Medical Description
Isolazine Tablet is used in the treatment of heart failure, added to standard therapy to relieve symptoms and improve outcomes. It is especially useful for people who cannot take the usual first-line medicines because of kidney impairment or raised potassium.
It contains isosorbide dinitrate and hydralazine, which reduce the workload of a failing heart from two directions. The nitrate widens veins so less blood returns to the heart, while hydralazine widens arteries so the heart pumps against less resistance.
Take it exactly as prescribed, spread through the day, and never stop suddenly, as fluid can build up and chest pain may be provoked. Weigh yourself each morning and report a gain of two kilograms or more over two to three days.
Headache and dizziness on standing are common, arising directly from the widening of blood vessels, and usually ease over the first weeks. Seek advice for joint pain with fever and rash, which may indicate a rare lupus-like reaction to hydralazine.
Never take sildenafil, tadalafil, vardenafil or riociguat with this medicine; the combination can cause a catastrophic drop in blood pressure. Avoid alcohol, rise slowly, and tell every doctor treating you that you take a nitrate.
Some other medicines with a combination of hydralazine and isosorbide dinitrate include Nitrohart tablet, Apresol Plus tablet and Isorus tablet.

Contraindications
- If you are allergic to Isolazine tablets or their other ingredients.
- It should not be used in patients taking medicines containing sildenafil, avanafil, tadalafil or vardenafil (used for treating erectile dysfunction in men).
- If you are taking a medicine called riociguat which is used to treat high blood pressure in the lungs (pulmonary hypertension).

- You have heart-related problems.
- You have severe kidney or liver problems.
- You are having any surgery.
- You have glaucoma (increased pressure in the eye).
- You are undernourished.
- You are suffering from hypothermia (a condition in which your body loses heat faster than it can produce).
- You have suffered from bleeding in the brain.
- You have an underactive thyroid gland.
- You experience headache, nausea, dizziness, fainting (symptomatic hypotension).
- You experience any abnormal sensation on your skin such as numbness and tingling.
- You have systemic lupus erythematosus (an autoimmune disorder), this medicine may induce this condition.
- You suffer from a deficiency of an enzyme called glucose 6 phosphate dehydrogenase.
Directions for Use
- Take the Isolazine tablets as directed by your doctor.
- Swallow it as a whole with water.
- Do not chew or break the tablet.
- To get the optimal results take it at a fixed time.

Interactions with food items
- Isolazine Tablet may be taken with or without food, but keep the pattern consistent from day to day so the effect remains steady. Taking it with food can reduce nausea.
- Alcohol should be avoided or kept minimal, as it adds substantially to dizziness and the risk of fainting and falls.
- Reduce salt, which is one of the most effective dietary measures in heart failure. Limit pickles, papads, packaged snacks, processed meats and restaurant food, and avoid adding salt at the table.
- Follow any daily fluid limit your doctor has set, and weigh yourself every morning to catch fluid retention early.
- Rise slowly after meals, as blood pressure can dip further during digestion, particularly after a large meal.
- If you develop tingling in the hands or feet, ask your doctor about vitamin B6, as hydralazine can interfere with it.
