Medical Description

Met Xl Trio tablet is used to treat high blood pressure (hypertension) when a single medication or dual therapy is not sufficient to control it effectively. It comprises three antihypertensive medications: metoprolol, cilnidipine, and telmisartan.

Metoprolol is a cardioselective beta-blocker that slows the heart rate and reduces the force of heart muscle contractions, lowering blood pressure and decreasing the heart's oxygen demand. Cilnidipine is a unique calcium channel blocker that not only relaxes and widens blood vessels but also blocks sympathetic nerve activity, preventing stress-induced spikes in blood pressure. Telmisartan is an angiotensin II receptor blocker (ARB) that prevents blood vessel narrowing by blocking the natural chemical angiotensin II, leading to smooth muscle relaxation and lower blood pressure.

Met Xl Trio tablet should be taken precisely as instructed by your doctor. Avoid missing any doses or discontinuing the medication without consulting your physician, as sudden stoppage can cause serious heart complications.

Uses

  • Met Xl Trio tablet is used to treat uncontrolled essential hypertension (high blood pressure).
  • It is also indicated for the management of stable ischemic heart disease.

Contraindications

  • You are allergic to metoprolol, cilnidipine, telmisartan, or any other ingredient in this medicine.
  • You have severe heart problems, such as sinus bradycardia (heart rate less than 45 beats per minute), second- or third-degree heart block, cardiogenic shock, or uncontrolled heart failure.
  • You have an obstruction of blood outflow from the left ventricle of the heart (such as high-grade aortic stenosis).
  • You have low blood pressure (hypotension).
  • You have reduced blood supply to your limbs (severe peripheral arterial circulatory disorders).
  • You have severe asthma or a history of severe bronchospasm or chronic obstructive pulmonary disease (COPD).
  • You have severe liver disease or a blockage in the ducts that carry bile from your liver to your gallbladder (obstructive biliary disorders).
  • You experience difficulty passing urine or are completely unable to urinate (anuria).
  • You have diabetes associated with frequent episodes of low blood sugar (hypoglycemia).
  • You have diabetes or impaired kidney function (GFR < 60 mL/min/1.73 m²) and are currently taking aliskiren.
  • You have metabolic acidosis (increased acidity in your blood) or are taking medications that stimulate heart contractions through beta-receptors.
  • You have an untreated adrenal gland tumor (phaeochromocytoma).
  • You are in the second or third trimester of pregnancy.

Alcohol

Alcohol consumption, along with Met Xl Trio tablet, can lower your blood pressure and can increase side effects like sleepiness, dizziness or drowsiness. It is advised not to consume alcohol during the treatment.

Talk to your doctor if
  • You have kidney disease, have had a kidney transplant, or have mild-to-moderate liver disease (severe liver disease is contraindicated).
  • You have known narrowing of the blood vessels supplying one or both kidneys (renal artery stenosis).
  • You experience dehydration, vomiting, or diarrhea, which can cause your blood pressure to drop too low. Ensure you stay properly hydrated.
  • You have a circulation problem in your arms, legs, fingers, or toes (peripheral vascular disease), as beta-blockers can aggravate symptoms of poor blood flow.
  • Do not stop taking Met Xl Trio tablet abruptly without your doctor's advice. Sudden discontinuation, especially if you have ischemic heart disease, can cause severe chest pain (angina) or even a heart attack. If the medication needs to be stopped, your doctor will reduce the dose gradually over 1 to 2 weeks.
  • Inform your doctor, surgeon, or dentist that you are taking this medication before undergoing any surgical procedure or general anesthesia.
  • Consult your doctor immediately if you have heart failure and experience warning signs of worsening heart failure, such as rapid weight gain or increasing shortness of breath.
  • Report to your doctor immediately if you develop sudden, severe abdominal pain, nausea, vomiting, or persistent diarrhea, as this may indicate a condition called intestinal angioedema.
  • If you have diabetes or an overactive thyroid gland (hyperthyroidism), this medicine may hide certain warning signs, like a fast heartbeat (tachycardia). However, other warning signs of low blood sugar, such as sweating, are usually not affected.

Directions for Use

  • Take Met Xl Trio tablet exactly as directed by your doctor.
  • Swallow the tablet whole with a glass of water; do not chew, crush, or break it.
  • Take it preferably with or immediately following meals.
  • Always take it at the same time each day for the best results.
  • Never change the dose or stop taking this medicine abruptly on your own.

