Medical Description
Vildamac M 50/500 Tablet is used with diet and exercise to control blood sugar in adults with type 2 diabetes when a single medicine is no longer enough. It contains vildagliptin and metformin, which lower glucose through two separate mechanisms.
Metformin reduces the amount of glucose the liver releases and improves how well muscles respond to insulin. Vildagliptin blocks the enzyme DPP-4, protecting the gut hormones that prompt insulin release after a meal and suppress glucagon, so insulin rises only when glucose does.
Take it with or immediately after food, usually twice daily, which substantially reduces the nausea, metallic taste, and loose stools metformin causes. Diet and exercise remain part of the treatment rather than an optional extra.
Because vildagliptin has been linked to rare liver enzyme abnormalities, liver function is checked before starting and periodically afterwards. Report yellowing of the eyes or skin, dark urine or persistent nausea straight away.
Pause the medicine during any illness causing dehydration, and before contrast scans or surgery. Kidney function is checked at least yearly, and long-term metformin use can lower vitamin B12 levels, so levels should be monitored.
Galvus Met 50/500 tablet, Jalra M 50/500 tablet, Zomelis Met 50/500 tablet, Torglip M 50/500 tablet, Gliptagreat M 50/500 tablet and Vildapride M 50/500 tablet also contain vildagliptin and metformin as active ingredients.
Uses
- Vildamac M 50/500 Tablet is used together with diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus.
- Vildamac M 50/500 Tablet is prescribed when metformin alone is insufficient, when a patient is already taking vildagliptin and metformin separately, and in combination with other diabetes treatments, including a sulfonylurea or insulin.
Contraindications
- If you are allergic to vildagliptin or metformin or other ingredients of Vildamac M 50/500 mg tablet.
- If you have uncontrolled diabetes along with nausea, vomiting, diarrhoea, rapid weight loss, lactic acidosis (symptoms include stomach pain, fast or rapid breathing, sleepiness).
- If you are in a diabetic coma due to a sudden rise or drop in blood sugar level.
- If you are an alcoholic or consume alcohol in excess regularly.
- If you have a pre-existing medical condition like kidney or liver disorder, heart disease, type 1 diabetes, etc.
- If you have infection or dehydration.
- If you are undergoing surgery or stress.
- If you are breastfeeding.

- Vildamac M 50/500 mg tablet is not recommended for patients below 18 years of age.
Directions for Use
- Vildamac M 50/500 mg tablet must be taken as directed by the doctor.
- Swallow as a whole with a glass of water.
- Do not cut, break or chew the medicine.
- It must be taken with or after having the meal. It would be best if you take it at the same time every day for optimal results.
- Avoid consuming it more than prescribed by the doctor.

- Avoid binge drinking and never drink on an empty stomach.
- Carry your medicines in hand baggage when travelling, with a doctor's letter.

Interactions with other medicines
- Some medicines affect the way Vildamac M 50/500 mg tablet works or the medicine itself reduce the effectiveness of other medicines taken at the same time.
- Inform the doctor regarding the medicines, supplements or herbals you take or might take to avoid possible interaction.
- If you are using other antidiabetic medicines, anti-hypertensive or heart medicine, painkillers, medicine for respiratory problems, etc.
Interactions with food items
- Take Vildamac M 50/500 Tablet with or immediately after food. This makes a substantial difference to the nausea, metallic taste, and diarrhoea from the metformin component.
- Keep meals regular and consistent in carbohydrate content. Skipped meals are a frequent cause of low readings if you also take a sulfonylurea or insulin.
- Avoid binge drinking and never drink on an empty stomach, given the combined risk of lactic acidosis, liver strain, and delayed low blood sugar.
- Drink enough fluid, especially in hot weather or during illness with vomiting or diarrhoea, as dehydration is the commonest trigger for metformin-related complications.
- Include milk, curd, paneer, eggs and fish for vitamin B12, since long-term metformin reduces its absorption.
- Build meals around whole grains, pulses, vegetables, and lean protein while limiting sugary drinks, sweets, refined flour, and fried food.
