Medical Description

Gammaven Injection is a highly purified preparation of human Immunoglobulin G (IgG), made from large pools of plasma collected from healthy human donors and treated with a solvent-detergent process. It provides passive immunity by increasing a person's antibody levels, and is used to prevent or treat certain infectious and immune-related conditions. Gammaven Injection is used as replacement therapy in primary and secondary immunodeficiency syndromes, to treat idiopathic thrombocytopenic purpura (ITP), Kawasaki syndrome, Guillain-Barre syndrome, chronic inflammatory demyelinating polyneuropathy (CIDP) and multifocal motor neuropathy (MMN), to reduce serious bacterial infections in children with HIV-related hypogammaglobulinaemia, and to reduce infections and graft-versus-host disease in people undergoing bone marrow transplantation. It works by supplying a broad range of IgG antibodies collected from many plasma donors, reflecting the antibody activity found across that donor population. These antibodies increase a person's overall antibody level and their capacity to react with foreign antigens, which helps prevent or modify certain infections in people whose own antibody levels are too low, or who have conditions where immune modulation is helpful. Gammaven Injection is given by a trained healthcare provider as a slow intravenous (IV) infusion, in a hospital or specialist healthcare setting, as recommended by your doctor. Gammaven Injection may cause side effects like headache, nausea, chills, fatigue, fever, upper abdominal pain, diarrhoea, back pain, excessive sweating and flushing. It can also affect the kidneys, increase the risk of blood clots and, in a few cases, cause destruction of red blood cells (haemolysis), a rare brain inflammation called aseptic meningitis, and lung problems. Warning signs that need to be reported to your doctor and require urgent medical attention include reduced urination, unexplained weight gain, fluid retention or shortness of breath (possible kidney problem); chest pain, sudden weakness on one side of the body, leg swelling or pain, or shortness of breath (signs of a blood clot); severe headache, neck stiffness, drowsiness, fever and sensitivity to light (signs of possible aseptic meningitis); a fast heartbeat, swelling, difficulty breathing, yellowing of the skin or eyes, or dark urine (possible haemolysis); and hives, tightness in the chest, dizziness, or a sudden drop or rise in blood pressure during the infusion (possible severe allergic or infusion reaction). Before starting Gammaven Injection, have your doctor review your complete medical history, including any allergy, IgA deficiency, kidney problems, diabetes, a history of blood clots, high blood pressure, heart or cerebrovascular disease, and pregnancy status.

Precautions and Warnings

Pregnancy

Pregnancy

It is not known whether Gammaven Injection can harm a developing baby or affect fertility when given to a pregnant woman. There is a possibility that plasma-derived products such as Gammaven Injection could transmit parvovirus B19, which, if it occurs during pregnancy, may cause fetal problems such as miscarriage, fetal hydrops or fetal death. Talk to your doctor if you are pregnant or planning to become pregnant.

Breast Feeding

Breast Feeding

It is not known whether Gammaven Injection passes into breast milk. Consult your doctor about breastfeeding while using Gammaven Injection.

Driving

Driving

Headache, dizziness and fatigue are commonly reported with Gammaven Injection. Avoid driving or operating machinery if you have any of these symptoms, and consult your doctor.

Alcohol

Alcohol

It is best to discuss alcohol use with your doctor, particularly because Gammaven Injection can affect the kidneys and increase the risk of blood clots.

Other General Warnings

Other General Warnings

Talk to your doctor if
  • You have kidney disease or diabetes, or are dehydrated, as these increase the risk of kidney problems.
  • You have a personal history of blood clots, high blood pressure, cerebrovascular or coronary artery disease, a clotting disorder or prolonged immobility, or you are overweight, as these increase the risk of thrombosis.
  • You have cryoglobulins, very high triglycerides or a monoclonal gammopathy, which can increase the thickness of the blood.
  • You have IgA deficiency, as you may be at greater risk of a severe allergic reaction.
  • You are 65 years of age or older, as you may be at increased risk of blood clots and acute kidney failure.
  • You have a history of migraine, as this may increase the likelihood of aseptic meningitis with this type of medicine.

Directions for Use

  • Gammaven Injection is for intravenous use only and is given by a healthcare professional.
  • Never attempt to change, delay, skip or repeat a dose on your own.
  • Gammaven Injection must not be mixed with any other medicine or given through the same infusion line as other medicines, and must not be mixed with immunoglobulin products from other manufacturers.

Storage and disposal

  • Gammaven Injection is a hospital-administered medicine, so the pharmacy normally handles its storage, preparation and disposal.
  • Store it between 2 degrees C and 8 degrees C in its original container, protected from light. Do not freeze.

Quick Tips

  • Report decreased urination, weight gain, fluid retention or shortness of breath to your doctor right away, as these may be signs of a kidney problem.
  • Chest pain, sudden weakness on one side of the body, leg swelling or pain, or shortness of breath may signal a blood clot and need immediate medical attention.
  • Report a severe headache with neck stiffness and fever, a fast heartbeat, swelling, yellowing of the skin or eyes, or dark urine to your doctor without delay.
  • Inform your healthcare team immediately if you develop severe breathing difficulty, dizziness or fever, particularly within a few hours of your infusion.
  • Tell your doctor about your vaccination history, since live vaccines are usually delayed for three months or more after treatment.

Dosage

Overdose

Overdose may lead to fluid overload and increased thickness of the blood (hyperviscosity). People at particular risk of these complications include elderly patients and those with kidney or heart impairment. Because Gammaven Injection is always administered and monitored in a healthcare setting by trained staff, the chance of overdose is very low.

Missed a Dose

If you miss a planned infusion or need to reschedule it, contact your doctor as soon as possible.

Interactions

Interactions with other medicines

  • Gammaven Injection must not be mixed with other medicines or given through the same infusion line as other intravenous preparations, and must not be mixed with immunoglobulin products from other manufacturers.
  • Antibodies in Gammaven Injection may interfere with the response to live virus vaccines, including measles, mumps and rubella (MMR) and varicella vaccines. Live vaccinations are generally delayed for three or more months after Gammaven Injection; for the measles vaccine this effect may last up to a year, so your doctor may check your antibody status.
  • Antibodies transferred through Gammaven Injection can interfere with the results of certain blood (serological) tests.
  • Tell your doctor about all prescription medicines, over-the-counter medicines, supplements and herbal products you are taking before and during treatment with Gammaven Injection.

Frequently Asked Questions (FAQs)

Contact your doctor immediately if you notice reduced urine output, unexplained weight gain or swelling; chest pain, sudden weakness on one side of the body, leg pain or swelling, or shortness of breath; a severe headache with neck stiffness and fever; a fast heartbeat, yellowing of the skin or eyes, or dark urine; or hives, breathing difficulty or dizziness during or shortly after your infusion.
Starting at a slow rate and increasing it gradually reduces the risk of infusion-related reactions, which are more likely if the medicine is given too quickly, especially during a first infusion or when switching between immunoglobulin products.
Tell your doctor or vaccination provider that you are receiving Gammaven Injection. The antibodies in it can blunt the response to live virus vaccines such as MMR and varicella, so these are usually delayed for three months or more after treatment. Your doctor will advise on the right timing for you.
