Medical Description

Asparawem powder for injection is an anticancer medicine belonging to a class of enzyme-based chemotherapy agents. It contains the L-asparaginase enzyme crisantaspase derived from the bacterium Erwinia chrysanthemi.

It is used along with other chemotherapy medicines to manage patients with acute lymphoblastic leukemia who have developed hypersensitivity to, or silent inactivation of, an E. coli-derived asparaginase.

It works by reducing asparagine (an amino acid) levels in the bloodstream. Normal cells can make their own asparagine, but certain leukaemic lymphoblasts depend more heavily on circulating asparagine. Reducing this amino acid in the blood stops protein synthesis in these cancer cells and leads to their death.

Common side effects of Asparawem powder for injection include nausea, vomiting, decreased appetite, raised blood sugar levels, blood clotting abnormalities and hypersensitivity reactions.

Asparawem powder for injection is given through injection by trained nurses or doctors in a hospital or clinic setting. You should attend all the tests that your doctor recommends during and after Asparawem powder for injection treatment, including blood tests, liver function, blood glucose, clotting tests and pancreatic enzymes (amylase and lipase).

Precautions and Warnings

Pregnancy

Pregnancy

Asparawem powder for injection should generally be avoided during pregnancy unless your doctor determines that the benefits outweigh the potential risks. Women who can become pregnant should use effective contraception during treatment and for the period advised by their doctor.

Breast Feeding

Breast Feeding

It is not known whether Asparawem powder for injection passes into breast milk. Because a risk to the baby cannot be ruled out, breastfeeding is not recommended during treatment. Discuss with your doctor when you can safely resume breastfeeding.

Alcohol

Alcohol

There is no specific established alcohol interaction with crisantaspase. However, alcohol may worsen nausea or other treatment-related symptoms and may add to liver toxicity, particularly when other hepatotoxic medicines are being used. Discuss alcohol consumption with your doctor.

Driving

Driving

Asparawem powder for injection does not usually directly affect driving ability. However, it may cause dizziness, tiredness, nausea, vomiting, or other symptoms that can affect alertness. Do not drive or operate machinery if you feel unwell or unable to do so safely.

How does this medicine work?

Asparawem powder for injection lowers the level of asparagine, an amino acid, in the blood. Some leukaemia cells cannot make enough asparagine on their own and depend on it from the blood.By reducing the available asparagine, Asparawem powder for injection stops these cancer cells from making proteins they need to grow and survive, causing them to die.

Interactions

Interactions with other medicines

  • Methotrexate or cytarabine: The order in which these medicines are given with Asparawem powder for injection can affect how well they work. Follow your prescribed chemotherapy schedule.
  • Prednisone: Using it with Asparawem powder for injection may increase the risk of blood-clotting problems and other side effects.
  • Vincristine: Careful scheduling is needed because giving Asparawem powder for injection first may increase vincristine-related side effects.
  • Imatinib: Using it with Asparawem powder for injection may increase the risk of liver problems, so monitoring may be needed.
  • Live vaccines: These should generally be avoided during treatment because your immune system may be weakened.
  • Asparawem powder for injection should not be mixed with other medicines in the same syringe or infusion unless compatibility has been confirmed.
  • Tell your doctor about all medicines, vitamins, supplements, and herbal products you take, especially those affecting blood sugar or blood clotting.

Frequently Asked Questions (FAQs)

Asparawem powder for injection can reduce the liver's production of certain clotting proteins and alter other components of the clotting system, which can increase the risk of both bleeding and blood clots. It can also raise blood sugar. Regular blood tests let your doctor monitor and identify symptoms early and manage these effectively by adjusting medicine doses.
Severe, persistent abdominal pain that radiates to the back, along with nausea and vomiting, needs immediate medical attention, as these are signs of pancreatitis.
Yes, hypersensitivity reactions can occur at any point during treatment, even after earlier doses were well tolerated. Healthcare facilities should have appropriate emergency treatment available during administration because severe allergic reactions, including anaphylaxis, can occur.
