Medical Description

Metital tablet is a combination of metformin and myo-inositol used to manage polycystic ovary syndrome (PCOS) in women, particularly those who exhibit insulin resistance.

Insulin resistance is a common characteristic of PCOS and can lead to irregular menstrual cycles, absent ovulation, and elevated androgen levels. By enhancing the body's response to insulin, Metital tablet helps tackle this underlying metabolic issue, ultimately leading to more regular menstrual cycles and improved ovulation.

Metital tablet is a prescription medication and should be taken under the supervision of a healthcare professional. Improvements in menstrual regularity are typically gradual and may take three to six months of continuous treatment. Your doctor may recommend periodic blood tests to monitor kidney function and vitamin B12 levels during long-term use.

Uses

  • Metital tablet is used in the management of polycystic ovary syndrome (PCOS) in women with insulin resistance. It improves the body's sensitivity to insulin and helps regulate glucose metabolism.
  • Helps restore regular menstrual cycles. Supports ovulation and ovarian function. It is intended to be used alongside dietary changes, regular exercise, and weight management, as advised by your doctor.

Contraindications

  • Metital tablet should not be taken if you:
  • Are allergic to metformin, myo-inositol, or any other ingredient of Metital tablet.
  • Have severe kidney disease or reduced kidney function.
  • Have diabetic ketoacidosis or diabetic pre-coma.
  • Have any acute condition that can affect kidney function, such as severe dehydration, severe infection, or shock.
  • Have a condition that reduces oxygen supply to the tissues, such as decompensated heart failure, respiratory failure, a recent heart attack, or shock.
  • Have severe liver disease.
  • Have acute alcohol intoxication or alcohol dependence.

Side effects

Most side effects are gastrointestinal and occur when starting treatment, and they settle as your body adjusts. Taking the tablet with food helps reduce them. Common side effects:
  • Diarrhoea
  • Nausea and vomiting
  • Abdominal pain or stomach discomfort
  • Loss of appetite
  • Metallic taste in the mouth
  • Bloating or flatulence

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You have kidney disease or reduced kidney function. Your doctor will check your kidney function before starting treatment and at least once a year afterwards, or more often if needed.
  • You have liver disease.
  • You have heart failure or have had a recent heart attack.
  • You are scheduled for surgery under general, spinal, or epidural anaesthesia. Metital tablet usually needs to be stopped before the procedure and restarted only after your kidney function has been reassessed.
  • You are due to have a scan or X-ray involving contrast dye.
  • You develop a serious infection, severe diarrhoea or vomiting, or become dehydrated, as these can affect kidney function. Contact your doctor, as treatment may need to be paused.
  • You drink alcohol regularly or heavily.
  • You have thyroid problems, as thyroid disorders can cause symptoms similar to PCOS.
  • You are taking this medicine long-term, as vitamin B12 levels may need to be monitored.
  • You develop any allergic reaction after taking Metital tablet.
  • You do not notice any improvement in your symptoms. Menstrual regularity often takes three to six months to improve.
  • Do not use Metital tablet for any condition other than that prescribed by your doctor, and do not share it with anyone else.

Directions for Use

Take Metital tablet daily with or right after food, as directed by your doctor. Swallow whole with a glass of water. Do not crush, chew, or break the tablet. Take it at the same time each day. Continue taking it even if you feel well. Do not stop or change the dose without talking to your doctor. Metital tablet works best when combined with a balanced diet, regular exercise, and weight management.

Storage and disposal

Store in a cool, dry place at a temperature below 30°C, away from direct sunlight.

Quick Tips

  • Drink plenty of water and consume fibre-rich foods to alleviate constipation.
  • If side effects persist or worsen, consult a healthcare professional.
  • Do not exceed the recommended dose unless advised by a doctor.
  • Use under medical supervision.
  • Keep out of reach of children.

Dosage

Overdose

Taking too much Metital tablet can cause lactic acidosis, a rare but serious medical emergency. Symptoms may include severe nausea and vomiting, stomach pain, muscle cramps or other unusual muscle pain, rapid or deep breathing, severe weakness, feeling cold, and dizziness or drowsiness. If you or someone else has taken too much Metital tablet, seek emergency medical attention immediately or go to the nearest hospital. Take the medicine pack with you.

Missed a Dose

If you missed a dose of this medicine, take it as soon as you remember. However, if it's time for your next dose, then skip the missed dose and continue with your regular dose schedule. Do not take a double dose to compensate for the missed one.

How does this medicine work?

  • Metital tablet contains two agents that improve insulin sensitivity through complementary mechanisms.
  • Metformin reduces glucose production by the liver, decreases intestinal glucose absorption, and increases the body's response to insulin. This lowers circulating insulin levels.
  • Myo-inositol acts as a second messenger in insulin signalling inside the cell. In women with PCOS, improving this signalling pathway supports ovarian function and follicular development.
  • Together, they help reduce elevated insulin levels and reduce excess ovarian androgen production. This supports hormonal balance, more regular menstrual cycles, and ovulation.

Interactions

Interactions with other medicines

  • Some medicines can affect how Metital tablet works, and Metital tablet can affect how other medicines work.
  • Inform your doctor about all medicines, supplements, and herbal products you are taking or plan to take.
  • Particular care is needed with iodinated contrast dye used in CT scans, angiography and some X-rays. Metital tablet should be stopped before the procedure and for 48 hours afterwards, as the combination can affect kidney function and increase the risk of lactic acidosis.
  • Particular care is needed with medicines that can affect kidney function, including painkillers such as ibuprofen and diclofenac, blood pressure medicines such as ACE inhibitors and ARBs, and diuretics (water pills).
  • Particular care is needed with insulin and other antidiabetic medicines such as sulfonylureas, which increase the risk of low blood sugar.
  • Particular care is needed with corticosteroids, salbutamol and similar medicines, and some decongestants, which can raise blood glucose levels.
  • Particular care is needed with alcohol, which increases the risk of lactic acidosis.
  • Particular care is needed with cimetidine, dolutegravir, ranolazine and certain other medicines that compete with metformin for removal by the kidneys.
  • Tell your doctor or dentist that you are taking Metital tablet before any surgery, or before any scan involving contrast dye.
