Medical Description

Adrea capsule contains hydroxyurea and is used to prevent recurrent, painful vaso-occlusive crises, including acute chest syndrome, in adults, adolescents, and children over 2 years with symptomatic sickle cell disease. It works partly by increasing foetal haemoglobin (HbF), which helps reduce red blood cell sickling and the resulting blockage of blood vessels.

Adrea capsule causes severe suppression of bone marrow function, so treatment should not be started if your blood counts are already significantly low, and your doctor will monitor your blood counts closely, especially early in treatment and whenever your dose is changed.

This medicine can cause harm to an unborn baby, so effective contraception is recommended for women who could become pregnant, and men taking this medicine should also discuss contraception and potential effects on fertility with their doctor. u

Because Adrea capsule affects blood-cell production, it can change blood counts and cause macrocytosis (enlarged red blood cells), which is an expected effect of treatment. Long-term use has also been associated with skin cancers, so sun protection and regular skin checks are advisable. Rarely, it may cause lung inflammation or scarring.

Before starting Adrea capsule, tell your doctor about any history of low blood counts, skin cancer, lung problems, and about all medicines you take, particularly antiretroviral medicines for HIV. Tell your doctor if you are pregnant, breastfeeding, or could become pregnant.

Uses

Prevention of recurrent, painful vaso-occlusive crises, including acute chest syndrome, in adults, adolescents, and children older than 2 years with symptomatic sickle cell syndrome.

Contraindications

If you have a known hypersensitivity to hydroxyurea or any ingredient of this medicine.

Side effects

Very common side effects:
  • Changes in blood counts (low white cells, low platelets, anaemia)
  • Loss of appetite
  • Nausea, vomiting, diarrhoea, constipation
  • Mouth sores (stomatitis)
  • Hair thinning or loss
  • Rash, skin darkening under the nails (melanonychia)
  • Fever
  • Macrocytosis (enlarged red blood cells)
Serious but less common side effects:
  • Severe suppression of bone marrow function
  • A form of anaemia caused by red blood cell breakdown (haemolytic anaemia)
  • Skin cancers with long-term use
  • Skin ulcers or tissue damage related to blood vessel inflammation (vasculitic toxicity), particularly in patients with a history of interferon treatment
  • Lung scarring, inflammation, or breathing difficulty
  • Leg ulcers, particularly with long-term treatment.

Directions for Use

  • Take Adrea capsule exactly as your doctor prescribed.
  • Swallow the capsule whole with water; do not open, crush, or chew it, as it contains a cytotoxic drug.
  • If you touch a broken or leaking capsule, wash the area thoroughly with soap and water.
  • Keep all scheduled blood test appointments, as your dose may be adjusted based on your blood counts.

Storage and disposal

  • Store below 25 degrees C, with excursions permitted between 15 and 30 degrees C. Keep the container tightly closed.
  • Keep out of the reach of children and pets.
  • This is a hazardous cytotoxic medicine; return any unused or expired capsules to your pharmacy for safe disposal rather than discarding them in household waste.

Missed a Dose

If you miss a dose, contact your doctor for advice. Do not take a double dose to make up for a missed one.

How does this medicine work?

Adrea capsule contains hydroxyurea, which increases the production of foetal haemoglobin in red blood cells. Foetal haemoglobin helps prevent the red blood cells from distorting into a sickle shape under low-oxygen conditions. By reducing the proportion of sickled cells, hydroxyurea helps prevent blocked blood vessels and pain crises that characterise sickle cell disease.

Interactions

Interactions with other medicines

  • Antiretroviral medicines used for HIV, particularly didanosine and stavudine: combined use has been associated with pancreatitis, liver toxicity, and severe nerve damage.
  • Other medicines that suppress the bone marrow may increase the risk of low blood counts.
  • Hydroxyurea may enhance the immunosuppressive effects of live vaccines. Discuss vaccination with your doctor before receiving any vaccine, particularly live vaccines.
