Medical Description

Lucal Injection contains calcium leucovorin (calcium folinate), a derivative of folic acid used as a protective agent and rescue therapy in cancer treatment. It works by supplying active folate, and can bypass metabolic blocks caused by certain folate-antagonist medicines. Lucal Injection helps reduce severe toxicity following high-dose methotrexate therapy and enhances the anticancer effect of fluorouracil (5-FU) during chemotherapy. Common side effects of Lucal Injection include nausea, vomiting, diarrhea, stomach discomfort, fatigue, skin redness, rash, and itching. Contact your doctor immediately if you experience signs of an allergic reaction (such as a severe skin rash, itching, hives, or swelling of the face, lips, tongue, or throat), sudden shortness of breath, wheezing, severe dizziness, or fainting. Lucal Injection is administered by healthcare professionals via intravenous (IV) infusion, intravenous injection, or intramuscular (IM) injection, as specifically prescribed by your doctor, according to your chemotherapy schedule.

Uses

Used alone or with other drugs to treat certain types of colorectal cancer and anemia and to lessen the toxic effects of the anticancer drug methotrexate or other substances that block the action of folic acid.

Contraindications

  • Lucal Injection should not be used:
  • If you have a known severe hypersensitivity or systemic allergic reaction to calcium leucovorin, folinic acid, or any of its ingredients.
  • For the treatment of pernicious anemia or other megaloblastic anemias caused by Vitamin B12 deficiency (as it may temporarily improve anemia while allowing neurological damage to progress).

Side effects

  • Common Side Effects
  • Nausea and vomiting
  • Diarrhea or loose stools
  • Stomach discomfort
  • Tiredness and fatigue
  • Mild fever or warmth
  • Skin redness, rash, or mild itching
  • Trouble sleeping (insomnia)
  • Agitation or mild restlessness
  • Serious Side Effects (Seek Immediate Medical Care)
  • Severe Allergic Reaction (Anaphylaxis): Severe skin rash, hives, swelling of the face, lips, tongue, or throat, low blood pressure, severe dizziness, or sudden trouble breathing.
  • Severe Gastrointestinal Toxicity (When combined with 5-Fluorouracil): Severe, uncontrolled diarrhea, severe mouth sores (mucositis), or dehydration. Calcium leucovorin can increase the toxicity of 5-fluorouracil.
  • Seizures: Calcium leucovorin may reduce blood concentrations of certain anticonvulsants, including phenytoin, phenobarbital, and primidone, potentially increasing seizure frequency.

Precautions and Warnings

Pregnancy

Pregnancy

Lucal Injection itself is not known to harm the unborn baby. However, it is often used with medicines that may be harmful during pregnancy. Your doctor will carefully assess the benefits and risks of the full treatment before administering it.

Breast Feeding

Breast Feeding

It is not known whether calcium leucovorin passes into human milk. Because it is typically given in conjunction with cytotoxic chemotherapy agents, breastfeeding should be discontinued during treatment as advised by your oncologist.

Driving

Driving

There is no evidence that calcium leucovorin itself significantly affects the ability to drive or operate machinery. However, avoid driving if you experience dizziness, fatigue, nausea, or other symptoms that may impair your ability to drive safely.

Alcohol

Alcohol

There is no specific established alcohol interaction with calcium leucovorin. However, alcohol may worsen nausea or other side effects associated with chemotherapy. Discuss alcohol consumption with your doctor.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • Have a history of allergic reactions to leucovorin or folinic acid.
  • Have pre-existing seizure disorders or are taking anti-seizure medications like phenytoin.
  • Experience severe diarrhea or painful mouth inflammation after receiving chemotherapy with leucovorin.
  • Have impaired kidney function, as delayed methotrexate clearance requires adjusted leucovorin dosages and prolonged monitoring.
  • Are receiving methotrexate or 5-fluorouracil, as calcium leucovorin should be used under appropriate specialist supervision.

Directions for Use

  • Lucal Injection must be administered exclusively by qualified healthcare personnel in a clinical or hospital setting.
  • It is given by intravenous infusion, intravenous injection, or intramuscular injection according to the prescribed treatment protocol. It must not be administered intrathecally.
  • Do not self-administer this medication.

Storage and disposal

Store vials according to the manufacturer's specified storage conditions. Healthcare personnel dispose of unused solution or single-dose vials following standard biomedical waste procedures.

Quick Tips

  • Follow the prescribed schedule carefully, as the timing of calcium leucovorin relative to methotrexate is important for effective rescue therapy.
  • Report severe diarrhoea, mouth sores, or difficulty maintaining fluid intake promptly.
  • Attend scheduled blood tests, particularly when Lucal Injection is being used for methotrexate rescue.

Dosage

Overdose

Because Lucal Injection is calculated and administered by healthcare professionals along chemotherapy protocols, overdose is uncommon. Excessive doses of leucovorin may reduce the therapeutic effect of methotrexate or increase the toxicity of 5-fluorouracil. Supportive treatment and appropriate protocol adjustments should be provided if an overdose occurs.

Missed a Dose

Because Lucal Injection timing is critical, especially when acting as a rescue agent to prevent methotrexate toxicity, report any delayed or missed administration to the attending oncology team immediately for emergency review and dose rescheduling.

Interactions

Interactions with other medicines

  • Phenytoin, phenobarbital, and primidone: May reduce the effectiveness of these medicines and increase the risk of seizures.
  • 5-Fluorouracil (5-FU): Can increase both its anticancer effect and toxicity, so close monitoring is needed.
  • Trimethoprim, co-trimoxazole, and pyrimethamine: May have their effects reduced or neutralised.
  • Tell your doctor about all prescription medicines, over-the-counter medicines, supplements, and herbal products you take.

Interactions with food items

Since Lucal Injection is administered parenterally (intravenous or intramuscular injection), food intake does not impact drug absorption.

Frequently Asked Questions (FAQs)

No. Lucal Injection is not a cytotoxic chemotherapy drug. It is a protective agent (chemoprotectant) used either to rescue healthy cells from the toxic effects of chemotherapy like methotrexate or to enhance the effectiveness of chemotherapy like 5-fluorouracil.
Calcium leucovorin rescue must be administered according to the methotrexate treatment protocol. Delays can reduce its ability to counteract methotrexate toxicity.
No. Lucal Injection must NEVER be administered intrathecally. Intrathecal administration of leucovorin is hazardous and can result in severe neurological harm or fatal outcomes.
Calcium leucovorin does not generally interfere with routine blood tests. However, its use during high-dose methotrexate therapy requires monitoring of blood counts, kidney function, and methotrexate concentrations according to the treatment protocol.
