Medical Description

Arbaz injection is a prescription medication used in chemotherapy that belongs to a class known as taxanes. It is primarily prescribed in combination with prednisone or prednisolone for adult male patients with metastatic castration-resistant (hormone-refractory) prostate cancer whose disease has progressed during or after previous treatment with docetaxel. This medication targets and stabilizes the internal cellular structures (microtubules) within cancer cells, halting their ability to divide and multiply and ultimately leading to cell death.

Arbaz injection is administered along with oral corticosteroids (prednisone or prednisolone). Treatment cycles are carefully scheduled to allow the patient's body and blood counts time to recover between infusions, all under close supervision by an experienced oncologist.

Patients undergoing this therapy require rigorous medical oversight. Regular laboratory blood tests, monitoring of white and red blood cell counts, checks on kidney and liver function, and careful observation for hypersensitivity reactions are essential to ensure patient safety. Patients aged 65 years and older are more likely to experience specific side effects, including neutropenia, febrile neutropenia, fatigue, dizziness, and infections, and thus require particularly close monitoring.

Uses

Arbaz injection is used in the treatment of metastatic castration-resistant (hormone-refractory) prostate cancer in adult male patients previously treated with a docetaxel-containing regimen, in combination with prednisone or prednisolone.

Contraindications

  • If you have a known hypersensitivity or severe allergic reaction to cabazitaxel, other taxanes, or polysorbate 80.
  • If your baseline white blood cell count (neutrophil count) is too low
  • If you have severe liver impairment (total bilirubin more than 3 times the upper limit of normal).

Side effects

Very Common Side Effects
  • Low white blood cell count (neutropenia), significantly increasing the risk of severe infections.
  • Low red blood cell count (anaemia), causing fatigue, weakness, and shortness of breath.
  • Low platelet count (thrombocytopenia), leading to easy bruising or bleeding.
  • Diarrhoea, nausea, vomiting, and loss of appetite.
  • Fatigue, asthenia (weakness), and fever.
  • Blood in the urine (haematuria).
  • Musculoskeletal pain, including back pain.
  • Taste changes (dysgeusia) and peripheral swelling (oedema).
Common Side Effects
  • Hair loss (alopecia), which is a common side effect of cabazitaxel.
  • Urinary tract infections.
  • Constipation, abdominal pain, and mouth sores (stomatitis).
  • Peripheral neuropathy (numbness, tingling, or pain in the hands and feet).
  • Shortness of breath, cough, and joint pain.
  • Dizziness and headache.
  • Low lymphocyte count and weight loss.
Serious but Less Common Side Effects
  • Severe, life-threatening neutropenic fever and serious infections, such as sepsis or pneumonia.
  • Severe, persistent diarrhoea leading to dehydration and kidney failure.
  • Serious gastrointestinal problems, including bleeding, perforation, bowel obstruction, and neutropenic enterocolitis (bowel inflammation). The risk is higher with neutropenia, older age, steroid use, concurrent NSAIDs or blood thinners, or prior pelvic radiotherapy.
  • Severe hypersensitivity reactions, including rash, flushing, low blood pressure, or breathing difficulties.
  • Interstitial lung disease or severe respiratory complications.
  • Kidney failure or severe urinary tract disorders.
  • Severe bladder inflammation (haemorrhagic cystitis) or radiation-recall cystitis, particularly in patients previously treated with pelvic radiation. This can occur even late in treatment and may require hospitalisation.

Precautions and Warnings

Pregnancy

Pregnancy

Arbaz injection is used only in male patients.

Breast Feeding

Breast Feeding

Arbaz injection is used only in male patients.

Driving

Driving

Use caution. Arbaz injection can cause side effects such as fatigue, weakness, or dizziness that may impair your alertness. Avoid driving or operating machinery if you feel unwell following an infusion.

Alcohol

Alcohol

Try to avoid alcohol during treatment. Discuss alcohol use with your doctor, particularly if you experience dizziness, fatigue, or other treatment-related side effects.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You are 65 years of age or older, which may make you more likely to experience certain side effects and require closer monitoring.
  • You have pre-existing liver impairment, as dosage adjustment is required. A lower dose is used for mild or moderate liver impairment, and the medicine is contraindicated in severe liver impairment.
  • You have pre-existing kidney impairment. No dose adjustment is generally needed unless kidney function is very severely reduced, in which case your doctor will monitor you closely.
  • You experience fever, chills, or any signs of infection during treatment, as prompt medical intervention is required.
  • You notice severe, persistent diarrhoea, abdominal pain, signs of dehydration, or blood in your stool.
  • You develop sudden allergic symptoms during or shortly after the infusion, such as rash, wheezing, or dizziness.
  • You have previously received radiation to the pelvic area, as you may be at higher risk of bladder-related side effects.
  • You have any underlying lung disease, as you may be at higher risk of lung-related side effects.

