Medical Description

Rotavac is an oral vaccine that helps protect infants from rotavirus infection, a common cause of severe stomach flu (gastroenteritis). It contains a live, weakened form of rotavirus.

Rotavirus commonly causes severe diarrhoea in babies and young children. It spreads very easily, and good hygiene alone does not prevent it. The illness causes watery diarrhoea and vomiting, often with fever, and the real danger is dehydration, which can become serious quickly in a small baby. Vaccination does not prevent every episode, but it helps reduce the risk of severe rotavirus gastroenteritis.

The vaccine contains a live but weakened virus, so it prompts the immune system to respond without causing the illness itself. The vaccine protects only against rotavirus, not against diarrhoea caused by other viruses, bacteria, or parasites.

Side effects may include mild irritability, fever, diarrhoea or vomiting. If the child experiences persistent or worsening symptoms, inform your doctor.

A doctor or nurse gives Rotavac by mouth as drops into the baby's mouth. It must never be injected. It is given as a course of 3 doses, starting at 6 weeks of age, with doses 4 weeks apart. The course should be completed by 8 months (34 weeks) of age. so follow the schedule your doctor or paediatrician sets and do not miss doses. You can give it at the same visit as other routine infant vaccines.

Tell the doctor before vaccination if your baby has a history of intussusception (a condition in which part of the bowel slides into the next part), a bowel malformation that increases the risk of intussusception, severe combined immunodeficiency (SCID), or ongoing gastroenteritis. Also tell the doctor if your baby has a weakened immune system, an acute illness, or lives with someone whose immune system is weakened. Seek medical help immediately if your baby develops repeated vomiting, bouts of severe crying with the legs drawn up, blood in the stool, a swollen stomach, or becomes unusually floppy after vaccination.

Uses

For active immunisation of infants from the age of 6 weeks for the prevention of gastroenteritis due to rotavirus infection.

Contraindications

  • Do not give Rotavac oral vaccine to your baby if they:
  • Hypersensitivity to any component of the vaccine. If symptoms suggestive of hypersensitivity develop after a dose, do not give further doses until the doctor has assessed the reaction.
  • Babies with Severe Combined Immunodeficiency Disease (SCID).
  • History of intussusception (IS)/intestinal malformations predisposing to intussusception.

Side effects

Common
  • Irritability or unsettled behaviour
  • Diarrhoea
  • Vomiting or spitting up
  • fever
Uncommon
  • Skin rash or dermatitis
  • Runny or blocked nose, or a cough
  • Constipation
  • Middle ear inflammation
Serious - seek medical help immediately
  • INTUSSUSCEPTION, a rare but serious blockage of the bowel. Warning signs include bouts of severe crying with the legs drawn up towards the tummy, repeated vomiting, blood or a dark jelly-like substance in the stool, a swollen tummy or a lump that can be felt in it, and the baby becoming pale, drowsy, or unusually floppy. Take your baby to hospital straight away if you see these signs.
  • Severe allergic reaction, with swelling of the face, lips, tongue or throat, difficulty breathing, a widespread rash or collapse
  • Signs of dehydration - very few wet nappies, a dry mouth, no tears when crying, drowsiness, or a sunken soft spot on the head

Precautions and Warnings

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You or your baby has a history of intussusception (a serious intestinal condition).
  • You or your baby has an uncorrected congenital malformation of the gastrointestinal tract that would predispose to intussusception.
  • You or your baby has Severe Combined Immunodeficiency (SCID) disorder.
  • You or your baby is suffering from an acute severe febrile illness (fever).
  • You or your baby is suffering from diarrhoea or vomiting.
  • Your baby has a known or suspected immune deficiency or has been exposed to immunosuppressive treatment before birth.
  • Someone in your household has a weakened immune system, such as due to cancer treatment or immunosuppressive medicines. The doctor will assess the situation before vaccination.
  • Your baby is taking any medicine that suppresses the immune system, or you received immunosuppressive treatment during pregnancy.
  • Your baby has a known severe allergy to any component of the vaccine. Rotavac contains trace amounts of neomycin and kanamycin, so tell the doctor about any previous severe reactions to these antibiotics.
Pregnancy

Pregnancy

Rotavac is a vaccine for babies and is not given to adults, so it is not used in pregnancy.

Breast Feeding

Breast Feeding

Yes. You do not need to stop, delay, or time breastfeeding around the vaccine.. Feed your baby as normal before and after the dose. Rotavac is given to the baby, not to the mother.

Directions for Use

  • A doctor or nurse gives Rotavac. FOR ORAL USE ONLY - IT MUST NEVER BE INJECTED.
  • It is given as drops into the baby's mouth, towards the inside of the cheek, with the baby seated and leaning slightly back. The healthcare professional prepares and gives it; parents do not administer it themselves.
  • It is given as a 3-dose course, starting at 6 weeks of age and spaced 4 weeks apart. Complete the course by 8 months (34 weeks) of age. Follow the schedule your doctor or paediatrician sets, and attend every appointment; an incomplete course provides less protection.
  • You can give it at the same visit as other routine baby vaccines.
  • If your baby spits out or brings up most of the dose, tell the person giving it; A single replacement dose may be given at the same visit at the healthcare professional's discretion.
  • There are no restrictions on feeding before or after vaccination. Your baby can be fed as normal.
  • Tell the doctor before vaccination about any previous reaction, any problem with your baby's immune system, any bowel condition, any current diarrhoea or vomiting, and any significant acute illness.
  • Keep the vaccination record you are given and bring it to each appointment.

