Medical Description

Globuprime 5 Gm Solution 100 Ml provides passive immunity by increasing a person's antibody levels, and helps prevent or treat certain infectious and immune-related conditions. Globuprime 5 Gm Solution 100 Ml is a highly purified preparation of human immunoglobulins, made from large pools of plasma collected from healthy human donors and processed to reduce the risk of transmitting infections. It works by supplying a broad range of IgG antibodies that reflect the antibody activity found across that donor population. These antibodies raise a person's overall antibody level and their capacity to react with foreign antigens, which helps prevent or modify certain infections in people whose own antibody levels are too low, or who have conditions where immune modulation is helpful. It is used as replacement therapy in primary and secondary immunodeficiency syndromes, to treat idiopathic thrombocytopenic purpura (ITP), Kawasaki syndrome (together with aspirin), Guillain-Barre syndrome and chronic inflammatory demyelinating polyneuropathy (CIDP), to reduce serious bacterial infections in children with HIV-related hypogammaglobulinaemia, and to reduce infections and acute graft-versus-host disease in people undergoing bone marrow transplantation. Globuprime 5 Gm Solution 100 Ml is given by a trained healthcare professional as a slow intravenous infusion, in a hospital or specialist healthcare setting, at the dose your doctor decides. It may cause side effects such as headache, nausea, chills, fatigue, fever, upper abdominal pain, diarrhoea, back pain, excessive sweating and flushing. It can also affect the kidneys and increase the risk of blood clots, and less often cause destruction of red blood cells, a form of brain-lining inflammation called aseptic meningitis, or lung problems. Before starting treatment, your doctor will review your full medical history, including any allergy, IgA deficiency, kidney problems, diabetes, previous blood clots, high blood pressure, heart or cerebrovascular disease, and whether you are pregnant.

Precautions and Warnings

Pregnancy

Pregnancy

It is not known whether Globuprime 5 Gm Solution 100 Ml can harm a developing baby or affect fertility when given to a pregnant woman. There is also a possibility that plasma-derived products such as this one could transmit parvovirus B19, which during pregnancy may cause problems for the baby, including miscarriage, fluid build-up in the baby, or loss of the pregnancy. Talk to your doctor if you are pregnant or planning to become pregnant.

Breast Feeding

Breast Feeding

It is not clear whether Globuprime 5 Gm Solution 100 Ml passes into breast milk. Speak to your doctor about breastfeeding before you start treatment, so that they can advise you.

Driving

Driving

Headache, dizziness and tiredness are commonly reported with Globuprime 5 Gm Solution 100 Ml. Avoid driving or operating machinery if you have any of these symptoms, and speak to your doctor.

Alcohol

Alcohol

Discuss alcohol with your doctor before you start treatment, particularly because Globuprime 5 Gm Solution 100 Ml can affect the kidneys and increase the risk of blood clots.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You have kidney disease or diabetes, or you are dehydrated, as these increase the risk of kidney problems.
  • You have had blood clots before, or you have high blood pressure, cerebrovascular or coronary artery disease, a clotting disorder, a long period of immobility, or you are overweight, as these increase the risk of a clot.
  • You have cryoglobulins, very high triglycerides or a monoclonal gammopathy, which can increase the thickness of the blood.
  • You have IgA deficiency, as you may be at greater risk of a severe allergic reaction.
  • You are 65 years of age or older, as you may be at increased risk of blood clots and sudden kidney failure.
  • You have a history of migraine, as this may increase the chance of aseptic meningitis with this type of medicine.
  • You are pregnant, planning a pregnancy or breastfeeding.
  • You are due to have any vaccination, particularly a live vaccine.

Directions for Use

  • Globuprime 5 Gm Solution 100 Ml is for intravenous use only and is given by a healthcare professional.
  • It is given as a slow infusion into a vein, in a hospital or specialist healthcare setting.
  • The infusion is usually started slowly and the rate increased gradually, which lowers the chance of an infusion reaction.
  • Never attempt to change, delay, skip or repeat a dose on your own.
  • It must not be mixed with any other medicine, or given through the same infusion line as another medicine.
  • It must not be mixed with immunoglobulin products from other manufacturers.
  • Tell the nursing staff straight away if you feel unwell at any point during the infusion.

