointments for blood tests, blood pressure checks, and heart and kidney assessments
• Drink plenty of fluids (as your doctor recommends) before and after infusion days to protect your kidneys.
• Report any sudden shortness of breath, chest pressure, lower leg swelling, or fever to your medical team immediately.
• Stay well-rested and pace your daily activities if you experience fatigue or anemia.
• Both male and female patients must use highly effective birth control during treatment and for the specified period after the final dose.

Interactions with other medicines

• Carfilzomib is mainly metabolised by peptidase and epoxide hydrolase pathways, so clinically significant CYP450-mediated interactions are not expected.
• Cardiotoxic medicines: Tell your doctor if you take medicines that can affect heart function, as carfilzomib may also cause cardiovascular complications.
• Blood thinners and NSAIDs: Concomitant use may increase the risk of bleeding or kidney problems in some patients and should be discussed with your doctor.
• Hormonal contraceptives: Contraceptives associated with an increased risk of thrombosis should be avoided where possible; discuss a reliable alternative with your doctor.
• Live vaccines: Avoid them during treatment unless your doctor specifically recommends them.
• Tell your doctor about all prescription and OTC medicines, vitamins, supplements, and herbal products you take before starting treatment.