Directions for Use

  • Arbaz injection is always prepared and administered by a trained healthcare professional as an intravenous infusion in a hospital or clinic.
  • Do not attempt to self-administer or prepare this medication at home.
  • Before each infusion, you will be given premedication directly into a vein, such as an antihistamine, a corticosteroid, and an H2 antagonist, along with anti-sickness medicine as needed, to reduce the risk of allergic reactions and nausea.
  • Keep all scheduled appointments for your treatment cycles and blood tests.

Storage and disposal

  • Hospital pharmacies and clinical staff handle storage of Arbaz injection vials. Store the vials according to the manufacturer's specified storage conditions.
  • Keep out of the reach of children and pets in a secure medical environment.
  • Proper disposal of vials, dilution bags, and infusion equipment is handled by clinical staff in accordance with hospital biomedical waste protocols, as this is a hazardous anticancer medicine.

Quick Tips

  • Keep all scheduled blood test appointments so your doctor can check your white blood cell counts before every treatment cycle.
  • Report any fever, chills, sore throat, or signs of infection to your medical team immediately, as neutropenic fever is a medical emergency.
  • Stay well-hydrated and inform your doctor right away if you experience severe diarrhoea or vomiting.
  • Watch for unusual bruising, tiny red spots on the skin, or bleeding from the gums or nose.
  • Report any blood in your urine, burning during urination, or urinary urgency to your doctor promptly, especially if you have previously had pelvic radiation.
  • Inform all your healthcare providers that you are taking this medication before undergoing any other medical procedures or dental work.
  • If you are 65 years of age or older, you may be more likely to experience certain side effects and should be monitored particularly closely.

Dosage

Missed a Dose

  • If you miss a scheduled infusion appointment, contact your hospital or oncology clinic immediately to reschedule.
  • Do not attempt to make up for a missed session on your own, as your doctor will adjust your treatment schedule safely based on your blood counts and recovery status.

Overdose

  • There is no known antidote for Arbaz injection overdose. Overdose has been reported to result from improper preparation or dilution of the medicine.
  • If an overdose is suspected, emergency medical intervention must be initiated immediately in a hospital setting. Symptoms can include profound bone marrow suppression, severe neutropenia, high fever, and severe gastrointestinal toxicity, and can be fatal.
  • Management involves supportive hospital care, including appropriate infection management and, when indicated, granulocyte colony-stimulating factor (G-CSF), and intensive clinical monitoring.

How does this medicine work?

Arbaz injection is a taxane antineoplastic agent. It works by binding to tubulin, the protein that forms microtubules, which are essential structures for cellular division and replication. By promoting microtubule assembly and stabilising them, Arbaz injection prevents their disassembly, thereby inhibiting mitotic and interphase cell functions. This effectively freezes cancer cells during division and stops tumour proliferation.

Interactions

Interactions with other medicines

  • Avoid strong CYP3A inhibitors, such as ketoconazole, itraconazole, clarithromycin, atazanavir, ritonavir, or telithromycin, as they can increase cabazitaxel blood levels and the risk of severe side effects. If a strong CYP3A inhibitor cannot be avoided, your doctor may reduce the Arbaz injection dose by about 25%.
  • Avoid strong CYP3A inducers, such as rifampicin, carbamazepine, or St. John's wort, as they can decrease cabazitaxel blood levels and effectiveness.
  • Inform your doctor about all prescription medicines, over-the-counter medicines, and herbal supplements you are taking, especially blood thinners, NSAIDs, or other chemotherapy medicines.

Interactions with food items

No specific food interactions are available for Arbaz injection.

Frequently Asked Questions (FAQs)

This medication significantly suppresses bone marrow activity, leading to low white blood cell counts. Blood tests ensure your immune system has recovered safely before your next infusion and help your doctor decide whether a dose adjustment or preventive medicine (G-CSF) is needed.
No. This is a specialised intravenous chemotherapy medication that must be prepared and administered strictly by trained medical professionals in a hospital or clinic setting.
Diarrhoea is a very common side effect and can occasionally be serious. Contact your doctor immediately. They can prescribe anti-diarrhoeal medicines, recommend rehydration fluids, or adjust your care plan or dose.