Storage and disposal

  • Store Met Xl Trio tablet at a temperature not exceeding 30°C.
  • Keep it protected from light and moisture, preferably stored in its original Alu-Alu blister packaging.
  • Ensure it is kept out of the sight and reach of children.
  • Do not use this medication after the expiry date mentioned on the package.

  • Take Met Xl Trio tablet at the exact same time every day, preferably with or immediately after a meal.
  • Never stop taking this medicine suddenly on your own. Doing so can cause a dangerous spike in blood pressure, severe chest pain, or heart complications.
  • In addition to taking your medication on time, make healthy lifestyle changes: eat a low-salt, low-fat diet; maintain a healthy weight; exercise moderately for 30 minutes daily (as advised by your doctor); quit smoking; and avoid alcohol.
  • If you feel dizzy or lightheaded when standing up (orthostatic hypotension), get up slowly from a sitting or lying position.
  • Stay well-hydrated; if you suffer from severe diarrhea or vomiting, consult your doctor, as dehydration can cause your blood pressure to drop too low.

  • Symptoms of an overdose may include severe low blood pressure (hypotension), extremely slow heartbeat (severe bradycardia), dizziness, difficulty breathing or wheezing (bronchospasm), irregular heart rhythms (AV block), heart failure, cardiogenic shock, loss of consciousness, or coma.
  • In the event of an overdose, contact your doctor immediately or visit the nearest hospital emergency department.

Missed a Dose

If you miss a dose of this medicine, take only your next scheduled dose at the regular time. Do not take a double dose to make up for a missed one. Do not interrupt or stop your dosing schedule without consulting your doctor.

Interactions

Interactions with other medicines

  • Inform your doctor about all the medications, supplements, or herbal products you are taking to avoid any potential interactions.
  • Especially if you are taking medicines to treat high blood pressure, heart failure, high cholesterol levels, anticoagulants, antifungals, antidiabetics, antihistamines, analgesics, immunomodulators, or medicines for brain-related disorders.
  • Do not take it with medications such as Amiodarone or Disopyramide, or with painkillers such as Indomethacin, Sulindac, Diclofenac, or Focetan, as these can decrease the effect of Met Xl Trio tablet.
  • If you have diabetes, dose adjustment may be needed as this medicine can cause a drop in blood glucose levels, and close glucose monitoring is advised.
  • Medicines used to treat high blood pressure and benign prostatic hyperplasia (Prazosine, Tamsulosin, Terazosin, and Doxazosin)
  • If you have diabetes and you are on Aliskiren, do not take this medicine.
  • Caution is needed if you are taking Fluoxetine, Lithium, Paroxetine, Bupropion, Thioridazine (medicines for psychiatric illness), Diphenhydramine, Hydroxychloroquine, and Mefloquine.
  • Medicines used to treat acidity or ulcers (antacids), like Cimetidine
  • General anaesthetics or local anaesthetics (Lidocaine)
  • Medications that may increase blood potassium levels, like salt containing potassium and potassium-sparing diuretics
  • Medicines used to treat infections like Trimethoprim, Rifampicin or Terbinafine
  • Medicines known as Diuretics (water pills) are used to treat high blood pressure, such as Furosemide and Hydrochlorothiazide

Interactions with food items

  • Take Met Xl Trio tablet preferably with or immediately following meals, exactly as directed by your doctor.
  • It is advisable to limit foods high in potassium, such as spinach, broccoli, and bananas, as Telmisartan can increase blood potassium levels.

If you experience any symptoms of dizziness, syncope, vision problems, sexual dysfunction, slowing of heart rate, etc., immediately report to your doctor. The doctor can adjust your dose depending on your blood pressure, heart rate, level of this medicine in your serum and other such important factors.
  • Limit the intake of salt in your diet, especially pickles and packaged foods like chips, which contain excess salt and should be avoided
  • Have less potassium-containing foods like spinach, broccoli or banana
  • Limit meat intake in your diet.
  • Instead of having packaged snacks, prefer fresh-cut fruits as they are rich in vitamins and fibre.
If you stop this medicine suddenly, you can suffer from chest pain, irregular heartbeat or a heart attack. Your doctor might tell you to decrease the dose gradually. Don’t suddenly stop taking this medicine without talking to your doctor.
