Medical Description
Dotidep is an antidepressant and a prescription medication. It is used to treat symptoms of Major Depressive Disorder with associated anxiety. The active component of Dotidep is Dosulepin.
Depression is a medical condition associated with low mood, hopelessness, difficulty sleeping, and tiredness. It occurs due to altered levels of chemical messengers in the brain such as serotonin, norepinephrine and dopamine. Anxiety is a condition characterised by increased worry and fear about routine situations. Anxiety can present with increased heart rate, sweating, restlessness, trembling, increased rate of breathing etc. Panic disorder is also a type of anxiety disorder with a feeling of loss of control and fear.
Dosulepin belongs to a class of medications called tricyclic anti-depressants. The medication should be taken in the dose and duration as prescribed by the doctor.
Uses
- Treatment of concomitant Major Depressive Disorder with Anxiety
Contraindications
If you have hypersensitivity reactions to these medications/inactive ingredients.
If you have a recent history of heart conditions such as irregular heart rhythm or myocardial infarction (heart attack).
If you have angle-closure glaucoma, prostate enlargement and urinary retention.
If you have severe liver disease or known seizure disorders.
If you have a history of mania or bipolar disorder.
If you fall in the paediatric population (<18 years of age).
TCAs have a black-box warning regarding increase in suicidal ideation and behaviour when used in children, adolescents, or young adults less than 24 years as an anti-depressant.

Talk to your doctor if
- You witness any discomfort or sudden mood changes.
- You should inform your doctor about any other medications you are taking. Dotidep interacts with many other medications metabolised in the body using the same enzymes.
Directions for Use
- Dotidep should be taken as prescribed by a doctor only.
- It is recommended to take this medication at the same time every day.
- Swallow this medication as a whole. Do not crush, chew or break this tablet.
- Finish the full course of this medication as prescribed. Stopping Dotidep suddenly can cause withdrawal side effects. Consult your doctor before stopping.
- This medication may require 2-4 weeks for onset of action and up to 6 weeks for maximum efficacy. Do not increase the dose without explicit advice from your treating physician.
Storage and disposal
Store in a cool and dry place away from direct light and moisture. Don’t use the medicine after the date of expiration. Keep the tablets away from children and pets. Do not flush the medication down the toilet. Dispose of it with biomedical waste only.

Overdose
Dosulepin toxicity can be severe and fatal. In case of an overdose, the patient may present with sedation, increased heart rate, low blood pressure, dilated pupils, dry mouth, and difficulty in passing urine or stools, abnormal heart rhythms, dilated pupils, dry mouth, low urine output, seizure, coma, shock. In case of an overdose, immediately rush the patient to the nearest tertiary care hospital for emergency management as it is life-threatening.
Missed a Dose
If you miss a dose of this tablet, it’s advised to take it as soon as possible. If the duration missed is too long, and it’s almost time for the next dose, it’s recommended to skip the missed dose and go back to the regular schedule. Don’t double the dose of the tablet.
Mode of Action
How Does It Work?
Dosulepin is a tricyclic anti-depressant. This medication acts on multiple brain pathways, especially those involving the chemical messengers Nor-epinephrine and Serotonin. It prevents a reuptake process, wherein the nerve cells reabsorb the chemicals. The prevention of re-uptake results in an increase in the accumulation of Nor-Epinephrine and Serotonin. This helps relieve the symptoms of depression in the user, as these chemicals affect the mood, cognition, memory, and personality. In addition, Dosulepin also affects other pathways which contribute to its effects.
Interactions
Interactions with other medicines
- Dotidep should not be used concomitantly with MAOIs (Monoamine oxidase inhibitors) or within 14 days of the use of MAOIs and vice versa. Examples of MAOIs include Selegiline, Rasagiline.
- Dotidep should not be used with medicines like tramadol, methadone, or linezolid, as it may result in extreme elevation of serotonin levels.
- Dotidep should not be used with medicines that can lower the seizure threshold.
- Dotidep should not be used with alcohol or other central nervous system depressants.
- Dotidep should not be mixed with opioid or benzodiazepines medications.
- Dotidep affects the heart rhythm and should not be used in addition to other medications that also have the same effect, as it may result in potentially life-threatening conditions. Examples include astemizole, cisapride, etc.
- Dotidep should not be used with BP medicines (anti-hypertensives) as it can increase their effect, resulting in severe lowering of BP.
- Dotidep is contraindicated in patients with hypothyroidism on treatment with Thyroxine, as this can result in abnormal heart rhythms.
