Medical Description

Wofungin dry powder for injection is an antifungal medicine given in hospital to treat serious infections caused by Candida or Aspergillus. Its active ingredient is caspofungin acetate, which provides caspofungin to act against these fungi. Caspofungin blocks formation of a material that fungi need to keep their cell wall strong, so fungal cells become fragile and cannot grow normally. Wofungin injection is prepared and given into a vein by a doctor or nurse in a hospital or clinic. It is used under close medical supervision for the patient's infection and overall condition.

Uses

  • Serious Candida infection: It treats invasive infection in the bloodstream or organs such as the heart, brain, eyes, bones or lungs, which needs hospital care.
  • Invasive aspergillosis: It treats serious Aspergillus infection when other antifungal treatment has not worked well or is not suitable.
  • Serious fungal infection in weakened immunity: It may be used when a weakened immune system, recent surgery, chemotherapy or transplantation raises concern for a serious fungal infection.

Contraindications

If you are allergic to any ingredient of this medicine, do not use it. Tell your doctor if you have liver disease, an allergy to other antifungal medicines, or a history of reactions during intravenous treatment. This medicine must be used under specialist supervision.

Side effects

  • Fever or chills: An infusion reaction may cause fever, chills or flushing during or after treatment. Tell the nurse immediately if these symptoms are severe.
  • Nausea: Caspofungin may upset the stomach during treatment. Report persistent vomiting so that dehydration can be prevented.
  • Headache: Changes during treatment may cause headache. Tell your care team if it is severe, sudden or associated with vision changes.
  • Injection-site reaction: Pain, redness or swelling may occur where the medicine enters the vein. Report spreading redness or severe pain.
  • Allergic reaction: Wheezing, breathing difficulty, facial swelling or a widespread rash may occur during infusion. Get emergency medical help immediately.

Precautions and Warnings

Pregnancy

Pregnancy

Tell your doctor if you are pregnant, planning pregnancy or could become pregnant. Caspofungin is used during pregnancy only when the specialist considers the expected benefit greater than the possible risk.

Breast Feeding

Breast Feeding

Tell your doctor if you are breastfeeding. It is not known whether caspofungin passes into human milk, so your doctor will decide whether breastfeeding should continue.

Driving

Driving

Wofungin injection may be followed by dizziness or weakness in some people. Do not drive or operate machinery if you feel affected.

Alcohol

Alcohol

No specific alcohol interaction is established. Avoid alcohol if you feel dizzy or unwell during treatment.

Directions for Use

This injection will be given by a doctor or nurse in a hospital or clinic. Do not self-inject. It is given into a vein after preparation by trained healthcare staff. Your doctor will decide the treatment plan based on the type and severity of infection, your age, body weight and liver function.

Storage and disposal

Wofungin injection must be stored and prepared by the hospital or clinic according to the product instructions. Do not keep or use a partly prepared vial at home. Healthcare staff will dispose of unused medicine safely.

Quick Tips

  • Attend every hospital appointment arranged for Wofungin treatment.
  • Tell the care team about previous allergies to antifungal medicines.
  • Report chills, flushing, breathing difficulty or facial swelling during the infusion immediately.
  • Tell your doctor about liver disease and every medicine or supplement you use.
  • Do not attempt to store, prepare or inject this medicine yourself.

Dosage

Overdose

Wofungin injection is given by healthcare staff, so an overdose is unlikely. If too much is administered, the team will monitor you for adverse effects. If you think you have taken too much of this medicine, contact your doctor immediately or visit the nearest hospital.

Missed a Dose

If you miss a hospital appointment for Wofungin injection, contact the treating team for instructions. If you missed any dose of medicine, take it as soon as you remember. If it is time for your next dose, then skip the missed dose and continue with your regular dose schedule. Do not take a double dose of medicine to compensate for the missed one.

How does this medicine work?

Caspofungin, the active part of caspofungin acetate, blocks production of a structural material that fungi need to maintain their cell wall. Without this material, fungal cells become weak and fragile and are destroyed or unable to grow.

Interactions

Interactions with other medicines

Some medicines can affect the way Wofungin injection works. Tell your doctor about all the medicines, vitamins and supplements you take. In particular, rifampicin, phenytoin, carbamazepine, efavirenz, nevirapine and corticosteroids such as dexamethasone may lower caspofungin levels; ciclosporin may increase exposure and require liver-related monitoring; tacrolimus levels may fall; and other antifungal medicines may require a treatment review.

Interactions with food items

No specific food interaction is expected because Wofungin injection is given into a vein. Tell your doctor about vitamins, supplements and herbal products you use.
