Medical Description
Herlapsa tablet is a targeted anti-cancer medicine that belongs to a class of drugs called kinase inhibitors. It is used with other chemotherapy medicines in the management of metastatic breast cancer whose tumour cells show excess amounts of a protein called HER2 and that have already received other cancer treatments that have stopped working or are no longer suitable.
Herlapsa tablet works against HER2 (human epidermal growth factor receptor) and EGFR (epidermal growth factor receptor), blocking signalling pathways involved in the growth and proliferation of cancer cells.
Herlapsa tablet may reduce heart function. Your doctor may check heart function with an echocardiogram or MUGA scan before and during treatment. Report breathlessness, unusual tiredness, leg swelling, or palpitations immediately
Take Herlapsa tablet on an empty stomach, at least 1 hour before or 1 hour after food, exactly as prescribed. Do not change the dose yourself. Severe or persistent diarrhoea requires prompt medical attention due to the risk of dehydration. Contact your doctor immediately if it occurs to ensure early management.
Common side effects include nausea, vomiting, fatigue, mouth sores, reduced appetite, indigestion, rash, and hand-foot syndrome. Seek medical attention for breathing problems, heart rhythm changes, or severe blood-count abnormalities. Your doctor may monitor blood counts, liver function, heart function, and ECG as appropriate.
Tell your doctor about any heart or liver problems and all medicines, supplements, and herbal products you take before starting the treatment. Avoid grapefruit and grapefruit juice. Women who can become pregnant should use effective contraception during treatment, and breastfeeding should be discussed with your doctor.

- You have a history of heart disease, including heart failure, irregular heartbeat, or a previous heart attack, as Herlapsa tablet can affect heart function.
- You have or have had liver problems.
- You develop signs of infection such as fever, chills, or a sore throat.
- You notice unusual bruising, bleeding, or blood in your urine or stool.
Directions for Use
- Take Herlapsa tablet exactly as prescribed by your doctor.
- Take Herlapsa tablet by mouth at least one hour before or at least one hour after a meal. Do not take it with food.
- Swallow the tablets whole with water. Do not divide, crush, or split your daily dose.
- Avoid grapefruit, grapefruit juice, and other grapefruit-containing products throughout treatment.
- Your doctor may pause, reduce, or adjust your dose depending on side effects, heart function test results, liver function, or other medicines you are taking. Never change, stop, increase, or restart your dose without first speaking to your doctor.
Storage and disposal
- Store Herlapsa tablet tablets at room temperature in the original container with the lid tightly closed.
- Protect the tablets from excess moisture.
- Keep Herlapsa tablet and all medicines out of the reach of children and pets.
- Do not use tablets that are past their expiry date or that appear damaged or discoloured.
Quick Tips
- Take Herlapsa tablet once daily at the same time, always on an empty stomach (at least one hour before or after food).
- Do not chew or crush the tablets.
- Avoid grapefruit and grapefruit juice throughout treatment.
- Attend all scheduled heart function tests and blood tests.
- Contact your doctor immediately if you develop breathlessness, palpitations, persistent cough, or severe diarrhoea.
- Use effective contraception during treatment.
- Tell your doctor about every other medicine, supplement, or herbal product you take, including new ones started during treatment.

Interactions
Interactions with other medicines
- QT-prolonging medicines: May increase the risk of abnormal heart rhythms when used with Herlapsa tablet.
- Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin, and some HIV medicines) may increase Herlapsa tablet levels and side effects.
- Strong CYP3A4 inducers (e.g., carbamazepine, phenytoin, rifampicin, and St. John’s Wort) may reduce Herlapsa tablet levels and effectiveness.
- Grapefruit and grapefruit juice: Avoid, as they may increase Herlapsa tablet levels.
- Herlapsa tablet may affect levels of other medicines metabolised by CYP3A4/CYP2C8 or transported by certain drug transport proteins.( eg statins, calcium channel blockers, and antidepressants)
- Tell your doctor about all prescription and OTC medicines, vitamins, supplements, and herbal products you take before starting treatment.
