Medical Description
Alunbrig tablet contains brigatinib, a targeted anticancer medicine in the protein kinase inhibitor class, used in adults with ALK-positive advanced non-small cell lung cancer (NSCLC), under the supervision of an experienced oncologist.
The tablet is taken as prescribed by the doctor, swallowed whole with water, with or without food, while grapefruit and grapefruit juice must be strictly avoided to prevent elevated drug levels in the blood.
Patients require regular monitoring of blood pressure, heart rate, vision, and routine blood tests for liver, muscle, and pancreatic enzymes, blood sugar, haemoglobin, and white cells.
Before starting, inform your doctor about any history of liver, kidney, heart rhythm, or blood sugar conditions, and ensure both men and women use effective contraception, with women of reproductive potential using a non-hormonal method.
Uses
- ALK-positive advanced non-small cell lung cancer (NSCLC) in adults who have not previously been treated with an ALK inhibitor.
- ALK-positive advanced non-small cell lung cancer (NSCLC) in adults who have previously been treated with crizotinib.

- Slow heart rate, causing dizziness, lightheadedness or fainting
- Vision changes, including blurred vision or sensitivity to light
- Muscle pain, tenderness or weakness with a raised CPK level
- Liver injury
- High blood sugar, which may be new or a worsening of existing diabetes
- Raised pancreatic enzymes, which may occur with severe upper abdominal pain
- Increased sensitivity of the skin to sunlight

- You develop new or worsening breathlessness, cough or chest discomfort, particularly in the first week of treatment or after a dose increase.
- You have severe headaches, dizziness or blurred vision, which may indicate raised blood pressure.
- You feel faint or notice your heart beating slowly.
- Your vision changes in any way.
- You develop unexplained muscle pain, tenderness or weakness.
- You have severe pain in your upper abdomen, which may indicate raised pancreatic enzymes.
- You develop yellowing of the skin or eyes, dark urine or pain in the upper right abdomen.
- You are unusually thirsty or passing more urine than usual, which may indicate high blood sugar.
- Your skin burns easily in the sun.
- You have any liver or kidney problem, as your dose may need lowering.
- You need to start any new medicine, as many interact with this one.
- This medicine is not for use in anyone under 18 years. Safety and effectiveness in children have not been established. No dose change is needed for age alone in older adults, although experience in patients over 85 years is limited.
Directions for Use
- Take Alunbrig tablet exactly as prescribed by the doctor.
- Swallow the tablets whole with water. Do not crush or chew them.
- You may take it with or without food, but avoid grapefruit and grapefruit juice.
- If you miss a dose, or vomit after taking one, do not take an extra tablet. Take your next dose at the usual time.
- Your dose may be lowered if you have severe liver or kidney impairment, or if you need certain other medicines.
- Do not stop or change your dose without speaking to your doctor.
Storage and disposal
- Store below 30°C.
- Keep in a safe place, out of the sight and reach of children and pets.
- Do not use after the expiry date. The shelf life is 3 years.
- Do not throw medicines into household waste or down the drain. Ask your pharmacist how to dispose of any unused medicine.

- Report any new or worsening breathlessness, cough or chest discomfort the same day, especially during the first week of treatment.
- Swallow the tablets whole with water. Never crush or chew them.
- Avoid grapefruit and grapefruit juice completely while on this medicine.
- Report unexplained muscle pain, tenderness or weakness, as your muscle enzyme level will need checking.

- If you have diabetes, monitor your blood sugar more closely.
- Women who could become pregnant must use non-hormonal contraception during treatment and for a few months after the final dose. Men with partners who could become pregnant must use effective contraception during treatment and for a few months after the last dose.
- Tell every doctor and dentist you see that you are on Alunbrig tablet, as it interacts with many medicines.
Dosage
Overdose
- There is no specific antidote for an overdose of Alunbrig tablet.
- If an overdose occurs, you will be monitored for side effects and given supportive treatment until they settle.
- If you think you have taken too much, contact your doctor or go to the nearest emergency department straight away.
Missed a Dose
- If you miss a dose of Alunbrig tablet, do not take it later or take an extra tablet. Take your next dose at its usual scheduled time.
- The same applies if you vomit after taking a dose. Do not replace it. Carry on with your next scheduled dose.
- Never take a double dose to make up for a missed one.
How does this medicine work?
- In ALK-positive lung cancer, a change in the ALK gene leaves the ALK protein permanently switched on, sending a constant signal that tells the cancer cell to keep growing and dividing.
- Alunbrig tablet blocks this protein. With the signal cut off, the cancer cells stop multiplying and die, which slows or shrinks the tumour.
Interactions
Interactions with other medicines
- Tell your doctor and pharmacist about all prescription and OTC medicines, vitamins, supplements, and herbal products you take.
- Strong CYP3A inhibitors may increase Alunbrig tablet levels and the risk of side effects; your doctor may adjust the dose if you cannot avoid them.
- Strong or moderate CYP3A inducers may reduce Alunbrig tablet levels and decrease its effectiveness.
- Women who could become pregnant should use effective non-hormonal contraception during treatment.
Interactions with food items
- Alunbrig tablet can be taken with or without food.
- Avoid grapefruit and grapefruit juice throughout treatment, as they raise this medicine's level in your blood.
- Small, light, frequent meals can make nausea and diarrhoea easier to manage.
- Alcohol is not addressed in the prescribing information. As a general precaution, limit it, as it can add to tiredness and nausea.
