Medical Description

Tapal is a prescription medication. It contains Tapentadol as its active ingredient. It is used for the management of moderate to severe pain that is not responding to other medications. Pain is an unpleasant feeling caused by the formation of certain chemicals in the body in response to an injury, tissue damage or disease. Tapentadol relieves this feeling by blocking the transmission of pain signals to the brain. It is important to only take this medication on prescription by a doctor, at the exact dose and for the entire duration as advised.

Uses

  • For relief of moderate to severe acute pain in adults who are 18 years or older

Contraindications

  • Patients who suffer from significant breathing difficulty such as respiratory depression.
  • Patients who have severe bronchial asthma
  • Patients who suffer from paralytic ileus (a type of intestinal obstruction in which food cannot pass through the intestines because of a problem in muscle contraction).
  • Patients who suffer from biliary colic (a type of abdominal pain that occurs as a complication of gallstones).
  • Patients with a known hypersensitivity to Tapentadol.

  • This medication can be taken with or without food.
  • Swallow tablet as a whole. Do not split, crush, or chew.

  • It is important to take this medication only for the duration prescribed to you by your doctor. Taking it for more than the prescribed time may lead to dependence and habit forming. Abruptly stopping may lead to withdrawal effects.

Overdose

An overdose of this medication can lead to many serious side effects. The most common symptoms are respiratory depression, drowsiness, which may progress to stupor or coma, constricted pupils, and cold and clammy skin. Sometimes bradycardia (decreased heart rate), hypotension (decreased blood pressure) and death may occur. Immediately seek medical care at an emergency department in cases of an overdose.

Missed a Dose

In case of a missed dose, skip the missed dose and continue with your regular scheduled dose. Do not double the dose of this medication.

Mode of Action

How Does It Work?

The active component of Tapal is a medicine called Tapentadol. It acts by attaching to certain structures in the body called opioid receptors. This causes blockage in the transmission of pain signals, resulting in analgesia (pain relief). Tapentadol is also known to act by increasing the levels of a certain chemical in the body called norepinephrine. This chemical blocks the transmission of pain signals, resulting in analgesia (pain relief).

Interactions

Interactions with other medicines

  • Co-administration with Tranylcypromine or other monoamine oxidase inhibitors (MAOI) is contraindicated as the risk and severity of adverse effects can be increased. Taking them within 14 days of treatment is also contraindicated.
  • Co-administration with SSRIs, SNRIs and tricyclic antidepressants is not recommended, as Tapentadol may increase the drug's central nervous system depressant effects and may result in a dangerous condition called Serotonin Syndrome.
  • Co-administration with drugs such as pentazocine, nalbuphine, and butorphanol is not recommended, as they may reduce the analgesic effect. They can also precipitate withdrawal symptoms.

Interactions with food items

  • This medication should not be taken with grapefruit juice as it can affect the efficacy of the tablet.

The amount of time for which the effects of this medication lasts varies depending on how long you’ve been taking this medication. Usually, they may last up to 1-2 days. However, it is important to continue taking the medication for the entire duration prescribed by your physician. Abrupt stoppage may lead to withdrawal syndromes.

Drowsiness is a common side effect of Tapal. However, in case you are unable to complete the course of Tapal due to any side effects, it is recommended to inform your doctor about the same. They can guide you regarding any change in dose or medication.

Most people taking this medication will begin to feel its effects within 30 minutes-1 hour of starting. However, it might take a few days for the full benefit of the drug to be felt.
