Medical Description

Zisavel Dry powder for injection is an antifungal medicine used in hospital treatment of serious invasive fungal infections, including invasive aspergillosis and mucormycosis. It contains isavuconazole, which prevents fungi from making ergosterol, a substance needed for a stable fungal cell covering. This stops the fungi from growing and allows the infection to be controlled. The powder is prepared and given into a vein by a doctor or nurse in a hospital or clinic, especially when a person has weakened immunity.

Uses

  • Invasive aspergillosis: Isavuconazole helps treat a serious fungal infection that can spread from the lungs to other organs, especially in people with weakened immunity.
  • Mucormycosis: Isavuconazole helps treat this severe fungal infection under specialist hospital care when the infection affects vulnerable patients.

Contraindications

If you are allergic to any ingredient of this medicine, do not use it. Ranolazine and pimozide should not be used with this medicine. Do not use it if you have familial short QT syndrome. Tell your doctor if you have liver disease, a heart rhythm problem or a previous allergy to antifungal medicines.

Side effects

  • Liver-related changes: Isavuconazole can change liver tests. Yellowing of the skin or eyes, dark urine or upper stomach pain needs urgent medical attention.
  • Digestive upset: Nausea, vomiting, diarrhoea or constipation may occur during treatment. Tell the treating team if you cannot keep fluids down.
  • Headache: Headache may occur during treatment. Tell the nurse if it is severe, persistent or accompanied by vision changes.
  • Low potassium: Reduced potassium may cause weakness, cramps or palpitations and may need blood-test monitoring. Tell the clinician about these symptoms.
  • Breathlessness, cough or swelling: These symptoms or back pain can occur and may need prompt assessment. Get emergency medical help for breathing difficulty or swelling of the face, lips or throat.

Precautions and Warnings

Pregnancy

Pregnancy

Pregnancy safety has not been established. Tell your doctor if you are pregnant, planning pregnancy or could become pregnant so the risks and benefits can be assessed.

Breast Feeding

Breast Feeding

It is not known whether isavuconazole passes into human milk. Ask your doctor whether breastfeeding should continue during treatment.

Driving

Driving

This medicine may cause dizziness, sleepiness or changes in vision. Do not drive if any of these occur.

Alcohol

Alcohol

Ask your doctor before drinking alcohol during treatment.

Directions for Use

This injection will be given by a doctor or nurse in a hospital or clinic. Do not self-inject. Healthcare staff will prepare the powder and give the solution into a vein exactly as prescribed. Complete the treatment plan unless the treating doctor changes it.

Storage and disposal

Store the unopened powder as directed by the hospital or pack instructions and protect it from unsuitable heat and light. Prepared solution must be handled and discarded by healthcare staff.

Quick Tips

  • Tell the treating team about every medicine, vitamin and supplement before the injection.
  • Report new breathlessness, severe dizziness, fainting or a fast or irregular heartbeat promptly.
  • Tell your doctor about liver disease, kidney disease or previous antifungal allergy.
  • Do not stop the prescribed treatment without speaking with the treating team.

Dosage

Overdose

An excessive amount may cause marked nausea, vomiting, dizziness, confusion or heart rhythm problems. If you think you have taken too much of this medicine, contact your doctor immediately or visit the nearest hospital.

Missed a Dose

If you missed any dose of medicine, take it as soon as you remember. If it is time for your next dose, then skip the missed dose and continue with your regular dose schedule. Do not take a double dose of medicine to compensate for the missed one.

How does this medicine work?

Isavuconazole blocks a fungal enzyme needed to make ergosterol, a substance that helps maintain the fungal cell covering. Without enough ergosterol, the fungus cannot grow normally and the infection can be controlled.

Interactions

Interactions with other medicines

Some medicines can affect the way Zisavel works. Tell your doctor about all the medicines, vitamins and supplements you take. In particular, ranolazine and pimozide should not be used with it; fosphenytoin and nevirapine can lower isavuconazole levels; and vinblastine levels may rise. Other medicines that strongly affect liver enzymes, including some seizure or transplant medicines, need specialist review.

Interactions with food items

Tell the treating team about all herbal products and supplements. Food does not remove the need for careful monitoring of this medicine.