Storage and disposal

  • Store the Rotavac oral vaccine in a refrigerator (2 degrees C-8 degrees C).
  • Do not freeze the vaccine.
  • Keep it in its original package to protect it from light.
  • Use the vaccine immediately after opening.
  • Dispose of any unused vaccine or waste materials in accordance with local regulations for biological waste.
  • Follow the product-specific instructions for use after opening. Single-dose and multi-dose presentations may have different requirements.
  • Do not use the vaccine after its expiry date, or if the packaging is damaged or the contents look unusual.

Quick Tips

  • Rotavac oral vaccine is for oral use only. It must never be injected.
  • Store the vaccine correctly (refrigerated, not frozen, and protected from light).
  • The healthcare professional should check the vaccine and its packaging before administration and follow the product-specific instructions if the appearance is unusual.
  • Keep to the schedule. The course has an upper age limit, so a long delay can mean your baby misses out entirely.
  • Learn the warning signs of intussusception before you leave the clinic: bouts of severe crying with the legs drawn up, repeated vomiting, blood in the stool, a swollen tummy, unusual floppiness. Go to hospital straight away if you observe these symptoms.
  • Feed your baby as normal. No fasting is needed before or after the dose.
  • If your baby develops diarrhoea or vomiting, watch for signs of dehydration, such as very few wet nappies, a dry mouth, no tears when crying, or unusual drowsiness. Seek medical advice if these occur.
  • Wash your hands carefully after nappy changes following vaccination, particularly if anyone at home has a weakened immune system.
  • Tell the clinic if your baby was born very prematurely, or has any bowel or immune condition, before the dose is given.
  • The vaccine protects against rotavirus, not against all causes of diarrhoea. Safe water, hand washing and prompt oral rehydration still matter.
  • Keep the vaccination card and bring it to every visit.

Dosage

Overdose

While some cases of overdose have been reported, the side effects observed were generally similar to those seen with the recommended dose of Rotavac oral vaccine. If more than the intended amount is given, or a dose is repeated in error, tell your doctor.Your baby should be assessed and managed according to the circumstances. .

Missed a Dose

If you miss a dose, consult your healthcare professional for guidance. They will advise on the best course of action to ensure your baby receives the vaccine's full protective effect. Contact your doctor promptly rather than waiting for the next routine visit. Complete the course by 8 months (34 weeks) of age, as delays may mean you cannot give the remaining doses. Do not give an extra dose on your own.

How does this medicine work?

Rotavac oral vaccine works by introducing a weakened form of the rotavirus into your baby's system. This helps your baby develop immunity (protection) against the virus without causing the illness. When your baby is later exposed to the real rotavirus, their immune system will be prepared to fight it, reducing the risk of severe rotavirus gastroenteritis.

Interactions

Interactions with other medicines

  • Tell the doctor if your baby has a weakened immune system or is receiving immune-suppressing treatment. Rotavac is a live vaccine, and vaccination in these circumstances needs an individual medical assessment.
  • The vaccine virus is passed in the baby's stool for a period after vaccination. Wash hands carefully after nappy changes. Take particular care if someone in the household has a weakened immune system, and tell the doctor if this applies.
  • Tell your doctor about all medicines and treatments your baby receives. Do not delay vaccination or change the schedule without medical advice.

Interactions with food items

No restrictions apply to your baby's food or liquid intake before or after vaccination. You do not need to stop breastfeeding or time it around the vaccine, and your baby can be fed as normal before and after the dose.

Frequently Asked Questions (FAQs)

Rotavac oral vaccine helps protect infants against rotavirus-caused gastroenteritis. It reduces the risk of severe rotavirus illness but does not prevent diarrhoea caused by every other infection.
It is given as a course of doses, not a single dose, spaced a few weeks apart. Your doctor or paediatrician will tell you how many and when, based on the immunisation schedule used. Completing the course provides full protection.
The course starts at 6 weeks of age and should be completed by 8 months (34 weeks). There is an upper age limit beyond which you cannot complete the course, so stick to the appointments your doctor schedules.
Yes, you can give Rotavac oral vaccine at the same time as many other routine childhood vaccines.
In the unlikely event that your baby spits out or throws up most of the vaccine, a single replacement dose may be given during the same vaccination visit.
Watch for signs of intussusception, a rare bowel blockage, such as severe crying with the legs drawn up, repeated vomiting, blood or dark jelly-like stools, a swollen tummy, paleness, or unusual drowsiness. Seek emergency medical care immediately if these occur. If your baby develops diarrhoea or vomiting, watch for signs of dehydration, such as fewer wet nappies, a dry mouth, no tears when crying, or unusual drowsiness. Contact a doctor promptly if these occur
Rotavac contains a weakened form of rotavirus. It is designed to stimulate protection against rotavirus gastroenteritis. As with other live vaccines, the vaccine virus may be passed in the stool for a period after vaccination. Wash hands carefully after nappy changes, especially if someone in the household has a weakened immune system. Seek medical advice if your baby develops persistent diarrhoea, vomiting or other concerning symptoms.
Not necessarily. The vaccine protects against rotavirus, one of several causes of diarrhoea in babies, and it reduces the risk of severe illness rather than preventing every episode. Follow your doctor's advice, continue feeding, and use oral rehydration solution as advised. Seek medical help if your baby shows signs of dehydration, has persistent vomiting, or will not feed.