Storage and disposal

  • Globuprime 5 Gm Solution 100 Ml is a hospital-administered medicine, so the pharmacy normally handles its storage, preparation and disposal.
  • Store it between 2 and 8 degrees Celsius, in its original container, protected from light.
  • Do not freeze it. Do not use it if it has been frozen.
  • Keep it out of the sight and reach of children.
  • Do not use it after the expiry date printed on the pack.
  • Unused product and any waste are disposed of by the hospital or pharmacy, in line with local requirements. Do not dispose of it yourself.

Quick Tips

  • Report to your doctor symptoms of reduced urination, unexplained weight gain, fluid retention or shortness of breath to your doctor straight away, as these can be signs of a kidney problem.
  • Chest pain, sudden weakness on one side of the body, leg swelling or pain, or shortness of breath may signal a blood clot and need immediate medical attention.
  • A severe headache with neck stiffness and fever, or a fast heartbeat with swelling, yellowing of the skin or eyes or dark urine, should be reported without waiting for your next appointment.
  • Tell the healthcare team immediately if you develop severe breathing difficulty, dizziness or fever, particularly within a few hours of your infusion.
  • Tell your doctor about your vaccination history. Live vaccines are usually delayed for 3 months or more after treatment.
  • Keep a note of the brand you are given at each infusion, as immunoglobulin products from different manufacturers are not interchangeable.

Dosage

Overdose

An overdose may lead to fluid overload and thickening of the blood, known as hyperviscosity. Older people and those with kidney or heart problems are at particular risk of these complications. Because Globuprime 5 Gm Solution 100 Ml is always given and monitored in a healthcare setting by trained staff, the chance of an overdose is very low.

Missed a Dose

Globuprime 5 Gm Solution 100 Ml is given as a planned infusion in a hospital or clinic. If you miss a planned infusion or need to reschedule it, contact your doctor as soon as possible so that the appointment can be rearranged. Do not try to make up for a missed infusion on your own.

Interactions

Interactions with other medicines

  • Globuprime 5 Gm Solution 100 Ml must not be mixed with other medicines, or given through the same infusion line as any other intravenous preparation, and must not be mixed with immunoglobulin products from other manufacturers.
  • The antibodies in Globuprime 5 Gm Solution 100 Ml may reduce the response to live virus vaccines, including the measles, mumps and rubella (MMR) vaccine and the varicella vaccine. Live vaccines are generally delayed for 3 months or more after treatment. For the measles vaccine this effect can last up to a year, so your doctor may check your antibody status.
  • The antibodies passed on through Globuprime 5 Gm Solution 100 Ml can interfere with the results of certain blood tests, so tell anyone taking a sample that you are on this treatment.
  • Tell your doctor about all prescription medicines, over-the-counter medicines, supplements and herbal products you take, before and during treatment.

Interactions with food items

  • Globuprime 5 Gm Solution 100 Ml is given into a vein by a healthcare professional, so interactions with food do not apply.

Frequently Asked Questions (FAQs)

Contact your doctor immediately if you notice reduced urine output, unexplained weight gain or swelling; chest pain, sudden weakness on one side of the body, leg pain or swelling, or shortness of breath; a severe headache with neck stiffness and fever; a fast heartbeat, yellowing of the skin or eyes, or dark urine; or hives, breathing difficulty or dizziness during or shortly after your infusion.
Starting at a slow rate and building up reduces the risk of infusion-related reactions, which are more likely if the medicine is given too quickly. This matters most during your first infusion, and whenever you are switched between different immunoglobulin products.
Not without your doctor deciding so. Immunoglobulin products from different manufacturers are prepared differently and are not interchangeable, and they must never be mixed together. If you are switched from one product to another, your infusion may be restarted at a slower rate and you will be watched more closely.
It can. The antibodies in this medicine may reduce how well live virus vaccines work, including the MMR and varicella vaccines. These are usually delayed for 3 months or more after treatment, and for the measles vaccine the effect may last up to a year, so your doctor may check your antibody levels first. Tell your doctor about any vaccination you are due.
Yes, it is made from plasma pooled from large numbers of healthy human donors. Donors are screened, and the plasma is tested and put through steps designed to remove or inactivate viruses, so the risk of passing on an infection is very low. It cannot be reduced to zero, which is why your doctor weighs the benefit against that small risk before starting treatment.
