Medical Description
S One S One capsule is a fixed-dose combination capsule containing tegafur, gimeracil, and oteracil, an oral chemotherapy medicine belonging to the fluoropyrimidine class of anti-cancer agents. It is indicated in adults for the treatment of advanced gastric (stomach) cancer when given in combination with cisplatin, another anti-cancer medicine. The body converts tegafur into 5-fluorouracil (5-FU), the active anti-cancer agent. Gimeracil helps maintain 5-FU levels, while oteracil reduces certain gastrointestinal effects of 5-FU.
Only a doctor experienced in cancer chemotherapy should prescribe this medicine, and treatment requires close monitoring of your blood counts, liver function, kidney function, and blood salts throughout the course. Diarrhoea and dehydration are dose-limiting side effects, so stay well hydrated and contact your doctor promptly if diarrhoea develops.
Because S One S One capsule affects rapidly dividing cells, common side effects include diarrhoea, nausea, vomiting, low blood counts, and fatigue. It may also cause eye problems such as excessive tearing or dry eyes. Rarely, it can cause hyperammonaemia, which may lead to confusion or altered consciousness. Hepatitis B reactivation can also occur, so you may need appropriate screening and monitoring before and during treatment.
Before starting S One S One capsule, tell your doctor about any previous severe reactions to fluoropyrimidine medicines, known DPD deficiency or reduced DPD activity, hepatitis B infection, or kidney or liver problems. Tell your doctor if you are pregnant, breastfeeding, or could become pregnant, as S One S One capsule may cause fetal harm and effective contraception is required during treatment and for the recommended period afterward.
Uses
Contraindications
- If you have a known hypersensitivity to tegafur, gimeracil, oteracil, or any ingredient of this medicine.
- If you have a history of severe, unexpected reactions to similar fluoropyrimidine chemotherapy medicines.
- If you have a known complete deficiency of the enzyme dihydropyrimidine dehydrogenase (DPD).
- If you are pregnant or breastfeeding.
- If you have severe suppression of bone marrow function.
- If you have end-stage kidney disease requiring dialysis.
- If you are taking any other fluoropyrimidine chemotherapy medicine at the same time.
- If you are being treated with, or have recently been treated with, the antiviral medicine brivudine.
Side effects
- Low neutrophil count, low white blood cell count, anaemia, low platelet count
- Loss of appetite
- Diarrhoea, vomiting, nausea, constipation
- Nerve tingling or numbness (peripheral neuropathy)
- Skin reaction affecting the palms and soles, rash, dry skin, itching, hair loss
- Musculoskeletal pain
- Fatigue, weakness
- Raised bilirubin and liver enzymes (seen on blood tests)
- Infections
- Excess tearing, dry eyes, or other eye irritation
- Blood clots, high or low blood pressure
- Breathlessness, nosebleeds, cough
- Mouth sores, abdominal discomfort
- Fever, weight loss, swelling
- Severe infection or sepsis
- Severe bone marrow suppression
- Reactivation of hepatitis B infection
- Kidney impairment or failure
- Confusion or altered consciousness related to raised ammonia levels
- Serious skin reactions, including toxic epidermal necrolysis and Stevens-Johnson syndrome

- Bowel perforation or serious bleeding

- You have kidney or liver problems.
- You have a current or past hepatitis B infection.
- You develop persistent or severe diarrhoea, vomiting, or signs of dehydration.
- You develop confusion, unusual sleepiness, or difficulty staying alert.
- You notice any eye irritation, excess tearing, or vision changes.
- You develop a severe skin rash or blistering.
- You are due to have any vaccination.
Directions for Use
- Take S One S One capsule exactly as prescribed by your doctor.
- Swallow the capsules with water, at least 1 hour before or 1 hour after a meal.
- Your doctor will usually also prescribe anti-sickness and anti-diarrhoeal medicines to have available at home.
- If you vomit after taking a dose, do not take a replacement dose.
- Do not increase your dose again once it has been reduced, unless your doctor specifically advises this.
Storage and disposal
- Store as directed on the product labelling.
- Keep out of the reach of children and pets.
- Wash your hands after handling the capsules.
- This is a hazardous cytotoxic medicine; return any unused or expired capsules to your pharmacy for safe disposal.
Quick Tips
- Stay well hydrated throughout treatment, and contact your doctor promptly if you develop diarrhoea, especially if it is frequent or occurs at night.
- If you develop a fever or symptoms of infection, contact your doctor immediately. Do not stop treatment unless your doctor advises you to do so.
- Contact your doctor promptly if vomiting is persistent or you are unable to keep fluids down.
- Report any eye irritation, excess tearing, or vision changes to your doctor.
- Never take the antiviral medicine brivudine while on this treatment or during the required interval before starting treatment.

Overdose
- There is no known antidote for overdose with this medicine.
- Reported symptoms of overdose include nausea, vomiting, diarrhoea, mouth sores, gastrointestinal irritation, bleeding, and severe suppression of bone marrow function.
- If an overdose is suspected, seek emergency medical attention immediately.

Interactions
Interactions with other medicines
- Brivudine (an antiviral medicine used for shingles): must never be taken with or shortly before this medicine, as the combination may cause fatal toxicity; a waiting period is required between the two.
- Other fluoropyrimidine medicines (such as capecitabine or fluorouracil): must not be combined with this medicine, as it increases the risk of severe toxicity.
- Coumarin-derivative blood thinners (such as warfarin): this medicine can increase their blood-thinning effect, so your blood clotting tests will need closer monitoring.
- Phenytoin (used for seizures): this medicine can raise phenytoin levels in the blood, so your doctor will need to monitor you more closely.
- Folinic acid, metronidazole, methotrexate, clozapine, and cimetidine: may increase the toxicity of this medicine; your doctor will use caution if these are needed together.
- Allopurinol (used for gout): may reduce the effectiveness of fluoropyrimidine treatment and should generally be avoided unless specifically advised by your doctor.
- Tell your doctor about all other prescription medicines, over-the-counter medicines, and herbal supplements you are taking.
