Medical Description

Esoz D 40 capsule is an anti-reflux medicine. Esoz D contains a combination of two esomeprazole and domperidone as its active ingredients. It is used to treat gastroesophageal reflux disease (GERD), which does not respond to esomeprazole medication alone. GERD is a condition in which acid from the stomach comes back up to the food pipe and causes heartburn, belching, nausea, and chest pain. It works by decreasing the amount of acid produced in the stomach and improving mortality in the gut. Thus, it reduces the backflow of stomach content to the food pipe. Take this medicine as per the prescribed dose and duration. It should preferably be taken at least an hour before the meal. Do not stop taking this medicine without informing your doctor.

Uses

Esoz D 40 capsule is used in adult patients to treat gastroesophageal reflux disease (GERD) in which there is a reflux of acid from the stomach into the food pipe, causing heartburn, chest discomfort, and acidity. It is useful when GERD is not responding to esomeprazole medicine alone.

Contraindications

  • If you are allergic to esomeprazole, domperidone or other ingredients of Esoz D 40 capsule.
  • If you have a blockage or tear in the intestines.
  • If you suffer from liver or kidney disorder.
  • If you suffer from bleeding in the digestive system.
  • If you suffer from a tumour of the pituitary gland (Prolactin pituitary tumours).
  • If you suffer from heart problems like congestive heart failure, heart rhythm problems, electrolyte imbalance.
  • If you are using a medicine that might affect the contraction of the heart muscle (QT prolongation).
  • If you are using medicines like Nelfinavir (used in HIV infection), Clarithromycin (antibiotic), Ketoconazole (antifungal) or Verapamil (used in high blood pressure and chest pain).

Breast Feeding

Driving

Other General Warnings

Talk to your doctor if
  • You have liver and kidney-related diseases.
  • You have a specific blood test (Chromogranin A).
  • You develop black stools or have a stomach tumour.
  • You experience fractures or have osteoporosis or taking steroids.
  • You develop skin lesions, especially in the sun-exposed area after taking the medicine.
  • You are using medicines to treat HIV or apomorphine (used to treat muscle stiffness).
  • You have heart problems and symptoms like palpitations, difficulty in breathing and unconsciousness.
  • You experience stomach pain or indigestion, unexplained weight loss, have problems while swallowing, vomit out food or blood after taking the medicine.
  • You experience low levels of vitamin B12. The medicine reduces the absorption of vitamin B12 after long term use. Supplements may be needed.
  • You are using capsules for long-term treatment (more than three months), the blood magnesium levels may decrease. The doctor may perform regular tests for the same.
  • It is recommended for patients below the age of 18 years.
  • Taking the medicine for more than one year may increase the chances of developing bone fractures.
  • The medicine leads to a slightly increased risk of bacterial infections due to Salmonella and Campylobacter.

Directions for Use

It must be taken as directed by the doctor. Swallow it as a whole with water, an hour before the meals. These capsules must not be opened, chewed, or crushed. Take the medicine at the same time every day and as directed by the doctor. Avoid consuming it more than the prescribed dose.

Storage and disposal

Store below 25°C at a clean and dry place, protected from direct sunlight, moisture and heat. Keep the medicine away from children and pets.

Overdose

An overdose causes nausea, vomiting, indigestion, diarrhoea, constipation, weakness, confusion, agitation, anxiety, fits, altered consciousness, sleepiness, etc. In case of an overdose, contact the doctor or visit the nearest hospital.

Missed a Dose

If you have missed taking a dose of Esoz D 40 capsule, take it as soon as you remember. If it's time for the next dose, skip the missed dose and continue with the regular dose schedule. Avoid taking a double dose to compensate for the missed one. Skipping doses or not finishing the full course of therapy may reduce the effectiveness of the treatment.

How Does It Work?

  • A pump called the proton pump in the stomach is responsible for the flow of stomach acids.
  • Esomeprazole binds to the proton pump in stomach cells and blocks the acid secretion.
  • Domperidone increases the motility of the stomach and intestines so that acid does not reflux back into the food pipe.

Interactions

Interactions with other medicines

  • Some medicines affect the way the Esoz D 40 capsule works or the medicine itself reduces the effectiveness of other medicines taken at the same time.
  • Inform the doctor regarding the medicines, supplements or herbals you take or might take to avoid possible interaction.
  • The medicine must not be taken concomitantly along with medicines like Nelfinavir (used to treat HIV infection), Amiodarone (used to treat muscle stiffness), Clarithromycin (antibiotic), Ketoconazole (antifungal) or Verapamil (used in high blood pressure, chest pain).
  • The capsule must also not be taken along with medicines used to treat heart problems (Quinidine, Sotalol), psychiatric illness (Haloperidol, Citalopram), antifungals (Pentamidine), antimalarials (Halofantrine, Lumefantrine), medicines used to treat allergies (Mizolastine), medicines to treat in cancer (Toremifene, Vandetanib), gastric emptying medicines (Cisapride) and other medicines like Diphemanil and Methadone.
  • Concomitant use of the capsule with medicines used to treat psychiatric illness (Diazepam), used to treat fits (Phenytoin) is not recommended as it may increase the side effects of the medicines.
  • Medicine used to treat heart failure like Digoxin may lead to toxicity when taken with the medicine.
  • If you are using Warfarin (blood-thinner) along with the medicine, caution is needed as there are chances of increased bleeding.
  • Other medicines like Rifampicin, Methotrexate, Tacrolimus, Voriconazole, Cilostazol, Clopidogrel, Azithromycin, St John's wort, etc must be used cautiously along with Esoz D 40 capsule.
